In passenger ships such units shall give an indication of any fire and its location in any space served by the system and shall be centralized on the navigation bridge or in the main fire control station, which shall be so manned or equipped as to ensure that any alarm from the system is immediately received by a responsible member of the crew. PSCO observed smoke detector C0775 did not activate locally or on the bridge during testing. 74 SOLAS 01 II-2/12.1.2.1

Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements. The doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.58 opposing closure, and shall have an approximately uniform rate of closure of no more than 40 s and no less than 10 s with the ship in the upright position. PSCO observed fire doors 3790 and 3759 unable to close. 74 SOLAS 97 II-2/30.4.1

Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements. The doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.58 opposing closure, and shall have an approximately uniform rate of closure of no more than 40 s and no less than 10 s with the ship in the upright position. PSCO observed multiple fire doors within varying MVZs close quicker than 10 seconds. 74 SOLAS 97 II-2/30.4.1

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready or immediate use. Lifeboat #2 and lifeboat #6 were found with exhaust leaks. The exhaust leaks were such that excessive smoke was accumulating in the engine compartment and throughout the lifeboat. 74 SOLAS 20 III/20.2 16ac

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. Category 10 space fan room # 7-2 contained carboardboxes amongst other combustible material. In addition, the room had a “workshop” table with tools and a chair for maintaining equipment. 74 SOLAS 97 II-2/26.2.2

Obstructions shall not be permitted in passenger and crew corridors forming escape routes. PSCO discovered multiple waste receptacles in escape routes. 74 SOLAS 97 II-2/41-2.4.10 10c

Fire protection systems and fire fighting systems and appliances shall be maintained ready for use. PSCO observed high pressure water mist system nozzle obstructed by the overhead forward of steering gear in the reefer area. The nozzle was installed i such a way that it was mostly inside the overhead/ceiling. 74 SOLAS 20 II-2/14.1.1 10c

Fire protetion systems and fire fighting systems and appliances shall be maintained ready for use. PSCO observed fire extinguisher and hydrant not readily available due to being obstructed by multiple ladders within a fie station. 74 SOLAS 20 II-2/14.1.1 10c

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired. While conducting the engine room portion of the exam, PSCO observed several cable transits located on watertight bulkheads in the engine room which were improperly installed with two manufacturer's types in one A boundary metal sleeve. 74 SOLAS 97 II-2/18.1.1 50 ac

07120-Doorways and corridors and intermediate landings included in means of escape shall be sized in the same manor as the stairways. PSCO observed escape door partially obstructed by an arcade machine bolted to the floor. Cite 74 Solas 97 II-2/28.1.5.3 action taken 10c

Code 15113: Safety management objectives of the Company should provide for safe practices in ship operation and a safe working environment. PSCO identified multiple ongoing projects in the engine room (boiler #1 re-tubing, diesel generator #3 overhaul and a fuel split project). These multiple projects led to the storage of new and old parts, palletized equipment, large 4-5 foot toolboxes and other equipment to be stored in walkways and in escape routes leading to two escape trunks, partially blocking the walkway to them. Large equipment was ratchet strapped in place following PSCO identification, but remain in engine spaces. Additionally, to be installed equipment is stored in the I-95 passageway awaiting installation. ISM A.1.2.2.1, 16C.

Code 07120: In addition to the emergency lighting required by regulations II-1/42 and III/11.5, the means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators. Exit sign shall be marked by lighting or photoluminescent strip indicators. Exit sign located in the backstage of the theater was broken and illuminated. 74SOLAS(97)II-2/28.1.10, 10ac.

IT SHALL BE POSSIBLE FOR EACH DOOR TO BE OPENED AND CLOSED FROM EACH SIDE OF THE BULKHEAD BY ONE PERSON ONLY. SWTD 1.18, DECK 1, COULD NOT BE OPENED USING THE LATCH FROM EITHER SIDE. THE DOOR COULD ALSO NOT BE OPENED FROM ONE SIDE IN THE POWER CLOSE MODE, PREVENTING EGRESS FROM BUNKER STATION. 74SOLAS (2020 CONS.) CH II-2 REG 9.4.1.1.4

Port side lifeboat #16 was not tested due to oil leak on hydraulic system vessel is to make repairs and provide class report to MSU Galveston Prior to next US port after sailing foreign.

Bunker samples are being stored in a temporary location. Provide a permanent location for the bunker samples large enough to hold one years worth of samples.

Provide brackets attached to bulkhead for video machines in arcade room deck 12.

Remove mechanical holdbacks on fire screen door 1852 - entrance to refrigerated storeroom .

Provide flag state acceptance of chemical storage within hotel store room as liquid store room. Ensure all fire control plans reflect final approved space utilization.

Prior to departure , demonstrate proficiency of certificated lifeboatmen with regards to location and operation of emergency equipment onboard lifeboats.

Main lint box B boundary wall panel found missing in main laundry. Temporary repairs have been made this date, permanent repairs to be verified by DNV upon arrival Miami no later than Oct 30, 2004.

Deck 8 bell box fwd interior door is missing door handle and has improper locking arrangement that prevents a person from escaping from within the spcae. This to be permanently recitfied to satisfaction of DNV upon arrival Miami no later than Oct 30, 2004.

Vessel will moor so that all STARBOARD lifeboats can be launched/recovered during next ACVE.

Vessel shall lower starboard side lifeboats in presence of USCG.

Port side lifeboats #8 and #6 experienced a slow rate of descent. The brakesystem required lubrication.
