Vessel exceeded 5 year dry dock requirement. Prior to carriage of passengers, vessel must be given credited drydock prior to carriage of passengers. 46 CFR 115.600(c)(1)

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended: (5) Batteries shall be checked for condition and security of stowage. 03 batteries in Engine Room contain excessive liquid in each battery storage container. 46 CFR 176.25-15(a)(5) (OLD T)

Remote fuel shutoff stations shall be marked in at least 1-inch letters indicating purpose of the valve and direction of operation. Remote fuel shutoff markings are missing. 46 CFR 185.30-20 (OLD T)

At each initial and subsequent inspection for certification, the marine inspector shall inspect all lifesaving equipment to determine its serviceability. Grab line on a lifebuoy was severed. 46 CFR 176.25-22(a) (OLD T)

A grease extraction hood must be equipped with a dry or wet chemical fire extinguishing system meeting the applicable sections of NFPA 17 ‘‘Dry Chemical Extinguishing Systems,’’ 17A ‘‘Wet Chemical Extinguishing Systems,’’ or other standard specified by the Commandant, and must be listed by an independent laboratory recognized by the Commandant. Two fusible links were not replaced at last annual inspection as required by NFPA. 46 CFR 118.425(b), 16c

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line must be marked to indicate the space served. Vessel lacked CO2 instructions, schematic, and labels. 46 CFR 122.612(a), 16c

At all inspections special attention shall be paid to insure that no fire hazards exist. Grease accumulation in galley space posed significant fire risk. Cleaned in person to the satisfaction of attending MIs. 46 CFR 176.25-10(b), 10c

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Vessel's fire panel was showing system trouble alarms and did not alarm when fire detection was activated. Conduct repairs in accordance with manufacturer instructions/standards and perform satisfactory tests in the presence of a qualified MI prior to carriage of passengers.

A door in a bulkhead required to be A-0 must be capable of meeting the requirements of an A-0 class bulkhead. Door going into C02 space is deformed and does not properly seal space. 46CFR116.435(c)(2)

Multiple self closing doors throughout vessel do not fully seat when closed. 46CFR116.435(c)(2)

For semiportable and fixed gas fire extinguishing systems, the owner or managing operator shall provide satisfactory evidence of the required servicing to the marine inspector. Servicing tags for galley fixed gas indicated 6 month servicing was required. Update servicing document to reflect service tag discrepancy. 46CFR115.810(b)(2)

All pages of the stability letter must be visible from the operating station. Stability letter was not posted at the operating station. 46CFR115.306

Each individual bilge suction shall be fitted with a suitable bilge strainer. Bilge suction in bow thruster space did not have a strainer. 46CFR56.50-50(g)

The owner or managing operator shall have the vessel ready for inspection of its smoke and fire detecting systems. Fire detector in steering gear room was missing. 46CFR115.810(a)(7)

A-Class decks must be insulated with approved structural insulation. Overhead in steering gear room was missing parts of fire insulation. 46CFR116.415(a)(i)

The following records must be kept: annual audit of the Vessel Security Plan. The security assessment was not kept in the security binder. 33CFR104.235(a)(8)

07109 - VESSELS WITH FIXED GAS EXTINGUISHING SYSTEMS SHALL HAVE A DEVICE THAT AUTOMATICALLY SHUTS DOWN POWER VENTILATION AND ENGINES THAT DRAW INTAKE FROM THE PROTECTED SPACE. THE AUTOMATIC SHUTDOWN DID NOT SHUT DOWN THE EMERGENCY GENERATOR OF BOW THRUSTER ENGINE. DETERMINE A COURSE OF ACTION FOR REPAIRS AND SUBMIT IT TO MI. 46CFR118.410(b)(10) 16C

Vessel must install strainers on bilge piping system in tank space and steering space. 46 CFR 119.510 / 46 CFR 56.50-50.

Vessel must repair two holes in the ventiliation duck located in the steering space.

Upon testing bilge system, the bilge pump was unable to properly dewater any of the compartments. Vessel is to prove proper operation of bilge system to CG Marine Inspector. 46 CFR 119.500(a).

Machinery equipment and arrangemnts may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Morse mechanical cable for the starboard main engine ZF drive was being held in place by a zip tie. Zip tie must be removed and repaired to manufacture specs. 46 CFR 115.800(a)

Materials and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. All dead end cables must be removed from the control station. 46 CFR 115.800(a)

Cable and wire for power and lighting circuits must be listed by Underwriters Laboratories as UL Boat or UL Marine cable. ROMEX cable under the operating station muste be removed and replaced. 46 CFR 120.340.(d)(2)

Materials used in piping systems must meet the requirments of this subpart and be otherwise acceptable to the cognizant OCMI. The packing material for the overboard firemain and firemain supply valves must be replaced. 46 CFR 119.700

Machinery equipment and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The hydarulic system on the bow thruster leaks heavily and must be repaired. 46 CFR 115.800(a)

Each battery must be free of exposure to water splash or spray. The batteries on the bridge must be equipped with a cover. 46 CFR 120.350(b)

Arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. When tasked with pumping out the bilge spaces, the crew was not aware of how to accomplish the task until the master stepped in. The company must provide training for the crew members on operation of essential systems, specifically the bilge pumpimg system. 46 CFR 115.800(a)

The 2nd deck zone piping has hole, repair and test. The portable fire extinguisher quantity on the 2nd deck will be increased to 3 and an additional deck hand will be added to the vessel to act as a fire watch on the 2nd deck for early intervention.

STBD Eng shows intermittent fault interrupting operation, vessel has prove proper operation of port eng and bow thruster at this time. Vessel may operate for one 4 HR trip on 27 AUG 16, with tug assist. Vessel must prove proper operation prior to carrying passengers after this trip.
