A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Fwd bilge suction piping after manifold was wasted. Replaced wasted piping and provide SAT test to the attending marine inspector.

Make repairs to crack in upper middle portion starboard deck rail. 46 CFR 177.100

Make repairs to wasted deck weld on port quarter freeing port stanchion. 46 CFR 177.100

Make repairs to cracked starboard bow chock. 46 CFR 177.100

Fill bulkhead wire penetrations located WT Bulkhead # 2, port side adjacent to salon head. 46 CFR 179.320

Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Exhaust gas was present during port generator operation in the lazarette space. Remove exhaust lagging, identify piping leak and submit repair proposal. Port generator is restricted to emergency use only until leak is repaired. Vessel is to draft safety plan for entry into space for crew awareness to include use of atmospheric testing and air changes until repair is completed.

When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Port forward bit showed weld failure in the deck inboard deck area. Submit repair proposal and complete repairs NLT 2024 dry dock to satisfaction of attending marine inspector.

Waste receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Remove all combustible waste receptacles and replace with non-combustible type.

Valves for removing water or impurities from diesel fuel in water traps or strainers are permitted. These valves must be provided with caps or plugs to prevent fuel leakage. All (5) fuel strainer assemblies on board were missing plugs or caps. Install plug or cap assembly for each strainer.

Stbd bow chock was completely damaged beyond use with a crack in bulwark. Damage looked to be localized to bulwark chock. Submit repair proposal and complete repairs NLT 2024 dry dock to satisfaction of attending marine inspector.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck, or, if it is readily accessible under service conditions, a screw-down valve without a reach rod may be fitted to the bilge line on the after side of the collision bulkhead. Reach rod was stuck and not operable on initial test. Vessel exercised and greased on site with SAT test from weather deck.

When the exhaust cooling water system is separate from the engine cooling water system, a suitable warning device, visual or audible, must be installed at the operating station to indicate any reduction in normal water flow in the exhaust cooling system. Waring device was not working for Port main diesel engine exhaust. Vessel replaced flow alarm on site with SAT alarm test from operating station.

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of 46 CFR. Waterlight had weak strobe. Replace battery/or repair as per manufacturer requirements.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). Retroreflective marking was wasted on numerous lifejackets. Make lifejackets in compliance to the satisfaction of attending Marine Inspector.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel's testing devices were expired. Until such time that valid testing devices are available onboard, vessel representative has stated that obtaining the devices and conducting the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. Vessel will need to replace testing devices as required and or show written policy attesting to how the vessel will obtain the devices and conduct the required alcohol tests within 2 hours from the time of occurrence of the SMI.

When a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. Vessel did not have stability letter posted. Post stability letter as required per regulation and to the satisfaction of attending Marine Inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel's charts were not updated. Provide current/corrected charts for operational areas. Vessel has paper charts, but utilizes electronic charts while navigating. Electronic charts are from May 2009. Provide proof of up-to date electronic charts unless vessel intends to use paper charts for navigation.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and Current tables. Vessel did not have electronic versions or hard copies of these publications. Electronic versions were kept on server that vessel did not have access to due to poor wireless connection. If vessel intends to use electronic versions of these publications they must be downloaded for immediate use.

Strong and effective rudder stops must be provided to prevent jamming and damage to the rudder and its fittings. These stops may be structural or internal to the main steering gear. Rudder stops had large gap at hard over (port & starboard). Make rudder stops as required as per regulation and to the satisfaction of attending Marine Inspector.

In a vessel of 12 meters or more in length but less than 50 meters in length, A sidelight must be visible for 2 miles. Port sidelight was not illuminating. Make port sidelight illuminate as per regulation and to satisfaction of attending Marine Inspector.

A 40-B portable fire extinguisher installed outside the space may be substituted for a fixed gas fire extinguishing system in a paint locker, with a volume of not more than 200 cubic feet. portable fire extinguisher discharge hose permanently affixed to port leading through paint locker bulkhead. Port inside paint locker was blocked with debris that could impede effectiveness of extinguishing agent. Clear area around extinguisher discharge port and keep clear at all times to the satisfaction of attending Marine Inspector.

Each Class 1 door on a vessel must be designed to withstand a head of water equivalent to the depth from the sill of the door to the margin line but in no case less than 10 feet. Vessel was not prepared to test watertightness. Test in accordance with ASTM F 1196. Another effective watertight test is the “chalk test.” Rub chalk on the knife edge of the door frame and close the door securely. It should leave a continuous chalk impression on the door gasket. If it is not continuous, the door requires adjustment or the frame requires replacement. Conduct testing in accordance with ASTM F 1196 or a chalk test and send results to attending Marine Inspector with Master attesting to completion.

Provide a repair proposal for the starboard side #3 frame that is fractured.

Public address speakers on aft deck inoperable. Speakers not installed in galley and lower passenger areas. Demonstrate proper operation of public address system in all spaces accessible to passengers.

Safety orientation not being conducted.

Monthly portable fire extinguisher checks not being conducted or logged.

Switchboard hertz meter inoperable. Demonstrate proepr operation.

Pressure gauge on starboard air receiver not reading accurately. Replace and demonstrate proper operation of relief valve.

Vessel is required to complete a hull exam every 2 yerars. Vessel is overdue for hull exam.

Make fire dampers seal better with gasket materials and additional securing devices.

Complete hydro testing of portable CO2 fire extinguisher for paint locker, or replace bottle with currently serviced unit.

Make emergency escape hatch for forward salon operable; passengers restricted to main deck untl operable.

Replace corroded wire from battery, stbd side gen compartment.

Provide crew training on rescue sling/block & tackle assembly.

Clad weld isolated pit in E/R just aft of hydraulic tank.

Repair/replace leaking valve on sight glass for lube oil tank.

Ventilate laz for inspection.

Clad weld isolated pit just inboard of stbd rudder.

Repair support bracket for port exhaust line in laz.

Complete credit drydock exam prior to carrying passengers after 31OCT16.
