A power-driven vessel underway shall exhibit: Sidelights. MI identified loose wiring on starboard sidelight. 33 CFR 83.23(iii)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. MI identified loose ladder in space under galley. 46 CFR 176.830

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. MI identified no fire servicing record available. 46 CFR 176.810(b)

A proper certificate issued by the Federal Communications Commission shall be regarded as evidence that the radio installation, if any, is in compliance with the requirements of that agency. Observed that the FCC Communications Safety Radio Telephony Certificate was expired. Replace the expired certificate with a valid certificate to satisfaction of the MI. (46 CFR 184.25-1(b)) [OLD T]

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead-end wires found under helm console in bridge space. Remove dead-end wires from the space to satisfaction of the MI. (46 CFR 183.200)

Connections must be made to battery terminals with permanent type connectors. Observed loose (red) wire connection on battery terminal in the engine room's port side, under "BOW" and "GALLEY" plate markings. Rectify loose connection to satisfaction of the MI. (46 CFR 183.350(d))

Each ring life buoy on a vessel must be readily accessible. Observed life ring had minor tears, cracks, and missing a reflective tape on one corner. Repair or replace to the satisfaction of the MI. (46 CFR 180.70(b)(2))

The water light must be: Attached to the life float or buoyant apparatus by a 12-thread manila or equivalent synthetic lanyard, at least 18 feet in length. Observed that the top lifefloat's water light line was brittle and sun-damaged. Rectify to satisfaction of the MI. (46 CFR 180.15-5 (d)(ii) [OLD T])

A proper certificate issued by the Federal Communications Commission shall be regarded as evidence that the radio installation, if any, is in compliance with the requirements of that agency. Observed that the FCC Radio Station Authorization was expired. Deficiency was rectified on the spot. (46 CFR 184.25-1(b) (OLD T))

A vessel must carry one floating waterlight. Observed waterlight's battery was expired. Deficiency was rectified on the spot. (46 CFR 180.70 (d))

Paddles shall be lashed on the sides of the life floats to which they belong. Observed paddles weren't attached to life floats. Deficiency was rectified on the spot. (46 CFR 180.15-10 (a) [OLD T])

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light. Each life jacket light must be securely attached to the front shoulder area of the life jacket. Observed missing light on a child's life jacket. Deficiency was rectified on the spot. (46 CFR 180.75(a))

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Observed switchboard at operating station missing one of its pieces. Deficiency was rectified on the spot. (46 CFR 183.200)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2.5 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. Observed vessel had buckets that were larger than 2.5 gallons. Deficiency was rectified on the spot. (46 CFR 181.610)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of operation of the main propulsion machinery both ahead and astern. Vessel's stbd main engine is producing steam exhaust with unknown cause. Vessel operator to provide root cause and fix to the satisfaction of the cognizant OCMI. 46 CFR 176.804 (a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices must be corrected. The vessel's water lift muffler for the vessel's generator was not fully secured for sea. 46 CFR 176.830(a)

Each ring life buoy must be marked with Type II retroreflective material. Vessel's ringbuoy is missing retroflective material. 46 CFR 185.604(i)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The vessel's hydrostatic release for the EPIRB was expired. 46 CFR 185.740 (b)

Electrical equipment on a vessel must be installed and maintained to: Prevent electrical ignition of flammable vapors. Fireboy system has been spliced with butt connections that showing exposed conductor. Make proper splice with marine grade connections that include heat shrink to satisfaction of attending Marine Inspector. 46 CFR 183.200(d)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Excessive amount of oil found in engine room bilge, under generator, on port engine turbo outlet, and on wiring on ceiling above generator. Additionally, fuel found on engine block on port engine inside of fuel filters. Clean engine compartment to satisfaction of the attending Marine Inspector. 46 CFR 176.830(b)

On vessels described by 46 CFR 175.110(c), flammable items not otherwise covered by the regulations of this subchapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Operator could not produce approved Lithium-Ion Battery Charging Plan and Marine Inspector could not confirm adherence to the plan. Make Plan available onboard vessel for inspection by attending Marine Inspector. 46 CFR 185.364

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. There are two Masters with this Company that could not provide Marine Radio Operator Permits. 46 CFR 184.502: 47 CFR 80.159(e)(1)

Each life jacket carried on a vessel on an oceans route, must have a life jacket light… Each life jacket light must be securely attached to the front shoulder area of the life jacket. Marine Inspector found 16 lifejackets without functioning lights, operator replaced 10 lights, six life jackets remain outstanding. Vessel limited to 20 miles from shore. 46 CFR 180.75(a)

An adult life jacket must be provided for each person carried on board a vessel. Marine Inspector found 16 lifejackets without serviceable lights and one with damaged kpok, vessel restricted to 35 persons onboard. 46 CFR 180.71

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Inspection of all machinery essential to the routine operation of the vessel. Vessel has a length of hose running internal on starboard side, passing through several bulkheads without a vapor tight barrier, that is unsecured and that owner could not identify. Identify hose and make proper installation to the satisfaction of attending Marine Inspector. 46 CFR 176.804

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Deck beam at midships on starboard side is showing signs of rot and needs to be repaired prior to credit drydock in 2025. 46 CFR 176.802(a)(1)

The owner or managing operator shall make sure that maintenance is carried out in accordance with the instructions required under paragraph (a) of this section. One of the life floats has deteriorated beyond repair and is showing signs of cracking and waterlogging. Replace to satisfaction of attending Marine Inspector by 08JUL2023 or sooner. 46 CFR 185.702(b)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel has not had drydock examination and has lapsed. Resolve prior to carriage of passengers.

A pre-engineered fixed gas fire extinguishing system must: be installed in accordance with the manufacturer's instructions. Fireboy pressure is outside of proper operating parameters.

A provision must be made for closing all supply duct cowls or scoops for a space protected by fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Vessel had installed rubber flaps that were not effective in closing air supply from outside space. Submit plans for ventilation covers before making modifications or alterations.

Each collision bulkhead must extend to the weather deck. Collision bulkhead required by 171.040(b)(2) "Old T" is missing a watertight hatch, has multiple undressed penetrations, and does not extend up to the deck at port and starboard hull to deck joint.

Each hatch cover must have securing devices. Engine room hatch does not have a way to secure it closed.

A vessel must comply with the applicable requirements for any radio installation as set forth in 47 CFR part 80. Vessel has DSC capable radios that are incorrectly installed. Radios are not connect to GPS and are not loaded with MMSI. Make all radios capable of DSC operation.

Each vessel that operates on the high seas (Outside 3nm) must have on board a FCC Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The hydrostatic release on EPIRB is expired. Restricted to voyages within 3 NM.

A weather tight door with permanent watertight coamings that comply with height requirements in Table 171.124 must be provided for each opening located in a deck house or companionway that gives access to the hull and is located in a well. Vessel has a vent for head that leads to an opening on outside of house that does not meet coaming height for route. Vessel is authorized a temporary plugging of opening. Permanent modification due prior to credit drydock. Submit plans before making alterations.

Each well deck on a vessel must be watertight. Vessel removed anchor windlass and has exposed bolt holes allowing water intrusion below the deck.

Electrical equipment on a vessel must be installed and maintained to prevent electrical ignition from flammable vapors. Lighting fixture over port engine is not properly secured.

The marine employer must have sufficient number of alcohol testing strips readily accessible on board the vessel. Alcohol strips on board are expired.

A vessel must carry a first-aid kit approved under approval series 160.41 or a kit with equivalent content and instructions. Vessel has several first-aid kits of which has expired medications.

Connections must be made to battery terminals with permanent type connectors. Temporary type clamps are prohibited. Vessel has wingnuts on most battery installations.

A vessel of more than 26 feet in length, but not more than 65 feet, must carry a minimum of one life buoy not less than 24 inches in diameter. Vessel has 20 inch life buoy. Replace with orange 24 inch.

Each battery must be protected to protect against shifting with roll and pitch of the vessel. Plywood support structure under house batteries in the midships compartment are rotting. Repair prior to credit drydock.

A vessel on an oceans route must carry six red and six orange hand flares. 04 expired flares.

A vessel not required to have a power driven fire pump must have three 2.5 gallon buckets labeled "FIRE BUCKET". Vessel has three 5 gallon buckets.

Vapor barriers must be provided where insulation of flammable and combustible liquids are present. The engine room had an unsealed penetration not allowing a sufficient vapor barrier. 46 CFR 177.405(e)

Every vessel nsubject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installationh by an FCC-licensed technician every five years. U.S. vessels are exempt from provisions of title III, part III of the Communications Act that are less than 50 gross tons and navigated not more than 300 meters from the nearest land. Vessel's FFC documentation expired. Restricted to operations less than 300m (1,000ft) from shore. 46 CFR 185.726(d)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the normally unmanned spaces. Bilge high level alarms have no audible sound. 46 CFR 182.530(a)

Every vessel must have a detailed inspection of the radio installation by an FCC-licensed technician every five years. U.S. vessels are exempt from provisions of title III, part III of the Communications Act that are less than 50 gross tons and navigated not more than 300 meters from the nearest land. Vessel's FFC documentation expired. Restricted to operations less than 300m (1,000ft) from shore until rectified. 46 CFR 80.903 & 47 CFR 80.933

46 CFR 176.25-50 - At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. The Marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. The vessel had an excessive amount of oil in the engine room. The engine room bilge showed a large accumulation of oil and indications that the oil level was recently very much higher. The starboard engine showed excessive amounts of oil spatter and accumulation to indicate a leak or leaks are present. Vessel operator must determine causative factors for excessive oil in engine room and demonstrate to the satisfaction of the attending MI the vessel can operate without unsafe conditions or any hazardous situations onboard prior to vessel movement.

The Marine Inspector may conduct any such tests or inspections he or she deems necessary to be assured of the vessel's seaworthyness. During the inspection of potential oil pollution, MI identified numberous hazardous conditions that need to be evaluated in further detail. Vessel must undergo an annual inspection prior to the carriage of passengers for hire. 46 CFR 176.500

Remove (1) B-II portable fire extinguisher from inside the engine room and mount outside of the respective space so as to be convenient to fight fire in the space protected.

Permanently mount sea strainers

Remove non-type approved return fuel line to generator. Install type approved fuel line

Remove wing nuts from all battery terminals and install permanent type connectors.

Repair/replace bilge pump mounting bracket on port engine accessory drive belt and prove proper alignment.

Prove proper operation of the fixed bilge system from all spaces

Inspect fire fighting systems and equipment by a certified inspector.

Install a securing device to engine room hatch and prove water tightness.

Perform sea trials to prove proper operation of newly installed engines

Remove or Terminate Dead End Wires in Engine Room.

Install Hose Clamps on port engine sea water cooling hose at thru hull valve connection.

Make bulkhead penetrations in engine room vapor tight to prevent escape of fire fighting system agent.

Remove engine room lighting fixtures and clean oil and dirt residue from inside ficture covers.

Remove rotted plywood covering main deck frames above main engines. (non-structural)

Instal battery covers with ventilation

Remove or secure tool box on shelf located forward bulkhead directly above sea water cooling thru hull valves and engine accessroy drive belts.

Remove standing water and debris from all bilges and prove proper operation.
