shall be centralized on the navigation bridge or in the main
fire control station, which shall be so manned or equipped as
to ensure that any alarm from the system is immediately
received by a responsible member of the crew. PSCO
observed smoke detector C0775 did not activate locally or
on the bridge during testing.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready or immediate use. Lifeboat #2 and lifeboat #6 were found with exhaust leaks. The exhaust leaks were such that
excessive smoke was accumulating in the engine
compartment and throughout the lifeboat.

Category 10 space fan room # 7-2 contained carboardboxes amongst other combustible material. In addition, the room had a “workshop” table with tools and a chair for maintaining equipment.

While conducting the engine room portion of the exam, PSCO observed several cable transits located on watertight bulkheads in the engine room which were improperly installed with two manufacturer's types in one A boundary metal sleeve.

Code 15113: Safety management objectives of the Company should provide for safe practices in ship operation and a safe working environment. PSCO identified multiple ongoing projects in the engine room (boiler #1 re-tubing, diesel generator #3 overhaul and a fuel split project). These multiple projects led to the storage of new and old parts, palletized equipment, large 4-5 foot toolboxes and other equipment to be stored in walkways and in escape routes leading to two escape trunks, partially blocking the walkway to them. Large equipment was ratchet strapped in place following PSCO identification, but remain in engine spaces. Additionally, to be installed equipment is stored in the I-95 passageway awaiting installation. ISM A.1.2.2.1, 16C.
