Each shore connection box must have a bottom entrance for the shore connection cable. The box must provide protection to the shore connection when the connection is in use. During the annual inspection, vessel had a nema enclosure junction box that had lost is watertight integrity due to a broken fitting from one of the conduits, which was no longer attached. Additionally, the starboard forward motor's armored cable was improperly secured; thus, exposing the cables to the environment and jeopardizing the explosion proof integrity of the equipment. Furthermore, one of the shore tie boxes cover was broken. Vessel shall enact permanent repairs to all equipment above mentioned to the satisfaction of the Coast Guard Marine Inspector. 46 CFR 111.83-5

Electrical connections need new fittings per 46 CFR Subchapter J 111.81-3 . Current fittings are wasted to the threads on connectors. May have one load, and after the discharge must repair to the satisfaction of the attending Coast Guard Marine Inspector.

MAKE PERMANENT REPAIRS TO WELD SEAM IN VOID #1 STBD BOTTOM PLATE IWO KNUCKLE APPROX. 10 FEET AFT OF COLLISION BULKHEAD TO THE SATISFACTION OF THE COGNIZANT OCMI.

Repair port discharge valve packing leak.

Repair bow hatch to open properly.

Replace/repair dog on #3 starboard hatch.

Make permanent repair to hole/crack in #3 starboard void/hull.

Renew as required throughout the vessel all failed or missing flame safety screens prior to the transfer of any flammable or combustible cargo.

Provide onboard valid and appropriate Certificate Of Inspection prior to engaging in the carriage of regulated cargoes.

Effect permanent and proper repairs as required to starboard Cargo Tank Pump motor controller switch housing and faceplate.

Repair as required numerous broken or missing scuttle dogs throughout the vessel.

PROPERLY REPAIR FRACTURE IN #1 STBD CARGO TANK TO SATISFACTION OF THE ATTENDING MARINE INSPECTOR PRIOR TO CARRIAGE OF CARGO.

EFFECT PERMANENT REPAIRS TO DAMAGE IN UNDERSIDE OF MAIN CARGO DISCHARGE HEADER, IN WAY OF CONNECTION WITH #1 CARGO TANK PORT AND STBD PIPING.

RENEW WASTED PLATING IN CARGO DRIP PAN PRIOR TO CONDUCTING CARGO OPERATIONS AND IN NO CASE LATER THAN THE COMPLIANCE DATE.

RENEW ALL WASTED METAL IN WAY OF ELECTRICAL MOUNTING BRACKETS PRIOR TO CONDUCTING CARGO OPERATIONS AND IN NO CASE LATER THAN THE COMPLIANCE DATE.

RENEW STARBOARD EXPLOSION PROOF FIXTURE IN WAY OF AMPMETER PRIOR TO CONDUCTING CARGO TRANSFER OPERATIONS AND IN NO CASE LATER THAN THE COMPLIANCE DATE.

REPLACE PORT CARGO PUMP ELECTRICAL CABLE PRIOR TO CONDUCTING CARGO TRANSFEROPERATIONS AND IN NO CASE LATER THAN THE COMPLIANCE DATE.

MAKE PERMENANT REPAIRS TO VAPOR RECOVERY SYSTEM IN WAY OF WASTED THROUGH SECTIONS TO BE ACCOMPLISHED TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR (CG) AND PRIOR TO LOADING OF VESSEL.

UPON RE-INSTALLING BOTH PORT AND STBD CARGO PUMPS AND ELECTRICAL DRIVE MOTORS AND PRIOR TO UTILIZING THE SAME FOR CARGO TRANSFER OPERATIONS CONTACT COAST GUARD FOR INSPECTION OF PROPER INSTALLATION. (ALL ELECTRICAL WIRING AND CONNECTIONS I.A.W. 46 CFR ELECTRICAL REGULATIONS)
