A vessel to which this sub-chapter applies may not be operated without having a valid U.S. Coast Guard Certificate of Inspection. Vessels topside inspection is past due.

Certificate of Inspection is passed 90 days of the vessel's annual inspection. Vessel is restricted from operating, IAW with the COI. An annual inspection will be required prior to operating as listed.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. MI found EPIRB hydrostatis release was expired. Replace EIPRB hydrostatic release.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found in wheelhouse, under helm. Remove dead end wiring and update electrical plans.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Generator windings were dirty and covered in dust. Clean windings in accordance with manufactures manual and notify local OCMI.

"(a) Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box (b) An alarm for a fixed gas fire extinguishing system must be clearly and conspicuously marked “WHEN ALARM SOUNDS-VACATE AT ONCE. CARBON DIOXIDE BEING RELEASED”. Where a different extinguishing agent is installed, that agent shall be marked in place of “carbon dioxide.” (g) Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. (3) Spaces into which carbon dioxide might migrate—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEARBY SPACE CAN COLLECT HERE. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED VACATE IMMEDIATELY.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units."

The owner shall make vessel available for drydock examinations, internal structural examinations at least once every 2 years if the vessel operates on saltwater. Vessel drydock 2 year time frame has elapsed. Conduct drydock prior to conducting operations. 46 CFR 176.600

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. Marine Inspector (MI) found several life jackets with inoperable light. Fix or replace inoperable life jacket lights.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. MI found the emergency lighting in the passenger accommodations was not adequate. Replace with brighter lighting.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. MI was told by Master that the fire fighting servicing was not completed. Have all fire fighting equipment serviced by qualified individual.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation (COD) currently in effect for that vessel. The vessel does not have a current COD. Vessel is to obtain a current COD.

STBD chine IWO the engine room found to have rot. Scrape and treat affected area.

Electrical outlet covers in berthing space, galley, and bridge cracked; replace. Galley electrical panel cover missing securing hardware; install screws. Galley electrical panel not properly sealed; seal openings. Electrical surge suppressor power strip in passenger berthing bunk connected to nearby outlet; remove.

Vessels ring buoy missing retro reflective tape; install retro tape.

No alcohol swabs available onboard vessel. Provide adequate supply of alcohol swabs for DAPI program requirements.

Passenger space bilge sensor alarm unit inoperable. Repair / replace.

Improper stowage of portside IBA. Re-secure IBA in its craddle.

Vessel’s FCC Safety Certificate is expired. Have an FCC representative conduct a new safety exam and provide a valid FCC Safety Certificate. Vessel is restricted to not more than 1000 feet from shore until deficiency is rectified.

Vessel’s alcohol test strips are expired. Provide valid alcohol test strips for vessel.

None of the licensed crew members have Marine Radio Operator Permits. Acquire Marine Radio Operator Permits for all licensed crew members.

The bilge alarm in the forepeak is not operational.

All navigation charts were damaged and/or outdated

Emergency instructions for vessel were missing. Provide and post Emergency Instructions for vessel in a place easily viewed by passengers and crew.

Found several wire nuts in space behind helm. Replace all wire nut connections with positive pressure connectors or place all wire nut connections in a junction box. None of the li

The master and crew of a vessel will be familiar with the content of and have mounted emergency instructions for activation of CO2 engine room fixed fire fighting system.

Replace / refasten starboard aft mooring cleat.

INSTALL FIRE MAIN SYSTEM IAW SUBCHAPTER T / SUBPART C MAX PAX REDUCED TO 49 UNTIL AN INSTALLED FIRE MAIN AND FIRE PUMP IS PROPERLY INSTALLED AT NEXT DRYDOCK PERIOD.

REPLACE 01 LIFE RING FOUND WITH DETERIORATED GRAB LINE.

PORT SIDE NAV LIGHT IS INOPERABLE. ENSURE BOTH NAV LIGHTS ARE OPERATIONAL.

INPUT VESSELS MMSI NUMBER IN VHF/DSC.

Sector San Diego does not have vessels electrical plans on file. Provide a one-line electrical drawing for all A/C switchboards and distribution panels. Include all circuit loads, conductor sizes, over current protection, and max panel ratings in amps. 46 CFR 177.202(b)(4)

Vessel under 65 ft and has no fixed firefighting system in compliance with NFPA 96. Provide UL approved smoke detector in galley in vicinity of cooking area. Sector San [name withheld] Policy Letter #20

Electrical equipment must be installed and maintained to protect passengers, crew and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Correct the following: (1)Label all circuit breakers or replace with breakers that are labeled (2) Remove wire nuts.

Galley must be surrounded by B-15 class fire boundries. Surround galley hood with B-15 material and ensure vent from hood is nono-combustible material. 46 CFR 177.410(c)(1)

Emergency lighting must automatically actuate upon failure of the main lighting system. Make E-lights actuate without needing to engage with inverter. 46 CFR 183.432(b)

Hydrostatic release for EPIRB did not have dates marked. Provide documentation of when hydrostatic release was installed and mark expiration date on hydrostatic release from two years of installation. 46 CFR 185.740(b)

Make repairs to starboard side: rudder, rudder supports, propeller, shaft, and shaft coupling. Perform sea trials prior to carriage of passengers.

Replace IBA weak link.

Repair/replace forepeak bilge alarm.

Passenger count reduced to 37 adults and 3 children until failed lifejackets are corrected.

Provide waste management plan.
