Piping, valves, and fittings in a fire main system must comply with subpart G, part 182, of this chapter. A pin hole leak was identified in the fire main system aft of the inboard MDE at the first vertical section of piping. Marked with blue sharpie. 46 CFR 181.310(b)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. Forward engine room escape hatches were not properly marked on both sides of the emergency exit. 46 CFR 185.606

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Floating waterlight was found inoperable. Vessel is restricted to daylight hours only until floating waterlight is fixed or replaced. 46 CFR 180.70(d)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible;(2) U.S. Coast Pilot or similar publication;(3) Coast Guard Light List; (4) Tide tables; and(5) Current tables. Vessel did not have updated electronic navigational charts or publications. 46 CFR 184.420

When installed in a machinery space, paint locker, a space containing flammable liquid stores, or a space with a fuel tank, a fixed carbon dioxide system must meet the following requirements.(i) The quantity of carbon dioxide in kilograms (pounds) that the system must be capable of providing to a space must not be less than the gross volume of the space divided by the appropriate factor given in Table 181.410(f)(4)(i). If fuel can drain from a space being protected to an adjacent space or if the spaces are not entirely separate, the volume of both spaces must be used to determine the quantity of carbon dioxide to be provided. The carbon dioxide must be arranged to discharge into both such spaces simultaneously. Engine room port side forward bulkhead has multiple holes from missing hardware rendering the space not vapor tight exceeding the CO2 volume available Make the port engine room vapor tight. 46 CFR 181.410 (f)(4)

An adult life jacket must be provided for each person carried on board a vessel. Vessel only equipped with 142 serviceable Life jackets. Vessel is restricted to 142 total persons on board. Vessel requires additional 14 life jackets to meet total allow persons on board. 46 CFR 180.71 46 CFR 180.71

Except on a vessel limited to daytime operations, the compass must be illuminated. Magnetic compass does not have illumination. Repair or replace magnetic compass light. Vessel is restricted to daylight operation until repair or replacement to the satisfaction of the attending marine inspector. 46 CFR 184.402 (c)

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. Fire pump pressure gauge is indicating less than 65 psi at the discharge side. Ensure fire pump reaches 65 psi no latter than 30SEP2024.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. 24 volt connections in port and stbd engine rooms is not within an enclosure. 46CFR183.340

On a vessel of more than 65' in length, the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. The engine-driven fire pump only achieved 30 psi while testing the upper fire-hose station. Prove proper operation to the attending CG Marine Inspector (MI).

On a vessel of at least 26' in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The starboard jet room high level alarm only intermittently produced a visual and audible alarm. Restore functionality of specified high level alarm and prove proper operation to the attending CG MI.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure and personnel injury. Sea suction hose for the port, aft engine was chafed in multiple locations. Replace hose.

A hatch exposed to the weather must be watertight. The glue for the gasket on the port jet room watertight hatch failed causing a lack of watertight integrity. Restore gasket.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. Inverter for the public address emergency source of power was producing an unknown alarm and squeal. Identify alarm and restore inverter to normal operating condition.

The master of the vessel shall ensure that each EPIRB is tested monthly. The EPIRB test shall be logged or otherwise documented. Vessel's logbook for the 2023 operating season was missing EPIRB tests for the months of April, June and July. Provide training to masters to ensure proper log keeping.

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. Aft MSD holding tank on board had pinholes and corrosion compromising the integrity of the tank. In addition, the discharge connection on the port side of the holding tank was compromised. Several drops were observed accumulating around the connection. Restore tank and connections to original condition prior to 2024 operating season.

Watertight integrity. (a) A hatch exposed to the weather must be watertight... The flush deck hatch for the starboard side forward peak void does not dog down watertight. Properly repair quick closing hatch cover so it will dog down watertight. 46 CFR 179.360

Lighting fixtures. (a) Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Light fixture in the starboard side generator void the 110v light fixture is missing the globe. Properly repair light fixture. 46 CFR 183.410

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the of the hose. Marine Inspector observed the port side bilge pump suction hose had one hose clamp at the end of hose near pump. Install second hose clamp on bilge suction hose.

Each deck where survival craft or rescue boats are stowed must be kept clear of obstructions that would interfere with the boarding and launching of the survival craft or rescue boat. Marine inspector observed a gangway stored on the bow blocking access to the life rafts. Restow gangway and securely stow gangway so that it does not impede the launching of primary lifesaving equipment.

The use of wire nuts is permitted under the following conditions: Connections must be made within an enclosure and the insulated cap must be secured to prevent loosening due to vibration. #3 port side void water pump has wire nuts installed. Place spliced wires in an enclosure. 46 CFR 183.340(i)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The flame screens or flame arresters must be of such size and design as to prevent reduction in the net cross sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Port side fuel tank vent was missing the required flame screen. 46 CFR 182.450(e)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Bilge high level alarm connections are not made in an enclosure. 46 CFR 183.340(g)

While testing both port and starboard engine room CO2 system time delays, delays failed to last the design time of delay. Repair or replace time delays and provide service report.

Bilge pumping system inoperable. Prove proper operation of bilge pumping system.

No fire service report on board vessel. Provide service report for annual CO2 system servicing.

Provide fixed fire extinguishing system for oil fire furnace on upper deck. Vessel restricted from using furnace until cleared by attending marine inspector.

Number 2 & 4 MDE missing exhaust lagging. Properly install exhaust piping lagging on all hot sections of exhaust piping.

Number 2 MDE has excessive oi8l leak from manifold behind oil filters. Make permanent repairs to stop leaking.

Conduct fire and man overboard drills as well as sea trials with attending CG Marine Inspector.

Repair engine guard on front of Starboard aft main engine

Cover exposited exhaust piping on starboard aft main engine to prevent contact of hot piping

Port aft main engine overheated and siezed. Repair and prove proper operation prior to conducting passenger operations.

Install Carbon Dioxide gas storage warning signage as per 46 CFR 185.612 (g)

Prove that fire pump produces 60 psi at pump outlet

Provide Alcohol testing equipment onboard for each crew member onboard

Provide a horn that produces the required dB and audibility range listed in 33 CFR 86

Repair electrical outlet on starboard side aft by bar.

Replaced chaffed battery wire coming off of the #2 engine in the port engine room

Install batteries in a battery tray or box that are resistant to the damage from electrolyte

Provide updates for paper charts onboard

Replace all expired items in all first aid kits onboard

Prove proper operation of smoke detector in the center head

Replace reflective tape on port aft main deck life bouy

Properly mark the number and size of life jackets being storage in lockers

Provide donning and using instructions for all life jackets onboard

Vessel must conduct and log drills every month if reasonable , but at least once within a 3 month period before getting underway with passengers

Fix oil leak on #2 engine inboard side

Fix weather tight gasket on both engine room hatches

Fix exhaust leak on #2 and #3 main engines

Fix exhaust cooling water leak on the #2 Main engine

Fix exhaust lagging on #1 and #3 main engine to protect all personal from hot surfaces

Fix or reinstall guard over all rotating equipment fuel oil pump guard is missing on all but one main engine and both port engines are miss or have damaged fly wheel guards

Durning next dry dock repair the 10 cracked welds in the Starboard #1 Void 3 are in the area of the aft trans. Frame at the deck on the aft side and 3 on the fwd side of the same frame and 4 cracked welds at deck fwd must frame

Prove proper operation of the port side bilge pump while being used cross connected to pump the starboard void

ID'd ~10 cracked welds attaching stiffeners to plating.

Replace EPIRB due to expired battery

Didn’t get underway or conducted drills at this time issued CG-835 to conduct drills with CG inspector prior to carrying passengers.

Engines and power ventilation were not operating at the time of the test. Demonstrate operation of the engine and power ventilation shutdowns to the attending CG Marine Inspector. 46 CFR 181.410(b)(10) Correct at next Annual inspection.

Effect repairs to the #4 engine and demonstrate full functionality of the fire main to the satisfaction of the [name withheld] 46CFR181.300 prior to carrying passengers.

Penetrations of required watertight bulkheads by rigid plastic pipes are prohibited unless the requirements of the reference are met. Provide a metallic valve adjacent to the penetration on both sides of the bulkhead or in some other manner in conformity with regulations.

Provide fixed fire extinguishing for oil fired heating and ventilation unit on aft second deck. As an alternative space may be ventilated such as same space for vessels built for NEW YORK WATERWAYS and granted a waiver due to "Open Space"

Mark location and quantity of child sized life jackets stored under the stairs.

Ensure that fire detection system is electronically supervised.

Found Port Aft engine Seawater Heatexchanger partly blocked from grounding. Required cleaning prior to returning to service.

The M/V ST. NICHOLAS is prohibited from carrying passengers until a proper damage survey is completed to the satisfaction of the attending marine inspector.

Ensure that hull is examined by end of July with careful attention to damaged areas resulting from May 2006 grounding including scraps noted on port and starboard hull from the keel up 1.5 feet on each side of keel.

MSD Tank mounts previously cut to allow tank to be slid side to side for hull repair were not rewelded. Repair tank mounts.

Provide clamp for shore discharge hose where missing at attachment to hull through fitting.

Provide double continuous welding for collision bulkhead as per plans for construction in accordance with DNV High Speed Craft Code.

18" of keel on port side; bow approx. 2' forward of collision bulkead; sheared away during grounding. No damage visible internally. Make permanent repairs to the keel.

Replace out of date Ni-cad battery with Dry Cell appropriate for floating water light

Furnace must not be used until documentation & proof is provided to nearest [name withheld] tag secured & fuel line sealed closed. LEGACY COMPONENT DESC: Burners

Vessel electric cable runs missing metallic support as required by Subchapter T. Install metallic supports IAW subchapter T.

Demonstrate that jet drive shaft penetrations in the aft port and starboardengine room bulkheads are watertight. Plans shall be submitted to the CoastGuard prior to making any modifications to the existing jet drive bulkhead penetrations.

Secure cover to exposed electrical connections on bulkhead aft of galley near heads. This must be locked or screwed shut to prevent unathorized access prior to operation with passengers.

Install portable fire extinguisher outside furnace room door.

INSTALL ADEQUATE HANDRAILS ON TOP OF THE STAIRS ON THE SECOND DECK, INSURING ADEQUATE REQUIRED SPACE IS MAINTAINED. THIS MUST BE CORRECTED PRIOR TO OPERATION WITH PASSENGERS.

CORRECT INADEQUATE SPACE IN RAIL ON 3RD DECK, TOP OF STAIRS, PORT SIDE, PRIOR TO OPERATION WITH PASSENGERS.

INSTALL MISSING COVER TO FLYWHEEL ON STARBOARD FWD ENGINE.

SECURE LEAK IN FUEL RETURN LINE AT BULKHEAD IN PORT E/R/ THIS MUST BE COMPLETED PRIOR TO OPERATION WITH PASSENGERS.

Place guards over intake side of fans under seats and secure the wireing atthose locations. These include the 3 fwd/port and 1 fwd/stbd fans on the main deck. This must be completed prior to operation with passengers.

Mark or secure furnace room door to prevent unauthorized entry to this space.
