Except as provided by paragraph (b) of this section, each navigation light must—(1) Meet the technical standards of the applicable Navigation Rules. Vessels navigation lights when switched to forward or aft position energize the port, starboard, and stern light all on the same side. The stern light should be energized on the opposite position as the fwd facing port and starboard navigation lights.

It shall be the duty of the owner, master, or person in charge to require and have performed at least once in every twelve months the tests and inspections of fixed fire extinguishing systems on board, as described in tables 71.25-20(a). Vessels fixed CO2 system inspection and maintenance report showed it was last completed 06DEC2024. Annual servicing of the installed fixed CO2 system is required.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Rescue boat was not on board at time of inspection. Complete required drills and prove proper operation to the satisfaction of the attending marine inspector.

Fire hazards are to be minimized insofar as is reasonable and practicable. Vessel's Forward Port Side RACOR was leaking fuel oil into the bilge.

Fire hazards are to be minimized insofar as is reasonable and practicable. Vessel's Forward Port MDE Lube Oil pump was leaking into the bilge. Vessel shall clean bilge prior to departing dock.

The outlet at each fire hydrant must be provided with a cock or valve that the firehose may be removed while the fire main is under pressure. Fire Station #2 valve was leaking while the fire main was under pressure.

Equipment shall be maintained and ready for use at all times and replace as indicated. Fire Station #19 required to be serviced as evidence by the gauge showing the arrow in the red. No spare was on-board.

Non-conducting deck coverings must be installed for personnel protection at the front and rear of the switchboard. The non-conducting deck covering was damaged by a large crack and requires replacement.

The COI issued by the OCMI specifies the minimum complement of officers and crew necessary for the safe operation. Captain, Chief Engineer, and crew were not available during the re-inspection.

The inspector shall examine all accessible parts of the vessel's hull, machinery and equipment to be assured it is in satisfacty condtion. MIs were not able to assess propulsion or bridge controls.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessel did not have alcohol testing devices on board.

The scope of the reinspection shall be the same for the annual inspection, but in less detail. Crew was not available to preform requried drills.

Each crewmen must participate in at least one abandon ship drill and one fire drill every month. Crew was not logging drills properly or preforming the abandon ship drills monthly. Educated LA DOTD manager on requirements.

All lifesaving appliances must be check IAW manufacturer instructions and per the required (weekly, monthly, annual inspections). Vessel did not have logs for rescue boat engine testing, lifesaving inspection, etc. Educated LA DOTD manager on requirements.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment shall be made with the knowledge of the OCMI. The number four MDE was overheating due to the #4 piston sleave being cracked. Make repairs and prove proper operation prior to carrying passengers but no later than 14 days which ever is first.

The installation of all systems of an electrical engineering or interior communications nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J. Each self-propelled vessel must have duplicate light sources for the side, masthead, and stern lights. The starboard side light for forward operation and the starboard side light for stern direction secondary navigation light were not operational. The primary light for both are operating properly. Prove proper operation of the required primary and secondary light source. 46 CFR 111.75-17(c)

The installation of all systems of an electrical engineering or interior communications nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J. An emergency means of communication required by this subpart must be a reliable means of voice communication and must be independent of the vessel’s electrical system. The installed sound powered phones were not operational. Prove proper operation of sound powered phone system. 46 CFR 113.30-3(b)

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel reported that the starboard engine lost engine RPM control and started to run away. John Deere technicians are going to troubleshot the system to figure out what caused the incident. Vessel will not operate until condition of the STBD generator is resolved to the satisfaction of the Coast Guard.

A receptacle outlet must not have any exposed live parts with the plug opening uncovered. Electrical outlet found in pilot house forward of the operating station was found to be not functioning as designed. Crew is to repair / replace outlet to the satisfaction of the attending marine inspector.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel representative reported a vehicle tire lodged in #3 propulsion thruster. Remove obstruction and provide tech report attesting to the proper operation of thruster. Vessel prohibited from the carriage of passengers until repairs are completed to the satisfaction of the Marine Inspector.

The installation of all systems of a marine engineering nature shall be in accordance with requirements of Subchapter F (Marine Engineering) of this chapter. Systems of this type include steering systems and power for going astern. Vessel experienced a loss of propulsion due to an undetermined failure. Since there is a communication failure between throttles and main propulsion, vessel will be required to conduct a Failure Mode Effect Analysis (FMEA) for propulsion and control automation. Repairs and analysis shall be conducted to the satisfaction of Coast Guard Marine Inspectors prior to the carriage of passengers but no more than thirty days.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Vessels newly installed Z-Drives had fault alarms not allowing vessels system to clutch in during sea trials.

At each annual inspection, the inspector shall conduct the following tests and inspections of hull equipment: All watertight doors shall be operated locally by manual power and also by hydraulic or electric power if so fitted. Vessels Class I watertight doors were unable to seal on gasket due to door being warped.

Each section of firehose must be a lined commercial firehose that conforms to UL 19 (incorporated by reference, see § 76.01-2). A hose that bears the label of UL as a lined firehose is accepted as conforming to this requirement. Vessel's fire hose on main deck had a pin hole.

Provide common talking means of two-way voice communication and calling among the navigating bridge, emergency control stations, muster stations, embarkation stations, and other strategic positions listed in 113.30-5. Vessels Aft machinery space sound power phone was inoperable throughout the vessel to include the navigation bridge.

The annual inspection shall include an inspection of the structure, boilers, and other pressure vessels, machinery and equipment. The inspection shall be such as to insure that the vessel, as regards the structure, boilers and other pressure vessels, and their appurtenances, piping, main and auxiliary machinery, electrical installations, life-saving appliances, fire-detecting and extinguishing equipment, pilot boarding equipment, and other equipment is in satisfactory condition and fit for the service for which it is intended...Upon arrival, it was determined the vessel was not prepared for the annual inspection. Company shall ensure vessel is inspection ready prior to scheduling an inspection for certification (annual inspection).

All vessels except unmanned barges shall be provided with a satisfactory bilge pumping plant capable of pumping from and draining any water-tight compartment. The bilge suction/overboard pipe in the SSG space is partially damaged by rubbing against the MSD tank top edge. Make repairs to the piping and make appropriate adjustments to prevent damage from reoccurring. Repairs can be made at upcoming drydock period. 46 CFR 56.50-50.

All hand portable fire extinguishers and semiportable fire extinguishing systemsshall be chacked as noted in table 71.25-20(a)(1). Some portable extinguishers were tagged indicated that they were inspected in April of 2020 vice March of 2020 as per the third party report. Portable extinguishers marked with a future date (April 2020) shall be reexamined and marked with an appropriate inspection tag. 46 CFR 71.25-20(a)(1)

The minimum instrumentation, alarms, and safety controls as per Table 62.35-50. Local indicator panel lights for low lube oil pressure and low water level are extinguished. Replace bulbs.

Hose end fittings must comply with SAE J1475. Forward E/R steering pump return line leaking hydraulic fluid. Repair/renew steering hose and clean all fluid from bilge.

At each annual inspection, the inspector shall examine Bilges for accumulation of oil. All bilges had oil soaked rags under machinery and steering gear. Remove rags and clean bilges free of oil.

Each rescue boat must have navigation lights in compliance with the applicable sections of Nav Rules. Forward combination light is inop. Repair/replace light.

The minimum instrumentation, alarms, and safety controls as per Table 62.35-50. When an alarm for #3 generator is activated, the alarm panel in the Generator room indicates an alarm in fwd E/R. The generator is in the aft engine room. Repair alarm panel to indicate a fault in the correct engine room.

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this sub-chapter. The #4 Z-drive suffered an internal failure upon start up after being shutdown for routine ship maintenance. #2 Z-drive was found to be making unusual noise as well. Repair/replace Z-drives and operationally test to the satisfaction of the attending Marine Inspector, prior to carrying passengers.

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this sub-chapter. Water pump was replaced on #3 engine. Pressure tested keel cooler to 25psi and found cooler losing approx. 1psi per minute. Suspect hole in cooler. Provide transit plan to drydock vessel and make available for further inspection. Provide repair proposal and make repairs/renew of cooler to the satisfaction of the attending Marine Inspector, prior to carrying passengers.

Electrical installations on a vessel must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. The number three "auxiliary" generator has exposed mounting studs on the alternator, creating a potential shock hazard. 46 CFR 111.01-1(a)(2)

At each inspection for certification and periodic inspection the marine inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as he feels necessary to check safe operation. Prove proper operation of the over-speed, high jacket water temperature and low oil alarms on the main propulsion engines and ship service generators. 46 CFR 61.20-3(a)

Fire pumps may be used for other purposes provided at least one of the required pumps is kept available for use on the fire system at all times. In no case shall a pump having connection to an oil line be used as a fire pump. Branch lines connected to the fire main for purposes other than fire and deck wash shall be arranged so that the requirements of paragraphs (b) and (c) of this section and any other services installed on the fire main can be met simultaneously. Valves feeding the fire hose stations were found close. Take precautions to ensure those valves are not accidentally closed. 46 CFR 76.10-10(f)

13199 - THE # 3 Z-DRIVE EXPERIENCED KNOCKING AND VIBRATION WHILE MANEUVERING UNDERWAY. REPAIR OR REPLACE # 3 Z-DRIVE TO ENSURE PROPER OPERATION CONDITION TO THE SATISFACTION OF ATTENDING MARINE INSPECTOR. 46 CFR 58.05-1

Vessel suffered a seal failure on their #4 z-drive unit. The Z-Drive was replaced by ships crew and operationally tested by Coast Guard Marine Inspectors.

It shall be the duty of the master or other person in charge of the vessel to see that all of the provisions of the COI are strictly adhered to. The vessel's COI routs and conditions only allow for the transit from the east landing to the west landing. The west landing is currently not safe to dock due to underwater hazards, pilings that could continue to damage the vessel. Therefore the vessel can not operate within the parameters of it's COI. Remove underwater hazards or provide safe barrier between the hazards and the vessel when it's mooring.

AS A RESULT OF RUNNING AGROUND THE #4 Z DRIVE HAD A LARGE ROCK WEDGED IN THE PROP. THE ROCK WAS REMOVED BY DIVERS BUT ALL BLADES WERE BENT WHICH IS CAUSING SIGNIFICANT VIBRATION WHEN THE VESSEL IS RUNNING. REPAIR OR REPLACE PROP TO ENSURE PROPER OPERATIONAL CONDITION TO THE SATISFACTION OF ATTENDING MARINE INSPECTOR.

M/V CAMERON II has suffered several transmission coupler failures. Appropriately 3 months ago the #1 engines transmission coupler failed and was replaced. Today the #3 and #4 engine transmission couplers failed within hours of each other. Given the circumstances an inspection shall be made to the #1 and #2 engines transmission couplers.

An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. Fwd engine steering gearbox reported inoperable. Repair to the satisfaction of marine inspector prior to carriage of passengers.

DIESEL ENGINES AND AUXILIARY ENGINES SHALL BE DESIGNED AND CONSTRUCTED IN CONFORMANCE WITH 46 CFR SUBCHAPTER F. UPON STARTING THE NUMBER 2 SHIP SERVICE GENERATOR OBSERVED EXHAUST LEAK EMITTING FROM AROUND THE LAGGING.

A VESSEL AT ANCHOR SHALL EXHIBIT AN ALL ROUND WHITE LIGHT. VESSELS ALL ROUND WHITE LIGHTS WERE NOT OPERATING PROPERLY. 8MAY19: TEMP FIX WAS MADE. BOARD WAS REMOVED AND SENT OFF FOR PERMANENT REPAIR. DEF EXTENDED TO 01JUN19

Fire main station #8 has a small crack in teh pipe approximately 1" from the pipes penetration through the deck.

Grab lines for multiple life buoys were deteriorated and in need of replacement.

Rescue boat outboard motor will not shift into reverse properly. Prove proper operation of rescue boat outboard motor (propulsion)

Trouble shoot and make necessary repairs to batteries and/or battery charger. Batteries in generator room are boiling and are hot to the touch. Batteries are emergency power supply. In addtion, the battery charger does not appear to be operating properly.

Navigation light found to be not working under emergency power.

Data plates were unreadable on several aire reeceivers. 3 of the 4 relief valves did not reset after being tested.

Vessel requires a technician to test overspeeds.

Number Two Prop low oil alarm light was not functioning properly.

Both the stenciling of the buoys themselves and the location stenciling needed to be be completed.

Two parallel seven foot by six inch insets on port side shell in way of #1 port void space, insets start approximately 6 inches below the deck line and are 6 inches apart. Multiple welds parted in way of the insets. Repairs to be in the form of a 10'x 5' plate plus new brackets and a repaired stiffener.

Loss of steering resulting in an allision with another vessel. Determination as to cause of loss to be made by third party no later than 29NOV2016.
