Each inflated rescue boat must be repaired and maintained in accordance with the manufacturer's instructions. All repairs to inflated chambers must be made at a servicing facility approved by the Commandant, except for emergency repairs carried out on board the vessel. After running the rescue boat boat for tests, the fwd chamber of the sponson was ripped. Repair / replace damaged sponson in accordance with manufactures instructions to the satisfaction of the attending marine inspector.

The rescue boat must meet the embarkation and launching arrangement requirements of §§ 199.110 (e) and (g), 199.150, 199.155, and if the launching arrangement uses falls and a winch, § 199.153. While raising the rescue boat back on-board the vessel the line from the rescue boat to the winch snapped. Replace winch line with proper strength and length requirements. A weight test will be required prior to being placed back in service.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. Starboard engine high temp alarm sensor broke when trying to extract from drywell. Replace temp sensor and test alarm to the satisfaction of attending marine inspector.

A survey shall be made every time an accident occurs or a defect is discovered that affects the safety of the vessel. A leak beyond what is "normal" was discovered in the port shaft seal. The leak, while being more than normal is not at the point of causing an unsafe condition. Repair shaft seal leak to the satisfaction of the attending Coast Guard Marine Inspector no later than 7/25/25. As a temporary fix to continue service until adequate repairs are made, the following must be implemented: 1. The equipment needed to engage the shaft seal air bladder must be ready for use. 2. The on duty licensed engineer must observe the port shaft area for the duration of each transit from bank to bank of the Calcasieu River. Notify this office once repairs have been satisfactorily made or if the current seal status worsens.

An efficient means of communication shall be provided between the wheelhouse and the bow or forward look out station. During a fire drill it was observed that the deck crew could not effectively communicate an emergency on the main deck to the wheelhouse via voice communications. Install or implement an effective means of communications between the main deck crew and the wheelhouse, to the satisfaction of CG marine inspector.

A survey shall be made every time an accident occurs or a defect is discovered that affects the safety of the vessel. Vessel was dry-docked in Houma, La with two deficiencies requiring hull and keel cooler repairs be made to the satisfaction of an attending CG marine inspector. No objective evidence could be provided to MSU Lake Charles that shows the repairs were made to the satisfaction of a CG marine inspector. Vessel is to be removed from service until a Coast Guard marine inspector is satisfied that proper repairs have been made. The vessel is not permitted to depart Cameron, La unless a deadship tow plan/application is submitted to MSU Lake Charles and approved by MSU Lake Charles.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel keel cooler was reported to be leaking under the vessel in the water. Repair keel cooler to the satisfaction of a Coast Guard Marine Inspector.

A survey shall be made every time an accident occurs or a defect is discovered that affects the safety of the vessel. A leak was discovered in the starboard side rudder post seal resulting in leakage into the lazarette. Effect repairs and conduct operational tests following assessment by technician scheduled to attend no later than 01/17/2025 and notify Coast Guard Marine Safety Unit Lake Charles upon completion of repairs or notable increase in water ingress.

A survey shall be made every time an accident occurs or a defect is discovered that affects the safety of the vessel. After unintentional collision with the east bank landing ramp, vessel sustained damage to the stbd bow area. Due to being a confined space, MIs were not able to enter the space to assess the extent of damage. Provide a marine chemist certificate that shows the space is safe for entry and coordinate with MSU Lake Charles to return to enter the space within 24 hours of the certificate being issued so that a complete survey can be completed. An examination of the space shall be done by a CG marine inspector prior to the vessel departing the dock but not later than 27NOV24.

A survey shall be made every time an accident occurs or a defect is discovered that affects the safety of the vessel. MSU Lake Charles marine inspectors discovered a 1.5 inch crack in the bottom plating of the vessel in way of the shaft alley port side, aft of the third transverse structure and between the 2nd and 3rd longitudinal. A permanent repair in the form of a doubler plate shall be made as proposed. As part of the temporary repair, a crew member shall perform at least an hourly round of the entire shaft alley space to assess any further intrusion of water. These rounds, and findings, to include negative findings, shall be logged. The next time the vessel is pulled from the water (regardless of the reason) a permanent repair and further assessment of the steel including gauging, if needed, shall be made to the satisfaction of the attending marine inspector.

A survey shall be made every time an accident occurs or a defect is discovered that affects the safety of the vessel. The shaft binding failed causing damage to the shaft guard, shaft tack, clutch oil pipe and brackets. It was found that a bracket used in the binding failed and caused the chain to wrap around the shaft. Make repairs and conduct operational tests to the satisfaction of attending marine inspector.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Three of the four turbos were leaking oil from the turbo exhaust onto the hot surface of the engine exhaust posing a significant fire hazard. Make repairs to the satisfaction of attending marine inspector.

Small rooms or spaces having a secondary means of escape which is not obviously apparent shall have a suitable sign in red letters "EMERGENCY EXIT" directing attention to such escape. The emergency exit in the steering compartment is not obvious and is not marked. Mark in the space at the exit to the satisfaction of the attending marine inspector.

Each passenger vessel must have a public address system capable of broadcasting seperatley or collectively to public spaces used for passenger assembly points and survival craft locations. It was noted during a simulated fire on the main deck (vehicle area) that the vessel's public address system was inoperable and the master was not able to effectively communicate with the crew. Make repairs and test to the satisfaction of the attending marine inspector.

46CFR71.40-1 "A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel.............. Under this activity the ACADIA has undergone various repairs to the vessel caused by serious corrosion issues documented by pictures in the document file. With the repairs completed and the COI issued there are still follow up inspections/monitoring of the vessel for future corrosion. IAW the proposal letter Dated 27Sep24 titled "Response 2 to USCG Sector New Orleans Acadia Corrosion Root Cause" in the document section of MISLE. Section 1 immediate testing with the vessel back in Cameron on both banks for electrical grounding problems. Section 2 Continued monitoring conducting the same tests as in Section 1 every Quarter. Noting any grounding. Section 3 to dry-dock the vessel when service in Cameron is complete or 18 months which ever comes first. With all testing notification shall be made to the Local OCMI of the testing results, issue encountered and corrective action. This "work-list 835" shall remain active until 10Apr26 (18 Months as per proposal letter).

Subpart 71.40 - Inspection After Accident§ 71.40-1 General or partial survey.(a) A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Complete a Root Cause Analysis for the shaft failure and apparent excessive wastage. The analysis needs to include a metallurgical analysis of the shafts, and a shaft alignment of the shaft tubes. Also why the propeller nut was missing on the remaining shaft. To be completed during the current drydocking of the vessel.

§ 78.17-50 Emergency training, musters, and drills.Onboard training, musters, and drills must be in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Vessels COI issued today 10Oct24, with no crew on board for drills, upon arrival at destination for operations contact the local OCMI to come out and witness all required drills prior to placing the vessel into service carrying passengers. To be completed within 30 days, 10Nov24.

The installation of all systems of an electrical engineering or interior communication nature shall be in accordance with requirements of Subchapter J (Electrical Engineering) of this chapter. Systems of this type include sound powered phones. Marine Inspectors discovered an inoperable sound powered phone in the engine room. Repair to the satisfaction of Coast Guard Marine Inspectors.

Auxiliary machinery shall be installed and maintained in accordance with 46 CFR Subchapter F. Vessel representative reported the starboard ship service generator shutdown due to a faulty wiring harness. Repair ship service generator to the satisfaction of the attending USCG Marine Inspector.

Auxiliary machinery shall be installed and maintained in accordance with 46 CFR Subchapter F. The starboard ship service generator had a leak on top aft of the engine located on a fuel line. Repair or replace the cause of the leak to the satisfaction of the attending USCG Marine Inspector. 46 CFR 70.20-1/46 CFR 58.10-10

Each Class 1 watertight door must have a quick action closing device operative from both sides of the door. The aft steering room emergency hatch had light penetrating the seal over fifty percent of the knife's edge and gasket material. Repair or replace to the satisfaction of the attending USCG Marine Inspector. 46 CFR 72.30-1/46 CFR 170.270(a)

Periodic safety tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Unable to verify safety tests on the ship service generators. Rectify to the satisfaction of the attending USCG Marine Inspector. 46 CFR 70.20-1/46 CFR 61.40-6

Propulsion systems should installed and maintained in accordance with 46 CFR Subchapter F. Vessel had a reduction in RPMs and an error code appeared indicating a loss in fuel pressure for the starboard main engine. Make repairs or replace appropriate sensors.

Steering systems should installed and maintained in accordance with 46 CFR Subchapter F. A loss of hydraulic steering occurred due to the hydraulic filter backing off. Attach filter and retest steering gear.

As the result of an allision with the ferry loading ramp due to the loss of steering, the 100 person IBA canister was significantly damaged. Replace with appropriate size and type of survival craft.

In a space where the general emergency alarm signal cannot be heard over the background noise, there must be a red flashing light or rotating beacon in addition to the general alarm signal. The light located in the machinery space is not operational and under normal operating conditions, the general alarm can not be heard. Repair and prove proper operation of general alarm light. 46 CFR 113.25-10(a)

Except as provided in 47 CFR 80.13(c), stations must have an authorization granted by the FCC (station license). The station license provided onboard is a fleet license. A FLEET license may be issued provided the licensee keeps a current list of vessel names and registration numbers authorized by the fleet license. Provide the required list that shows this vessel is authorized on the fleet license or provide an FCC station license specific to this vessel. 47 CFR 80.55(b)(1)

A person may not employ or engage an individual in a position required to hold an MMC unless that person maintains a current medical certificate. The Master assigned to the vessel could not produce a valid medical certificate. Provide appropriate licensed/credentialed Master prior to getting underway. 46 CFR 15.401(c)

Operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license. The operator is responsible for the proper operation of the station. Master could not produce a valid FCC Radio Operator's Permit. Provide permit as required. 47 CFR 80.155

A self-propelled vessel of 65 feet or more in length, engaged in commercial service must have on board a properly installed, operational AIS Class A device. AIS lcd screen is damaged and unreadable. Repair/replace AIS display.

The applicable standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), may be used as the standard for the design, construction, and testing of main and auxiliary machinery except as modified in this subchapter. Pilothouse digital gauges for port and starboard engine rpm and shaft tachometers are not displaying properly. Repair/replace digital tachometers.

Auxiliary machinery vital to the main propulsion system must be provided in duplicate unless the system served is provided in independent duplicate, or otherwise provides continued or restored propulsion capability in the event of a failure or malfunction of any single auxiliary component. Electric driven air compressor failed. Renew air compressor to the satisfaction of the attending Marine Inspector.

During COI inspection the full crew was not onboard to perform drills. Prior to carrying passengers, man overboard, fire, and abandon ship drills must be conducted with the full crew in the presence of the USCG.

Vessel reported damage to their shaft cover 6" before the cutlass bearing assembly. No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel will submit a repair proposal and tranist plan for approval to the OCMI.

46 CFR 61.20-3(a) The high temperature alarm on the main engines and the over speed trip of the generators were unable to be tested.

46 CFR 111.30-15 The 230 volt battery ground detector bulbs are out.

46 CFR 112.15(d) The marine inspector discovered that the emergency lights on the forward starboard and stern center on the open deck were not working.

46 CFR 199.70(b)(3)(ii) Lifejackets in the on-deck storage bins were found with inadequate retro-reflective material.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. Starboard propulsion shaft coupling flange is coming loose from shaft. Vessel shall proceed under tow to Bollinger Quick Repair in Harvey, LA for emergency drydock to affect permanent repairs to starboard propulsion shaft to the satisfaction of attending Marine Inspector.

A card of instructions giving a clear summary of the radiotelephone distress procedure must be securely mounted and displayed in full view of the principal operating position. The emergency broadcast placard is not posted at each radio station. Post the emergency broadcast placard at each radio station. 47 CFR 80.868

All general alarm bells shall be identified by red lettering at least 1/2 inch high: "GENERAL ALARM-WHEN BELL RINGS GO TO YOUR STATION." The general alarm bells are not identified in accordance with 46 CFR 78.47-7. Identify each general alarm bell in accordance with 46 CFR 78.47-7.

Fire pumps shall be fitted with a pressure gauge on the discharge side of the pumps. The pressure gauge on the discharge side of the pump did not display the pressure when the fire pump was activated. Replace gauge with a properly operating pressure gauge. 46 CFR 76.10-5

All valves and closing appliances required to be operated for damage control purposes in case of emergency shall be conspicuously marked with letters at least 1 inch high identifying the control and the direction of operation. Two valves on the bilge suction manifold are not marked. Mark the valves in accordance with 46 CFR 78.47-38(a).

Minimum system monitoring and safety control requirements for this vessel require bilge high level alarms in propulsion spaces. The mounting bolts for the bilge high level sensor in the steering room are broken. Sensor is operating using temporary mounting hardware. Make permanent repairs to sensor.

Passenger vessels are required to have FCC Station License. Vessel has valid FCC Station License, but there is no copy onboard. Place a copy of the FCC Station License onboard the vessel.

Electrical installations on vessels must ensure protection of crew from electrical hazards. Place cover over junction box under wheelhouse console.

Emergency lighting must be provided to allow crew to find their way to survival craft and embarkation stations. Emergency lights on deck aft of wheelhouse were not operating.

All alarm panels must be continuously powered. Battery back-up for engineering alarms was not operating.

Required alarms in high ambient noise areas must be supplemented by visual means that are visible throughout these areas. Yellow alarm light was not operating.

Emergency lighting must be provided in machinery spaces and escape routes to allow crew to find their way to open decks. Emergency light in space between engine room and steering space (escape route) was not operating.

Each machinery space must be ventilated to ensure that, when machinery is running at full power, an adequate supply of air is maintained for operation of machinery and comfort of crew. Breaker for upper engine room ventilation will not energize. Repair breaker to allow ventilation to operate.

Electric installations on vessels must ensure maintenance of services necessary for safety under all conditions. UV breakers for ventilation show signs of possible failure in the near future which would render breaker inoperable. Replace breakers and conduct review of component installation to ensure proper installation.

Gears, couplings, and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Shaft of the engine driven fire pump must be fully guarded from accidental contact with personnel.

Piping subject to internal head pressure from oil in the tank must be fitted with a positive shut-off valve located at the tank that can be manually operated from a safe and accessible position outside of the compartment. Vessel has operable remote valves in place, but they are not properly labeled to distinguish them from other remote valves that are no longer in service. Remove old valves or ensure that the active remote valves are properly marked and distinguishable to the crew.

All fire extinguishing equipment must be checked to ascertain that the system is in proper operating condition. Mechanical seal of electric fire-pump leaks excessively.

Approved fire extinguishing systems must be installed in internal combustion engine machinery spaces. Timer delay bottle was approved/rated to actuate supply valve at 30 seconds (in accordance with onboard procedures), but timer delay actuated early (at 26 seconds). Replace or repair timer bottle so that actuation occurs at rated time stamped on bottle (with an allowance of +20%/-0% of stamped time). If new bottle time stamp is anything other than 30 seconds, ensure that onboard procedures are revised to reflect the expected delay time.

Each machinery space must be ventilated to ensure that, when machinery is running at full power, an adequate supply of air is maintained for operation of machinery and comfort of crew. Breaker for upper engine room ventilation will not energize. Repair breaker to allow ventilation to operate. 02 MAY 2019: **UPDATE: 8-week lead time on parts. Due date extended to 27 JUN 2019**

Safety relief valves for air service shall be provide with a lifting device. Relief valve lifting device on start air receiver was not operating at time of inspection.

Non-metallic flexible hose must be in accordance with SAE J1942. Fuel hose on port main diesel engine was chaffed to inner steel braid and needs to be replaced.

Spaces having a secondary means of escape which is not obviously apparent shall have a suitable sign in red letters "EMERGENCY EXIT" directing attention to such escape. Emergency escape route through steering space and shaft tunnel is not suitably marked (attention is directed to an inoperable escape hatch, not the intended escape hatch. Repair escape markings.

Each opening in a watertight bulkhead must have a means to close it watertight. Emergency escape scuttle in steering room needed excessive effort to open and close. Ensure proper operation of watertight scuttle.

Instructions shall be posted relating in order the different steps to be taken in changing to emergency steering. Post emergency steering instructions at the emergency steering station located at the engineering control console. 02 MAY 2019: **UPDATE: Instructions were posted at HPU for steering gear for shifting to manual steering, but no instructions were posted at the ECC like initially intended by deficiency. Also, Rudder Angle Indicator at the Emergency Steering console at ECC was not operating. Added requirement to fix RAI before clearing this deficiency. Due date extended to 16 MAY 2019.

Install cotter pins or other suitable device on jockey bar pin.
