Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Aluminum hull plating was found with a 6-inch longitudinal crack next to the stbd generator exhaust. Submit a repair proposal to the attending marine inspector.

A vessel which, under § 178.310 of this part, complies with requirements in sub chapter S of this chapter, must have stability details on the vessel's Certificate of Inspection, a stability letter issued by the cognizant Officer in Charge, Marine Inspection (OCMI) or the Commanding Officer, Marine Safety Center, or an approved stability booklet. BILGES: The vessel’s bilges and voids shall be kept pumped to minimum content at all times consistent with pollution prevention requirements. Vessel's engine room bilge had approximately 4-6 inches of oily water in centerline bilge. Clean out bilges to the satisfaction of attending marine inspector.

When the exhaust cooling water system is separate from the engine cooling water system, a suitable warning device, visual or audible, must be installed at the operating station to indicate any reduction in normal water flow in the exhaust cooling system. Port exhaust audible and visual warning device at the operating station was inoperable. Repair alarm panel or flow switch and provide satisfactory test of port exhaust audible and visual alarm to the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel master did not have a valid radio operator permit. Present valid radio operator permit for master on board during inspection or show a vessel log that can prove there is a crew member on board during vessel operation with a valid radio operator permit to the attending marine inspector.

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, and on a vessel of more than 19.8 meters (65 feet) in length, each hose must be lined commercial fire hose that conforms to UL 19 or hose that is listed and labeled by an independent laboratory recognized by the Commandant as being equivalent in performance. The port side hose was found leaking through a point approximately two feet from the fire plug end of the hose. Replace with approved and properly labeled hose and destroy the damaged hose.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. The vessel was unable to get suction from the after bilge pickup in the engine room. The forward bilge suction was functioning. Demonstrate proper operation of engine space bilge suction as designed and approved.

A vessel of at least 7.9 meters (26 feet) in length must be provided with individual bilge lines and bilge suctions for each watertight compartment, except that the space forward of the collision bulkhead need not be fitted with a bilge suction line when the arrangement of the vessel is such that ordinary leakage may be removed from this compartment by the use of a hand portable bilge pump or other equipment, and such equipment is provided. The fore peak bilge suction line was found wasted and unable to provide adequate suction for dewatering. Vessel made temporary repairs using electrical tape and demonstrated suction, and provided additional electric submersible pump to provide space dewatering. Make proper permanent repairs or provide repair plan with appropriate intermediate repairs.

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Penetrations in the transverse watertight bulkheads located at frames 10 and 20 were found with unsealed cable penetrations. Make penetrations watertight.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Macerator pump was found installed with only zip ties. Properly mount macerator pump.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Galley bilge was found with excessive trash and debris. Remove items and debris.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit provided did not meet 46 CFR 160.041. Provide complete and current first aid kit.

Spanner wrenches must be provided where a 40 millimeter (1.5 inch) diameter firehose is required by § 181.320. Both fire hydrants were missing wrenches. Provide spanner wrenches.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] piping located in the starboard aft corner of the engine room was found leaking at a coupling. Repair leak.

Cable and wire for power and lighting circuits must: (1) Meet Section 310-13 of NFPA 70 (incorporated by reference; see 46 CFR 175.600) except that asbestos insulated cable and dry location cables may not be used; (2) Be listed by Underwriters Laboratories (UL), as UL Boat or UL Marine cable; or (3) Meet § 111.60-1 in subchapter J of this chapter for cable, and § 111.60-11 in subchapter J of this chapter for wire. Engine start batteries were found with battery jumper cables using spring clips connecting the two banks. Vessel representative replaced jumper cables with 0000 cable and proper terminal connectors, but cable did not meet 46CFR183.340 amd was not what was listed in plan bill of materials. Replace with UL-marine or other approved cable.

When the cognizant OCMI or the Commanding Officer, Marine Safety Center determines, in accordance with §178.210, that a vessel must have stability details indicated on its Certificate of Inspection or a stability letter, the owner or operator must submit the information listed in paragraph (b) of this section: Provide an approved stability letter noting stability details for a minimum of 50 passengers, 4 crew and cargo.

Pursuant to 46 CFR 170.010, substitutions for calculations, information, or tests required in sub-chapter T may be approved by the Officer in Charge, Marine Inspection, if the substitution provides an equivalent level of safety. While the vessel is carrying more than 50 total persons the maximum cargo load must be reduced to no more than 15,000 lbs.

Replace lagging on port and starboard engine exhuast. Found 5X& holes in both outboard side exhaust lagging.

Found electrical panel to be missing drip shield in generator room. Provide drip shield for panel B in generator room.

Provide spanner wrenches for fire hydrants.

Electrical breaker panel on the bridge had a breaker that did not fit inside the face of the plate and was exposing the inside of the panel.

electrical panel in the number two forward void under the galley was missing two screws and the front cover was swinging open, exposing the wires inside.

An electrical cord with semi exposed wires made with a temporary type splice, held together with only electrical tape, was utilized on the bridge to extend the length of an extension cord for an installed fan.
