The whistle, bell and gong shall comply with the specifications in Annex III to these Rules (33 CFR part 86). Vessel was not provided with a bell. Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space and may require the vessel to be dry docked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Fractures were found in the forepeak in way of the 5th and 6th longitudinal from the deck on the starboard side at the gussets aft of the first transverse frame, as well as at the first frame and the deck stiffener joint on the port side. Provide repair proposal and complete repairs to the satisfaction of the attending USCG marine inspector.

The vessel was involved in a marine casualty on 20NOV2021 during which it sustained damage to the bow sideshell and port inboard running gear. Additional damage to running gear and hull beneath the waterline is unknown. Prior to moving the vessel, provide a damage assessment of the vessel's hull, running gear and associated components.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel hull plating sustained a 2' x 6" longitudinal gash in way of the port forepeak space 2nd frame, approximately 6" above the chine. Provide repair proposal to include cropping and renewing damaged plate. Upon approval of repair proposal, conduct repairs to the satisfaction of the attending USCG marine inspector. Vessel may transit within the Sector LA/LB zone in fair weather and without passengers or cargo to conduct repairs.

Port inboard shaft connection bolts sheared at the engine coupling, causing shaft to slide back and damage the stuffing box. Unknown additional damage exists to the running gear below the water line. Following the damage assessment required by deficiency 01, provide repair proposal and conduct repairs to the satisfaction of the attending USCG Marine Inspector. Vessel may transit within the Sector LA/LB zone in fair weather and without passengers or cargo to conduct repairs.
