Containers, brackets, racks, and other similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Resolution A.760(18) indicating the device stowed in that location. Vessels life saving appliances does not match the fire control plan.

Approved portable fire extinguishers and semi-portable fire extinguishing systems must be installed in accordance with Table 95.50-10(a) of this section. The location of the equipment must be to the satisfaction of the Officer in Charge, Marine Inspection.

Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Cargo pump prime mover has one thermal couple wired with twist connectors.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Liferaft was taken off vessel for maintenance and last inspection due was 09MAR2024. Provide new service record. 46 CFR 199.190(a)

Each load line certificate is issued for the following length of time: Coastwise certificate is issued for 5 years. Provide new ABS Load Line Certificate once received. 46 CFR 42.07-45(d)(1)

The letter referenced in 155.710(e)(1) must designate the holder as a person-in-charge of the transfer of fuel oil and state that the holder has received sufficient instruction. PIC letter not designated for crew on board. 33 CFR 155.715

The approval period for a Shipboard Oil Pollution Emergency Plan expires 5 years after the approval date. SOPEP on board has an approval letter that expired on 08OCT2023. Submit SOPEP plan and provide new approval letter. 33 CFR 151.27(e)

When cylinders of Class 2 materials being transported by vessel are stowed in a vertical position they must be stowed in a block and cribbed or boxed-in with suitable sound lumber and the box or crib dunnaged to provide clearance from a steel deck at least 10 cm off any metal deck. Oxygen and acetylene cylinders were stowed on the bare metal deck in the upright position. 49 CFR 176.200(c)

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. The Fire Control Plan was recently modified without Coast Guard approval. Submit plan to MSC for Coast Guard review. 46 CFR 91.55-10

Electrical installation on vessels must ensure protection of passengers, crew other persons, and the vessel from electrical hazards. The wired connection on top of the deck house was frayed and weather tight seal was found broken. 46 CFR 111.01-1

Vessel has "daisy chain" extension cord from the coffee maker plugged into a power strip in the E/R. Remove extension cord. 46 CFR 111.01-1

Means shall be provided for closing all ventilators in case of a fire. Damper for machinery space in overhead louvers were unable to self close.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Records were not onboard for the inspection.

Metal supports shall be designed to secure cable without damage to insulation or armor. Sensor cable on main engine secured in a way that is causing damage to shielding.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. Vessel was involved in an allison with Northstar Independence. Vessels STBD bow hit the Independence's port quarter. Vessel is restricted prior to movement. Vessel must provide proof and a statement that the vessel suffered no damage, no water ingress, no pollution, and no injuries. 46 CFR 92.01-10 60C

Each vessel must carry lifebuoys approved under approval series 160.150. 3 life buoys were wasted and missing sealing plugs. The fwd life buoy has wrong retro installed.

Each vessel must carry a category 1 406 MHz satellite EPIRB meeting the requirements of 47 CFR part 80. (b) When the vessel is underway, the EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the vessel sinks. EPIRB hydrostatic release is expired and registration sticker is missing.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. The self-igniting light may not be attached to the lifebuoys required by this section to be fitted with lifelines. Wrong batteries were installed in self-igniting light.

the “American Bureau of Shipping” or such other recognized classification society which the Commandant may approve as the load line assigning and issuing authority for a vessel, as provided in sections 3 of the load line acts. Vessel conducted temporary epoxy repairs to wasted areas in the house overhead and aft bulkhead and ABS was not notified.

Load line ring, lines and letters shall be painted in white or yellow on a dark ground or in black on a light ground. Stbd load line not painted.

First aid supplies are required to be readily available under paragraph §1910.151(b). An example of the minimal contents of a generic first aid kit is described in American National Standard (ANSI) Z308.1-1998 “Minimum Requirements for Workplace First-aid Kits.” Contents expired or missing.

On vessels of over 25 gross tons, the original certificate of inspection shall, in general, be framed under glass and posted in a conspicuous place where it will be most likely to be observed. Original not present on the vessel.

Enough emergency lights throughout machinery spaces to allow essential operations and observations under emergency conditions and to allow restoration of service. Emergency light needs to be provided for the SWBD.

Medical certificates issued to all other mariners will be issued for a maximum period of 5 years. Hosiah Thompson medical certificate was not present.

The vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Plastic can with oily rags present in engine room.

Hose assemblies may be installed between two points of relative motion but shall not be subjected to torsional deflection (twisting) under any conditions of operation and shall be limited, in general, to reasonable lengths required for flexibility. Hydraulic leaks were present on STBD pump assembly.

Each flexible cord and cable must meet the requirements in section 24.6.1 of IEEE 45-2002, Article 400 of NFPA NEC 2002, NEMA WC-3, NEMA WC-70, or UL 62 (all five standards incorporated by reference; see 46 CFR 110.10-1). Power strip utilized in office is not UL listed for marine use.

Sections 25, except 25.11, of IEEE 45-2002 (incorporated by reference; see 46 CFR 110.10-1); Metallic band strapping used for cable support should be steel and corrosion treated if not a corrosion-resistant material.Cable retention devices should be installed not less than every 610 mm on vertical runs and not less than every 2.5 m on horizontal runs. Banding missing on wiring not limited to stbd fwd leg sensor.

The certificate of Inspection will describe the conditions under which it is issued. Vessel is required to have person in charge designated in writing and was not present.

Liferaft onboard servicing is expired and not installed with a float free arrangement. Provide properly serviced liferaft properly installed with a hydrostatic release.

Paint locker fixed firfighting CO2 system has not been annualy serviced. Provide servicing certificate.

Three gate valves were found to be leaking diesel on fuel piping. Fix leaks to satisfaction of attending CG Marine Inspector.

Louvers for ventilation fan blower would not close when fan was secured. Demonstrate proper operation.

Three flame screens on diesel fuel and lube oil vents were found in a wasted condition. Install new flame screens.

Four lifejackets were found in a wasted condition. Four new lifejackets were brought onboard; corrected on the spot.

Damage Survey conducted due to severe wastage in the port fwd rake. UT Report shows multiple areas exceeding the maximum allowable wastage. Repair proposal and repair inspections will be to the satisfaction of the attending Marine Inspector. Vessel is not to conduct operations until repairs have been completed and approved by the Coast Guard and ABS.

Damage to vessel's hull was found by vessel reps; repairs began without proper notification to OCMI. Make vessel available for internal structural exam; Make affected repairs available to the satisfaction of the attending marine inspector prior to operating vessel. 46CFR91.45-1(a)/46CFR91.45-5(a)

Provide proof of annual servicing for 6 person inflatable liferaft.

Air receiver had cut off valve between tank and safety relief valve. Remove cut off valve and prove proper operation of safety relief valve to satisfaction of CG Marine Inspector.

Vessel to develop and have approved a plan proposal to repair crack and ensure no other leaks in fwd p/s rake as well as cargo storage area. It needs to be determined where water came from in p/s cargo storage area. Vessel to advise Coast Guard of any changes in water levels in both spaces. Vessel to remain moored until proposal is approved and repairs are ready to be made.

Vessel to develop and have approved a plan proposal for the clean up and identification of fuel oil leak into stbd side centerline void. Vessel to complete prior to departure.

Develop a repair proposal and have approved by Coast Guard of cracks/leaks in fwd rake and possible cracks/leaks in other spaces. Vessel is to maintain and log 30-min rounds of fwd rake and cargo storage area. Vessel to remain moored until cleared.
