Extension cords must not be used as a permanent connection to a source of electrical power. MI identified a general household power strip in use within the control console. Remove the power strip and replace with UL Listed approved strip.

Each cable and wire on a towing vessel must be installed to have sufficient current-carrying capacity for the circuit in which it is used. MI found an outlet connection on the bridge cat-walk wasted from its mount and on the deck. Re secure electrical outlet to mount.

A towing vessel fire hose must be a lined commercial fire hos , fitted with a corrosions-resistant material and capable of providing a solid stream and spray. MI identified two leaks on the port side deck station. Replace hose with new commercially lined hose.

Structural integrity for an existing vessel may be deemed by the OCMI, or TPO, to be in compliance if maintained to conform to the rules of a recognized classification society. MI found two wastage holes on the external storage deck outside the ship. Crop-n-Renew

A non-tank vessel of 400 gross tons or more as measured under the convention measurement system in 46 U.S.C. 14302 or the regulatory measurement system of 46 U.S.C. 14502 for vessels not measured under 46 U.S.C. 14302 must have a Non-Tank Vessel Response Plan (NTVRP). IAW 33 CFR 155.5021, non-tank vessels subject to this part may not Operate upon the navigable waters of the United States unless in compliance with a vessel response plan (VRP) approved under § 155.5065. USCG found that the ENDURANCE did not have the required NTVRP. Submit for Approval a vessel specific NTVRP IAW 33 CFR 155.5065 "Procedures for Plan submission and approval."

Ring Buoys were found in poor condition. Replace vessels ring life buoys.

Starboard Navigation Light was not operational upon time of inspection. Repair starboard Navigation green light.

Marine Inspector identified severe wastage on a tank valve pipe at the base of the deck. Crop and Renew wasted valve Pipe.

A vessel choosing the Coast Guard option must undergo an annual inspection within 3 months before or after the COI Anniversary date. Owner/operator never scheduled and missed the anniversary window. Complete required Annual Inspection.

The owner of a towing vessel must ensure that the strength of each component used for making up the towing vessel to the tow is adequate for its intended service. Vessel's crew noted in the log books that the tow line and winches appear to be in worn/unsatisfactory condition. Provide evidence that winches and tow cables have been properly inspected and found satisfactory or replaced prior to towing operation. 46 CFR 143.460 46 CFR 140.801

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces. The AB berthing does not have a smoke detector installed. Install an approved (UL217) smoke detector in the berthing prior to any persons utilizing the berthing area. 46 CFR 142.330(b)

Each ring lifebuoy on a towing vessel must be approved under 160.050 and marked in block letters with the name of the vessel. Two of the vessel ring lifebuoys are un-serviceable, replace un serviceable ring life buoys. One ring life buoy has faded vessel name, re stencil vessel name on the ring life buoy. 46 CFR 141.360(b)(1) 46 CFR 141.360(b)(4)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Multiple life jackets and immersion suits were found to have corroded batteries. Replace batteries in life jacket and immersion suits lights. 46 CFR 141.230

Each towing vessel must be equipped with sound signals IAW 33 CFR 84. Vessel's whistle was stuck in the open position and can only be controlled by an air valve. Make permanent repairs to ships whistle. 46 CFR 140.720

Each towing vessel must be equipped with navigation lights. Numerous secondary bulbs were blown on vessel's navigation lights. Replace secondary bulbs. 46 CFR 140.720

Towing vessels must carry up to date nautical publications. US Coast Pilot and Coast Guard light list were out of date. Provide up to date publications. 46 CFR 140.705.

A towing vessel subject to the Coast Guard inspection option must undergo an annual inspection within 3 months before or after the COI anniversary date. Vessel was due for the annual inspection on 16July2022. Make vessel available for inspection. 46 CFR 137.200

All fire detection systems must be inspected or tested at least once every 12 months. Vessel's fire detection system has not been inspected or tested in the past 12 months. 46 CFR 142.240(a)

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Two crew members were missing their medical certificates. 46 CFR 10.301(a)

All hand portable fire extinguishers must be inspected or tested at least once every 12 months. Excess extinguisher located next to grill on exterior was not serviced within 12 months. 46 CFR 142.240(a)

The master must ensure that all life saving equipment is properly maintained and ready for use at all times. Port side bridge ring life buoy is worn. 46 CFR 141.230

Installed Fire Detection System has not been installed or certified by a NICET IV Fire Alarm Engineering Technician, registered professional engineer with experience in fire-detection design, or authorized classification society with equivalent experience.

Vessel does not have a current eNOI on file for the 2013 Vessel General Permit (VGP) or any VGP-related documentation (i.e. visual inspection, annual inspection, reporting, etc.) on board the vessel. VGP 1.5.1.1

The vessel's AIS is currently broadcasting the previous vessel name/owner info. Update to reflect new name/owner info.

The auxiliary line for the line throwing appliance is missing.

The EPIRB battery was expired. Replace to new. Vessel restircted to no more than 3 miles from land.

Remove lagging from both MDE exhaust and make available for inspection the exhaust pipes. Evidence of leaks IWO manifold.

Repair/replace bow thruster fire pump packing. Pump is leaking excessivley.

PROVIDE PERMANENT HAND CRANK FOR RESCUE BAOT DAVIT.

PROVIDE FOUR (40 NEW RELIEF VALVES FOR DRY MUD TANKS. SET AT OR BELOW 42PSI.
