Fire tube boilers which cannot be entered, must be subjected to a hydrostatic test of 1.5 times the maximum allowable working pressure. During the test a leak was discovered on the front of the boiler on the bottom where the burner assembly connects to the main body of the boiler. Vessel must complete boiler repairs in accordance with 46CFR59 and complete hydro test prior to operating the boiler. 46 CFR 61.05-10(c) 46 CFR Part 59

The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J (Electrical Engineering) of this chapter. Systems of this type include propulsion control systems. Vessel reported both starboard engines having malfunctioning actuators, causing loss in pilothouse control of propulsion. Have technician service engine to determine cause and affect needed repairs prior to getting underway. 46 CFR 96.05

The installation of all systems of a marine engineering nature, together with the details of design,construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. Vessel reported starboard engine, 01 of 04 main engines serving 2 propellers, declutched without cause. Engineering crew expect faulty wiring likely responsible for declutch. Crew bypassed signal to prevent re-occurrence, manually clutched in. Vessel is permitted to proceed to Cleveland. Upon arrival, conduct repair to satisfaction of cognizant OCMI (MSU Cleveland) prior to departing. 46 CFR 96.03-1(a)

The certificate of inspection (COI) issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. Vessel master reported consent based departure of an able seaman. Replacement must report within 14 days. 46 CFR 15.501(a) 16c

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Vessel has oil leaking from the STB side shaft / prop. Vessel must submit a repair proposal to affect permanent repair to the leaking oil while in the port of Duluth. Vessel may transit within the port of Duluth provided the STB shaft is lock in place, and the vessel has at least two tugs of sufficient size and power to assist the vessel in maneuvering within the port. Vessel may not leave the port. Prove no additional damage has occurred to the STB side shaft, props, or stern hull area. 46 CFR 58.05-1(a)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Vessel reported that the port main engine of the port screw declutched, resulting in a reduction in propulsion. 46 CFR 58.05-1

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Vessels port tail shaft is bent to the point of causing abnormal vibrations when at operating speeds. Must demonstrate proper repair of the port tail shaft to USCG Marine Inspector prior to departing the port of Duluth. 46 CFR 58.05-1(a)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. A blade on the port prop is bent out of acceptable standards. 46 CFR 58.05-1(a)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. The port stern tube head tank is missing approx. 140 gallons of oil. Vessel is unable to identify what happened to that oil. Vessel must provide evidence to what happened to the missing oil. 46 CFR 58.05-1(a)

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel. Vessel must prove the structural integrity of the port strut, port rudder, port stern tube housing and surrounding area to the attending Marine Inspector. 46 CFR 91.25-50(a)

Inspected vessels are issued a Certificate of Inspection (COI) which indicates the minimum complement of officers and crew (including Lifeboat Operators) considered necessary for safe operation. On May 6th, 2025 the USCG became aware that the vessel dismissed the deck department of the vessel on May 5th due to unplanned repairs. Vessel did not submit a request to reduce manning. Vessel is not in compliance with the manning required on their COI. 46 CFR 15.105(c)

Each non-pressure-containing steering-gear component and each rudder stock must be of sound and reliable construction, meet the minimum material requirements of § 58.25-75, and be designed to standards at least equal to those established by the ABS Marine Vessel Rules. Rudder post is suspect to be out of alignment. Rudder post housing is suspected of damage and is leaking sewage down the rudder post onto the rudder and is spilling into Lake Superior. Prove to the USCG the rudder assembly is in proper working order via class report and / or technical report showing the rudder post alignment is within tolerance. Must also prove to the USCG the post housing is structurally sound. 46 CFR 58.25-5(d)

Each non-pressure-containing steering-gear component and each rudder stock must be of sound and reliable construction, meet the minimum material requirements of § 58.25-75, and be designed to standards at least equal to those established by the ABS Marine Vessel Rules. Port rudder stock is out of alignment. Repair port rudder post to be true and properly aligned at next dry dock. 46 CFR 58.25-5(d)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Treated sewage is leaking down the rudder post onto the rudder and into Lake Superior. Vessel must secure use of the treated sewage tank and affect repairs to the tank to be approved by the USCG prior to using the treated sewage tank. 33 CFR 159.7(b) 46 CFR 91.30-1(a)

Cargo vessels must meet the requirements in this subpart. Per Coast Guard letter dated May 1, 2024, the vessel was approved an alternative lifesaving arrangement with the caveat that an approved rescue boat and davit under 46 CFR 160.156 also be installed in place of the open top lifeboat. Plans for installation of a Coast Guard approved rescue boat shall be submitted to the OCMI within 30 days, and installation shall be completed prior to completion of winter layup 2026. 46 CFR 199.260

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Lifeboat was unable to be launched as designed. Repairs must be completed and satisfactorily tested prior to departure. 46 CFR 199.190(a)

All piping, valves, and fittings shall meet applicable requirements of Subchapter F. Firemain valve in the upper windlass room, directly next to the entry door from the main deck, is leaking. Repairs or replacement shall be made to prevent leaking of water into the windlass room. 46 CFR 95.10-15

Hatchways in position 1 and 2 must have weathertight hatch covers. #10 and #18 starboard side cargo hatch covers are cracked on the corners. Repairs shall be made to make weathertight. 46 CFR 45.145(a)

Main propulsion machinery must be provided with automatic shut-off controls to protect the engines. The Main Diesel Engines, (4), are under a warranty break-in period which limits their RPMs for the first 500 hour of use/operations. Upon completion of this break-in period the vessel must conduct an overspeed shutdown test to the satisfaction of the attending Marine Inspector.

Periodic testing and inspection of low pressure heating boilers require the following inspections/tests, which are past due: fireside/waterside/hydrostatic pressure test. These inspections and tests must be completed to the satisfaction of the attending Marine Inspector.

Master requested temporary manning reduction due to consent based discharge of AB Punzalan. The vessel is authorized a reduction manning of one AB during transit form Gary, IN to Duluth, MN. Three person watch system and work/rest hours must be maintained in accordance with applicable laws. Prior to departing Duluth, MN a replacement with the same or greater credential shall be found for the vacancy. The vessel/company shall make reasonable efforts at each port to find a replacement. If no replacement is found, the master must provide a new request to the cognizant OCMI with a new Temporary Manning Request Checklist D9-WI-DOM-009(01)-F(01).

Vessel submitted a temporary manning deviation request to sail short one AB required in the vessel's maintenance department for their voyage from Two Harbors, MN to Gary, IN. Vessel is authorized to operate short one AB for a one-time transit to Gary, IN in accordance with the submitted checklist/plan. If work/rest limitations and the 3 watch system cannot be maintained due to vessel traffic, adverse weather, or other reasons, the vessel shall cease operations and anchor or moor, in accordance with their submitted plan, until the vessel is in compliance with applicable regulations. Station bill shall be updated and emergency and security duties shall be fulfilled throughout period of deviation. 46 CFR 15.515(a)

Vessel submitted a temporary manning deviation request to sail short one OS required in the vessel's maintenance department for their voyage from Two Harbors, MN to Gary, IN. Vessel is authorized to operate short one OS for two weeks in accordance with the submitted checklist/plan. If work/rest limitations and the 3 watch system cannot be maintained due to vessel traffic, adverse weather, or other reasons, the vessel shall cease operations and anchor or moor, in accordance with their submitted plan, until the vessel is in compliance with applicable regulations. Station bill shall be updated and emergency and security duties shall be fulfilled throughout period of deviation. 46 CFR 15.515(a)

Master made notification to [name withheld], the vessel will be sailing without a 2nd mate. This is a request for a temporary reduction in manning until the company is able to hire/find a 2nd mate. The vessel is authorized to sail with one less 2nd mate for 07 days until 17July2024. Three person watch system and work/rest hours must be maintained in accordance with applicable laws. The vessel/company shall make reasonable efforts at each port to find a replacement. Prior to the expiration of the deficiency, the vessel/company is required to provide proof of adequate effort to find crew replacements.

01-13108-A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to ensure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel had unexplained loss of propulsion while transiting downbound St. Marys River. Vessel is required to provide Technician report attesting to the completeness and satisfactory operation of the propulsion system onboard prior to departing Potagannissing Bay Anchorage. 46CFR91.30-1(a) 17c

The vessel shall prove proper operation of the lifeboat, davit, and releasing mechanisms at next dock that will facilitate such a test but no later than April 24, 2024.

Vessel experienced a jacket water pump seal failure causing damage to the number 1,4,6,7,8 cylinder heads on their starboard Maine Diesel Engine. Vessel is to make full repairs prior to departure from St. Clair, [name withheld] transiting river systems and navigating restricted areas of navigation in Detroit COTP Zone, vessel shall use an assist tug and have an anchor watch. Submit tech report and chief engineer attestation of repairs when repairs are complete.

Vessel reported loss of main ship service generator power for 15 minutes, due to a tripped breaker. Operating generators must provide a continuous and uninterrupted source of power for the ship's service load under normal operational conditions. Troubleshoot, identify the cause of the loss of power, repair, and provide tech report to the satisfaction of ABS and local Coast Guard marine inspector prior to departure of Two Harbors, MN. 46 CFR 111.10-1(b)(4)-"Old J"

Means shall be provided for closing all other openings to the space protected by fixed CO2 from outside such space. Vessel emergency generator space door had kick out plate that would not properly close. Repair kick out plate to ensure proper closing of space.

In a space where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon. Vessels general alarm light over the STBD mains was inoperable. Repair general alarm light.

All emergency lights shall be marked with a letter "E" at least 1/2 inch high. Vessels emergency lights throughout the machinery space were not label. Label all emergency lights.

Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Vessel main diesel engines were missing guards. Replace guards on machinery.

Exhaust pipe installations must conform to the requirements of ABYC P-1. Vessels SSDGs, port inboard MDE and stbd outboard MDE exhaust lagging missing. Replace lagging.

Each junction box must have a cover. Junction box on bulkhead IVO OWS did not have a cover. Replace cover.

Electric installations on vessels must ensure the protection of crew and the vessel from electrical hazards. Vessel generator windings had dust built up. Clean SSDG and EDG windings.

Each opening in a watertight bulkhead must have a means to close it watertight. #7 port side ballast tank forward opening was leaking. Replace/repair gasket and cover.

Vital systems that are automatically or remotely controlled must be provided with instrumentation to monitor system parameters necessary for the safe and effective operation of the system. Vessels port side outboard MDE high crankcase pressure alarm inoperable. Repair/replace sensor.

Vessel reported that AIS is not transmitting or receiving. Vessel is required to have a functioning AIS for safety of navigation. Vessel shall notify each Sector Command Center/VTS of transit upon entering their zone. Vessel authorized to continue transit from Lake Superior to Conneaut, OH where repairs are to be completed and confirmed, by the USCG, prior to leaving that port.

Each conductor of a control, interlock, or indicator circuit must be disconnected from all sources of power by a disconnect device actuated by the opening of the controller door, or the power must first be disconnected to allow opening of the door. The control panel safety interlocks have been removed from the inboard and outboard boilers, permitting the panel to be opened without disconnecting the power. Install the control panel saftey interlocks.

Vessel was unable to lower lifeboat due to ballast and weather conditions during the Annual Inspection. Vessel is required to lower and maneuver the lifeboat in the presence of a CG MI within 45 days.

During 2021 tests of the auxiliary boiler safety relief valves (SRVs) the USCG Marine Inspector discovered that both boiler SRVs relieved 2-3 psi in excess of the 45 psi boiler MAWPs (and reciprocal 45 psi SRV settings). Inspection tags indicate that the boiler steam gauges were last calibrated on 12 JAN 2005. Re-calibrate, or replace, steam gauges and demonstrate satisfactory operation during 2022 test of SRVs, and prior to use of boilers in fall/winter 2022.

A 1/2" long crack was discovered in the reinforcing ring assembly within the combustion chamber of auxiliary boiler #CHI76-84-OB/S-4108. Repair crack IAW the boiler manufacturer's suggested repair/weld procedure and Section VII of the ASME Boiler and Pressure Vessel Code. Provide service report and photographic documentation of this repair to MSU Duluth. Repair must be completed, and approved, prior to resuming boiler operation.

Vessel was unable to get lifeboat underway during the inspection due to its location at the dock. Vessel is required to lower the lifeboat and have the crew maneuver it to meet the requirements of 46CFR 199.80 (d).

Prior to operation of AUX boilers Automation test procedures are required to be conducted per 46 CFR 63.15-9. Vessel also is due for mounts opened and removed, prior to operation of AUX boilers per 46 CFR 61.05-10.

While transiting the Sault Locks, the vessel shut down the starboard propulsion system due to a CPP issue that resulted in a stuck full ahead position. Make repairs to the system and provide proof of proper operation.

Due to the vessels mooring arrangement, unable to verify proper operation of the davit, winch, brakes and release hook. Crew was unable to launch the lifeboat & demon- strate proper operation of the lifeboat in the water, recover, and cradle. Demonstrate the above in the presence of a Coast Guard Marine Inspector.

Both auxiliary boilers safety-valve inspection and fusible plug inspection is overdue. Boilers must be made available for examination by CG Marine Inspector prior to placing either boiler in service.

Both auxiliary boilers must have their automation systems tested prior to placing either boiler in service. Automation testing is to be conducted in the presence of an attending CG Marine Inspector.

Cable penetrations were found to not meet structural fire protection requirements. Decks separating the accommodations and control stations from machinery spaces shall be of "A" class construction. Some penetrations were found to be lacking an approved sealing system. Install properly listed and approved systems to ensure class "A" boundaries are properly maintained before March 31, 2021.

Vessel sustained hull fracture when departing dock IVO of #8 ballast tank on the port side. Complete permanent repairs to the satisfaction of class prior to carrying cargo or engaging in commercial operations.

Vessel's port CPP system experienced crack and leak in a half inch steel pipe nipple that is threaded into the reducing valve on the low pressure side of the CPP pump. Conduct permanent repairs to system within 30 days, to the satisfaction of class and the cognizant OCMI.

On August 30, 2019 M/V AMERICAN INTEGRITY experienced a reduction in propulsion as the starboard outboard generator was taken off-line due to anticipated repairs. Vessel shall make full repairs to generator to satisfaction of USCG.

Vessel is to replace oil pump on starboard outboard main diesel engine prior to departure from next port. 46 CFR 91.30-1

Vessel is to make permanent repairs to starboard ships service diesel generator which is experiencing engine hunting. 46 CFR 91.30-1

Vessel reported loss of stbd anchor for unknown reasons on Lake Superior IVO Duluth, MN. Actions are being taken to locate lost anchor. Replace anchor in-kind IAW the applicable ABS standards for anchors and demonstrate functional test to the satisfaction of ABS(46CFR96.07-5).

Helmsman must remain on watch at all times until autopilot failure(loss of CDU) alarm is installed and tested to the satisfaction of the attending CG MI.

Vessels lifeboat must be launched and operated by the crew/maneuvered in the water and recovered at least once every three months during abandon ship drill; at the time of inspection, the vessels ballast condition prevented this action. Vessel to get lifeboat launched, maneuvered in the water with oars and recovered to the satisfaction of an attending USCG MI (per D9 direction)

Vessel is authorized to conduct underway troubleshooting of POME de-clutching issue enroute to next port of call. Make permanent repairs and demonstrate satisfactory clutch operation on the POME to the satisfaction of a USCG Marine Inspector. Repairs to be completed prior to departure from the Saint Clair Power Plant, MI.

Effect permanent repairs to the port outboard main diesel engine prior to departure from next port of call, Superior, WI.

Conduct permanent repairs and provide a written statement attesting to the proper operation of the starboard inboard engine to the satisfaction of a CG marine inspector.

Conduct review of electric power and control systems to determine causes/s of repeated slow steering and failure of auxiliary contacts in the low voltage starter controllers in the steering system. Report must be to the satisfaction of the cognizant OCMI and ABS.

Port steering system operating slower than stbd system (intermittent). Prove proper operation of both steering systems to the satisfaction of CG MI prior to departing the port of Toledo.

Make permanent repairs to primary clutch deflate (disengage) valve.

Helmsman shall remain on watch at the main steering console at all times when underway until Design Verification Test Procedures (DVTPs), in-regards to the auto-pilot failure alarm, are corrected to the satisfaction of the OCMI. (46 CFR 58.25-5(c))

Install proper thresholds and ensure proper closing of fire screen doors at the port/stbd int. fwd stairwell and the officers mess servicing door to ensure adequate structural fire protection.

Conduct fire drill.

Conduct satisfactory weight test of the lifeboat

Provide fuse cover for missing space in the nav light panel.

Perform operational test of lifeboat when mooring arrangment permits.

Ensure operational readiness of all lifesaving lifting and lowering appliances.
