No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. Schedule 40 piping is installed in fireman not in accordance with drawing. Renew to schedule 80 and perform hydrostatic test in accordance with 46CFR56.97-30 to satisfaction of attending marine inspector. 46CFR78.17-80(a)

The fire main has had two failures in the past year. In light of this, a hydrostatic pressure test of 1.5 times MAWP shall be conducted at the next scheduled dry-dock of the entire fire main. The hydrostatic test pressure must be maintained for a minimum total time of 10 minutes and for such additional time as may be necessary to conduct the examination for leakage. All tests and repairs are to be made to the satisfaction of the attending CG Marine Inspector.

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use and that all such equipment required by the regulations in this subchapter is provided, maintained, and replaced as indicated. A section of the deluge sprinkler system branch line piping, starboard side zone 4 (frame 89), was damaged. Provide repair plan to replace damaged piping during next scheduled dry docking. 46 CFR 78.17-80

In order to meet the ASME standard, a certified weld procedure is first required and then implemented by a qualified welder. Currently there is a soft patch on the firemain overboard discharge spool piece, inway of butterfly valve. Approved temp repair authorized Peterson Welding to fabricate new spool piece, however they do not have CG or ABS certified welders. Therefore repair is only temporary. Vessel to renew spool piece at next dockside availability, using an ABS or CG approved procedure and welder that is qualified to that procedure.

Periodic tests and inspection shall be made of the main and auxiliary machinery, boilers, and other equipment as prescribed in this part. The "hot box" the vessel was utilizing for the periodic tests does not fit the new jacket water temp probe and could not be set to a specific temperature but rather the power must be held in to raise the temperature. Provide method of testing jacket water temperature probes that meets Periodic SSafety Test procedure requirements for Coast Guard periodic inspections and maintenance needs.

Each vessel must have a gyrocompass. Vessel report that the gyro was not operating as intended. Vessel shall submit tech report attesting to proper repairs prior to returning to service. 33 CFR 164.35(d)

01 - 07106 - A fire detection and alarm system must automatically reset to a normal operating condition after alarm and fault situations are cleared. Power disruption caused fire panel to fault on Zone 4, only cleared by removing and reinstalling control card. Replace card at earliest opportunity and monitor for similar behavior in same/other Zones. - 46 CFR 76.27-10(c) - 10.C

In each crew berthing areas and washing areas information on the procedures and resources to report crimes, including sexual assault and sexual harassment, must be adequately posted. These procedures and resources must include at a minimum the following information: 1. Company policies regarding sexual assault, sexual harassment, and drug and alcohol usage. 2. Telephone numbers, websites and email addresses for reporting allegations of sexual assault and sexual harassment to the Coast Guard. 3. Company procedures to report violations of company policy and how to access resources. 4. Resources provided by outside organizations such as sexual assault hotlines and counseling. Provide a copy of informational bulletins addressing policies and procedures per safety management system satisfaction of attending Marine Inspector. 46 U.S.C. § 11101

The owner of a vessel subject to inspection under section 3301 shall ensure such vessel is equipped with a vessel master key control system, manual or electronic, which provides controlled access to all copies of the vessel's master key of which access shall only be available to an established list of crew, by position, allowed to access and use the master key. This list must be maintained onboard the vessel and included within vessel’s safety management system. Additionally, a log book must be maintained with records of access and use of the mater key and shall include the following information at a minimum; date and time of access, room or location accessed, and name and rank of crewmember using the master key. This log book is required to be included in the safety management system. It may be in electronic format and shall be located in a centralized location that is readily accessible to law enforcement personnel. Provide a plan to incorporate a master key control system and recordkeeping procedures onboard the vessel to the satisfaction of the attending Marine Inspector. 46 USC 3106 01MAR2024: Extended deficiency until 31MAR2024 pending final review of the proposed SMS amendment by ABS. 05APR2024: Extended deficiency until 30APR2024 to address SMS procedure discrepancies.

With respect to sexual harassment and sexual assault, procedures for and annual training requirements for all responsible persons and vessels to which this chapter applies on: Prevention; bystander intervention; reporting; response; and investigation. Provide a plan to incorporate the sexual assault and sexual harassment requirements into the Safety Management System to the satisfaction of the attending Marine Inspector. 46 USC 3203 & 46 USC 3205 02FEB2024: Due date extended to 01MAR2024 as third party is under contract to complete SMS update by 01MAR2024. 01MAR2024: Extended deficiency until 31MAR2024 pending final review of the proposed SMS amendment by ABS.

Each vessel must meet the applicable requirements in subchapter S of this chapter. Each opening in an exposed weather deck must have a means for closing weathertight. The MSD space escape hatch failed a hose test due to a crack on the sealing edge. Temporary repair as approved by attending MI is authorized through October 2024. Provide permanent repair proposal. 46 CFR 72.30-1 46 CFR 171.122(f)

The line locker bulkhead to the weather deck is no longer weathertight due to wastage, multiple holes from wastage was discovered. Temporary repair as approved by attending MI is authorized through October 2024. Provide permanent repair proposal. 46 CFR 72.30-1 46 CFR 171.122(b)

Each device must be free of design defects such as rough or sharp edges that may cause bodily injuries or that would allow toxic substances to escape to the interior of the vessel. Excessive corrosion was discovered under the angled side plating on the bottom plate of the MSD holding tank. Provide repair proposal to be completed to satisfaction of attending Marine Inspector no later than October 2024. 33 CFR 159.95 (a)(1)

The damper and the portion of duct containing the damper shall be constructed of at least 1/8 inch steel plate suitably stiffened. The bottom blade on the number 5 (1S2) vehicle deck supply automatic damper was found wasted through. Temporary repair as approved by attending MI is authorized through October 2024. Provide permanent repair proposal. 46 CFR 72.05-50 (b)

Provide and post schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate for the Emergancy

Vessel must conduct inspection of all active tags and audit Lock Out Tag Log as per the Company’s Lock out Tag Out Procedures. Vessel shall complete 2 inspections/Audit and forward results to USCG.

Provide adequate protection for all exposed portions of the main engine exhaust systems as required by 46 CFR 58.10-5(d)

Main engine exhaust lagging is deteriorated and failing, renew all failed exhaust lagging and ensure all exhaust components are adequately protected.

Prove proper operation of all automatic sprinkler system alarms as per approved PSTP's

Properly adjust the following fire screen doors on car deck: Steward storage, Engineers head, electric work shop and paint locker.

Reinstall wire grommet holding the wire for fire alarm in electrical work shop.

Properly secure cover for fuse box on car deck in electrical work shop.

Install missing bilge suction screens IVO Port and Starboard Bilge Well in the Engine Room.

Conduct repairs to the Gyrocompass prior to 5/26/2021, vessel can continue operate under the following Restrictions; Daylight operation only, More than three nautical miles of visibility and all Deck Watch standers are aware of the inaccurate Gyrocompass prior to accepting the watch.

Vessel's #1 SSDG tripped off-line while vessel was underway. Repair/renew #1 SSDG to original condition prior to carrying passengers.

Make permanent repairs to the fire detection system it currently showing a trouble condition in the supervision of the input trigger circuit. The error indicates an open loop in the PAD-2, however, the detective devices function. Make permanent repairs to the fire detection system and demonstrate to the satisfaction of attending CG Marine Inspector. 46 CFR 76.05

Strap which secures lifeboat onboard vessel during transit failed. Vessel fashioned temporary strap which may be cut in an emergency until replacement strap is installed. Vessel shall provide photo of newly installed strap and receipt/report of work done following replacement.

Crop and renew affected area as indicated on the most recent pit survey by completion of next scheduled dry dock.

Make Fire Screen doors leading into the observation deck area capable of fully closing regardless of closing sequence.

An ABS certified pit surveyor/NDT specialist shall conduct an extensive pit survey for hull plating and frames from 68-78 from inboard most longitudinal to level with the space's deck plate.

Conduct daily rounds of affected area, log and email report weekly to USCG Sector Anchorage. Immediately notify USCG of any conditions change.

Provide USCG Sector Anchorage ABS's Condition of Class for the temporary concrete repair within 30 days of this CG-835.

Provide permanant repair plan to USCG for review.

Update USCG on any changes of condition onboard the vessel related to the keel cooler leak prior to effecting permanant repairs.

Complete permanant repairs to the effected areas IVO stbd keel cooler near frame 70 and 71. If temp repairs degrades and begins leaking during the interim, it will need to be reported to the USCG immediately. Temp repairs will acceptable until 08FEB2019.

Re-write sections of the Periodic Safety Test Procedure (PSTP) due the written procedure not matching field procedure or demonstrated procedure. a) Accommodation Space Sprinkler System Tests Installed Fire Sprinkler system, Section D. Zone Flow Switch Alarm. Demonstrated test showed actual water tripping the flow switch versus manually tripping the switch, which is in accordance with 46 CFR 61.40-10(b). Re-write procedure as demonstrated during inspection. b) Diesel Generator Set Tests, Section J. Switchboard Voltage Cut Out Switch and Manual Voltage Control. The procedure needs re-sequencing (d) Reset Voltage switch back to Auto position. Ensure that Voltage and Hz are reading normal in Auto mode. and (e) Generator will need to be shut down and restarted to reset switchboard logic. By reversing these two steps will reduce questions and time in the field. c) Oil Fired Boiler Test. actual set points are missing from the test procedure, incorporate all test points. Example, normal operating pressure, high limit control pressure, oil pressure switch, relief valve set point. All points are missing from the document. Re-write the above requirements for the PSTP and submit to the MSC within 60 days, in accordance with 46 CFR 61.40 Design Verification and Periodic Testing of Vital system Automation.

Repair or replace the manual control system for the class three watertight door in way of engine room and shaft alley, due to exceeding the 90 second required time in accordance with ASTM -F-1197, Incorporated by referenced in 46 CFR 170.270.

Pin missing that provided linkage to engine and bridge for air control

Repair/replace Port davit winch motor.

Install access panels/plates for all automatic fire dampers to allow for inspection and verify operation.

Remove doubler and make permanent repairs to bulkhead IWO galley reefer/freezer. Conduct gauging of deck IWO leaking reefer/freezer.

Renew port and starboard lifeboat canopies to original specifications.

Replace damaged lifeboat windows on port and starboard lifeboats. Renew with original equipment.

Provide proof of operable marine radar via tech report.

Replace temporary piping installed on starboard main engine jacket water cooling discharge line with manufacturer produced or approved part.

Affect permanent repairs to starboard main engine jacket water cooling discharge piping.

Renew faulty governor on the port main diesel engine.

Correct two test procedures in the Machinery PSTP; (1)Sprinkler system test uses magnets this is in conflict with the manufacturer instructions, (2)The ESP 1000 is in need of simplification as discussed with crew and [name withheld] to MSC. In accordance with 46 CFR 62.30-10.

Both ship service generators fuel injection lines leak-off into the bilge and near hot surfaces. Install an approved safety system to collect excess fuel and an alarm to notify watch stander of issue. The alarm should be both visual and audible. Fire Hazard, 46 CFR 56.50-1(k).

Repair the car deck public address system, 46 CFR 113.50-5.

Double fire screen door #7 won't close properly from the bridge. Prove proper operation of door when actuated from bridge.

Prove proper operation of low lube oil pressure shutdown on #2 SSDG.

Make permanent repairs to cracks in starboard main engine block bed rails.

Port and Stbd main engine cooling lines are attached with modified "dresser" couplings. Provide suitable approved connections for each engine.

Port side Lifeboat Davit is inoperable. Provide proper repair plan or tech report for port side lifeboat davit brake system. Provide plan for mitigating reduction of life saving capacity on the port side of vessel.

Repair inoperable general emergency alarm light on the car deck and in the engine room.

Amend automation test procedures to reflect proper psi for the clutch air alarm. Sumbit to MSU Valdez for approval

Replace all navigation lights with UL1104 certified lights.
