An adult life jacket must be provided for each person carried on board a vessel. Six adult life jackets found to be unserviceable. Repair or replace life jackets. Vessel is restricted to 34 total persons on board. 46 CFR 180.71(a)

The master of the vessel shall ensure that the EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Battery was expired. Replace battery or EPIRB. Vessel operation is restricted to within 3 nm. 46 CFR 185.728(b)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. Operating station portable fire extinguisher stored in a location not and not mounted at the operating station as required by Table 181.500(b). Mount at operating station. 46 CFR 181.520

A vessel must comply with the applicable requirements for any radio requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR Part 80. Radio microphone not located at the primary operating station as per Title 47 Subpart S 80.905(d) A VHF-DSC radiotelephone installation or a remote unit must be located at each steering station except those auxiliary steering stations which are used only during brief periods for docking or for close-in maneuvering. A single portable VHF-DSC radiotelephone set meets the requirements of this paragraph if adequate permanent mounting arrangements with suitable power provision and antenna feed are installed at each operator steering station. 46 CFR 184.502

Except as otherwise provided in this section every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Missing one portable light. Provide. 46 CFR 183.430

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables. Missing current tables. Provide. 46 CFR 184.420

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Missing emergency broadcast instructions placard at radiotelephone installation. Provide. 46 CFR 184.506

A vessel must comply with the applicable design, equipment, personnel, procedures, and record requirements of 33 CFR parts 151. Missing garbage placard IAW 33 CFR 151.59. Provide. 46 CFR 184.702

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Missing the above requirement. Provide the above. In lieu of the valve direction, add "Pull fuel supply line quick disconnect fitting to disconnect from portable tank." 46 CFR 185.608

Battery chargers must have an ammeter connected in the charging circuit. Provide ammeter. 46 CFR 183.350(f)

Twist on type connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Several wires in port aft compartment with twist on type connectors. Make connection with pressure/crimp type wire connector. 46 CFR 183.340(i)(2)

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. Exposed electrical wire strands in port aft compartment. Resolve to the satisfaction of the attending MI. 46 CFR 183.200(c)

An EAP training program must be conducted for the employer's crew members and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crew members and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Crew members and supervisory personnel never received the above required training. Provide a roster signed by crew members and supervisory personnel. Also, provide training material used to conduct the training to the attending MI. 46 CFR 16.401(b)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (3) A space with a non-watertight closure. The port high water level alarm is inoperable. Prove proper operation of the high water level alarm to the satisfaction of the Marine Inspector.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from electrical shock of personnel. Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Forward railing stanchions on port and starboard side of the vessel were found to be loose. Investigate wasted foundation. Provide repair plan to repair railing stanchion back to original strength.

A vessel must comply with the applicable requirements for any radio and EPIRB installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR Part 80; Vessel did not have FCC Station license. Prove validity of station license.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice; Vessel's preventative maintenance plan (PMP) did not have inventory of rigging components to include: the age of component, size, type of material, and a replacement schedule for all rig components based on inspection results, service history, and manufacturer or designer recommendations. Add inventory of rigging components to PMP iaw NVIC 02-16.

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline on a ring life buoy must: Be of at least 18.3 meters (60 feet) in length; Lifeline on vessel's ring life buoy was 50 feet. Attach lifeline that is at least 60 feet in length.

On a vessel of at least 26 feet in length, a visible and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces; vessel only had one visible and audible alarm and is unable to indicate which space has the high water level. Add separate visible and audible indicator to determine the space that has the high water level.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock; Weight of vessel shore tie is too heavy and is removing the plug from it's connection to vessel batteries. Add support to support weight of shore tie cable.

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended; the port rudder bolt assembly is not sized appropriately. Replace assembly with appropriately sized components.

Replace 2 unserviceable lifejackets. Passenger count is reduced from 37 to 35 until corrected.
