The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Make ready for inspection and testing the following items: Watchman alarm, interconnected fire detection system, installed liferafts, fire pump and hoses, emergency lighting, fixed fire fighting system shutdowns. 46 CFR 176.840

Each length of hose must: (1) Be lined commercial fire hose that conforms to Underwriters' Laboratories, Inc., Standard 19 or Federal Specification ZZ-H-451E. A hose that bears the Label of Underwriters' Laboratories, Inc., as lined fire hose is accepted as conforming to the requirement. Fire station #4 hose had multiple pin holes located at the hydrant connection. 46 CFR 181.15-10 Old T

Lighting fixtures shall be constructed in accordance with the requirements of Underwriters' Laboratories, Inc. Standard for Marine Type Electric Lighting Fixtures, Subject 595, or shall bear the proper Underwriters' Laboratories, Inc. "Marine Type" label, or shall be constructed in accordance with the requirements of other recognized testing laboratories. Several deck 02 outdoor lights were missing the protective guard around the globe or were found broken. 46 CFR 183.10-20(I) Old T

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Fire detection sensor in the steering gear room was missing.

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. fuel flow when servicing accessories. The shutoff valve at the tank shall be operable from outside the tank compartment. Fuel shutoff valves were replaced but the emergency fuel pull cable was not connected to the valves and could not be tested. 46 CFR 182.20-40(3) - Old T

Spaces containing machinery shall be fitted with at least two ducts to furnish natural or mechanical supply and exhaust ventilation. Both engine room fans were supplying air to the engine room with no exhaust ventilation. 46 CFR 182.20-45(c) - Old T

All enclosed spaces within the vessel shall be properly vented or ventilated. Where such openings would endanger the vessel under adverse weather conditions, means shall be provided to close them. All ventilation pull cables were too short and could not be locked into place properly. 46 CFR 177.20(a) - Old T

The lights prescribed by these Rules shall, if carried, also be exhibited from sunrise to sunset in restricted visibility and may be exhibited in all other circumstances when it is deemed necessary. Stern light was inoperable. 33 CFR 83.20

Except as otherwise provided in this section, all vessels shall be provided with not less than two avenues of escape from all general areas accessible to the passengers or where the crew may be quartered. Escape ladder from aft crew 100's space was not properly secured at the bottom of the escape ladder. 46 CFR 177.15-1 Old T

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Public address system speakers were inoperable in the passageway in crew aft 100's. 46 CFR 184.610(b)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock. The following electrical issues were identified: - Electrical outlet in crew berthing forward 100's outboard room did not have a cover installed. - 02 florescent lights in crew berthing forward 100's did not have bulbs installed and were missing the covers. - Crew berthing room #107 had a broken electrical switch and broken light cover. - Crew berthing room #115 had a broken electrical plug cover. - Crew berthing room #114 had a missing light cover and missing light bulb. 46 CFR 183.200

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each normally unmanned space. Forepeak high water alarm was inoperable. 46 CFR 182.530

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Reefer and freezer were not properly secured in aft food storage space.

On vessels described by 46 CFR 175.110(d) of this chapter, a passenger safety bill must be posted by the master in each cabin or stateroom, and in passenger accommodation spaces. Several passenger staterooms were missing passenger safety bills as required. 46 CFR 185.515

The station bill must be posted at the operating station and in a conspicuous location in each crew accommodation space. Crew accommodation spaces were missing the required station bill. 46 CFR 185.514

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 03 life rings onboard were found with severe UV damage and cracking. 46 CFR 176.808(d)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. EPIRB hydrostatic release unit was found expired. 46 CFR 185.740(b)

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. 02 fire extinguishers were found past due on its inspection. 46 CFR 176.810(a)(1)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Starboard air conditioning space electrical enclosure was missing the cover and wires were found exposed. 46 CFR 183.200

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Aft engine room bilge alarm float switch was found broken. 46 CFR 182.530(a)(12)

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment Including the following: Inspection of piping, controls, and valves. Fire main branch valve in engine room was leaking excessively at the bonnet. 46 CFR 176.810

In systems and applications where flexible hoses are permitted to be clamped Double hose clamping is required where practicable. Multiple vital systems on port MDE had single hose clamps. 46 CFR 182.410(d)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Shower light fixture in room 111 was missing the globe. 46 CFR 183.410

Combustibles and other foreign materials are not allowed within ventilation ducts. Galley griddle exhaust ventilation had excessive build up of grease. 46 CFR 176.830

A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. All weather deck public address speakers were inoperable. 46 CFR 184.610(c)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. GFCI outlets were not installed next to the sink in "TEBENKOF" and WRANGEL" passenger accommodation spaces. 46 CFR 183.200(b)

The Bridge-to-Bridge Act and the regulations of this part apply to the following vessels in the navigable waters of the United States: (a) very power-driven vessel of 20 meters or over in length while navigating. Vessels Bridge to Bridge Radio Certificate was expired. 47 CFR 80.1001

Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessels FCC station license was expired. 47CFR 80.13

A certification that the ship has passed an inspection must be entered into the ship's log by the inspecting technician. Vessels FCC Safety Radiotelephony Certificate was expired. 47 CFR 80.59(a)(2)

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Installed CO2 system did not have the required instructions or schematic of the system. 46 CFR 185.612(a)

Electrical equipment on a vessel must be installed and maintained to Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical enclosure in the AC room was missing the cover and had exposed wiring. 46 CFR 183.200(b)

A vessel of more than 19.8 meters (65 feet) in length must have at least one fire axe located in or adjacent to the primary operating station. A fire ax was not located in or adjacent to the operating station. 46 CFR 181.600

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date Coast Pilot [name withheld], Tides and Currents. Vessels nautical publications were not updated. 46 CFR 184.420

Nonconducting mats or grating must be provided on the deck in front of each switchboard and, if accessible from the rear, on the deck in the rear of the switchboard. Nonconductive matting was missing in front of main switchboard. 46 CFR 183.330(f)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Light fixture in lazarette overhead was secured with electrical tape. 46 CFR 183.01-15 OLD T

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Hydrostatic release units for both life rafts were improperly installed preventing a float free arrangement.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Starboard quarter 2nd deck had 02 cracked vertical stancions at the acocomodation access door.

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: "Emergency Exit, Keep Clear", unless such markings are deemed unnecessary by the Officer in Charge, Marine Inspection. Emergency escape was not properly marked. 46 CFR 185.30-15 OLD T

The Rules concerning lights (§§ 83.20 through 83.31) shall be complied with from sunset to sunrise, and during such times no other lights shall be exhibited. All navigation light lenses were cracked and discolored preventing the intensity of the lights from achieving the required 2NM range. Vessel is restricted to daylight operations and periods of unrestricted visibility. 33 CFR 83.20(b)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Forward storage space passageway and space has several overhead lights without protective covers installed.

For items not covered by standards in this subchapter, inspection shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspectionsm as good marine practice. Forward lounge bar had several CO2 beverage bottles not properly secured. 46 CFR 176.25-1 OLD T

All cable and wire must Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Wire run is not properly supported in forward storage space overhead. 46 CFR 183.340(b)(4)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Electrical outlet in forward storage space does not have a cover plate attached. 46 CFR 183.01-15 OLD T

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Gen Board Bus 2 Panel 3 had an open slot requiring a blank. 46 CFR 183.01-15 OLD T

Remote fuel shutoff stations shall be marked in at least 1-inch letters indicating purpose of the valve and direction of operation. Fuel shutoff handles were not properly marked.

For items not covered by standards in this subchapter, inspection shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspections as good marine practice. Ice machine located outside the galley is not properly secured. 46 CFR 176.25-1 OLD T

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter. Lazzarette had excessive hydraulic fluid in the bilge from recent maintenance. 46 CFR 176.25-50 OLD T

All enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close them. Port and starboard side ventilation closures require gaskets due to large gap when closed. 46 CFR 177.20-1 OLD T

Extensive wastage to longitudal framing below room 103 shower. Crop and renew to original condition.

Prove proper operation of engine room fixed extinguishing system machinery shut-downs.

Identify and make permanent repairs to all exhaust leaks in engine room.

While the vessel is engaged in towing the vessel must be controlled by a licensed Master or Mate of towing. Vessel has until April 1st 2022 to man vessel with licensed Master and Mate of towing.

Vessel must provide updated stability calculations to include towing to Coast Guard Marine Safety Center to update vessels stability letter by April 1st 2022

Vessel shall provide correct light and day shape configuration for the tow being conducted prior to towing.

Vessel shall start and maintain records for towline and terminal gear used for towing astern or alongside prior to towing

Provide an interlock on the electrical distribution panel to prevent generators from being simultaneously connected to the switchboard with shore power.

Ensure all hot water heaters are properly installed and secured from rolling by straps or other devices.

Engine room fire detection must be registered visibly and audibly in the pilothouse; system failed to provide an audible alarm.

A vessel must be provided with a bilge dewatering system; an operational test of the system failed and one remote operated bilge valve could not be exercised.

Piping in excess of 150 degrees F must be suitably insulated, exhaust lagging in the starboard steering room and the port generator was not provided.

An alarm for fixed gas fire extinguishing systems must be clearly and conspicuously marked; the engine room CO2 siren sign was badly faded and inconspicuously placed.

An exposed hazard such as rotating machinery must be properly protected by a cover; the generators and main engines' flywheels were exposed.

The name and hailing port must be marked on some clearly visible exterior part of the stern; no name or hailing port were provided.

A high water alarm must be provided for each space with through hull fittings; the bilge sensor device in the steering room was not properly mounted and inoperable.

Lighting fixtures must have a guard and wires must be properly terminated in a junction box; both fwd and aft crew's accommodations were missing lighting fixture covers and both had bear wirs protruding through the ceiling.

Wires and cables must be properly supported and installed in a manner that does not interfere with moving machinery; lighting cables were interfering with the starboard side rudder bar.

Except for vessels limited to daytime operations the compass must be illuminated; the light was inoperable.

Conduct drills prior to seasonal operations.

[name withheld] reported failure of #2 Engine propulsion, likely in the reduction gear. Master instructed to repair proper function of #2 shaft for forward and reverse operation.

Gauges to indicate L/O pressure shall be provided for propulsion equipment. Repair or renew the port main engine L/O pressure gauge at the operating station.

Each vessel that operates on the high seas mush have Cat 1 EPIRB. Provide and properly install EPIRB.

A vessel must carry and approved or equivalent first aid kit and instructions. Provide an appropriate first aid kit for the vessel.

Properly distribute life preservers and mark life preserver stowage in the crew’s accommodations.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck for passengers and crew. Correct E-light discrepancies and make available for reinspection.

Suitable covers, guards, or rails shall be installed in way of all exposed hazards. Provide cover for ventilation opening for the Engine Room supply fan.

Complete servicing and maintenance of the engine rooms fixed fire extinguishing system.

Shutoff valve shall be installed at the engine end of the fuel line. Provide shutoff valve for the port main engine.

Each flexible hose designed for use with hose clamps shall utilize two clamps at both ends. Provide an additional clamps for both generator's sea-water piping.

Vessel owners shall minimize accidental personnel contact with energized circuits. Remove the two dead-ended cables in the FWD 100's void, reconnect and protect identified cable next to hatch in aft 100's, and properly terminate large cable in ovhd of aft steering.

Each conductor shall be with metal supports not more than 24" apart. Properly run and support cable providing powering to aft corridor outlet in the FWD 100's.

Each flexible hose designed for use with hose clamps shall utilize two clamps at both ends. Provide an additional clamp for the port main engine jacket water cooler connection.

Penetrations in watertight bulkheads must have means to make them watertight. Properly secure open ended piping penetrations in the steering compartment fwd bulkhead stbd of centerline.

Bilges shall be free of excessive accumulation of oil, trash, debris, or other matter that may clog bilge pumping systems. Remove or more appropriately store lagging, tarps, and construction materials in the steering compartment.

Provide proper number of IBAs for persons on board. 46CFR180.200

Three sea valves were rejected for improper sealing. These valves include # 35A, a 1.5 inch gate valve on the port side for the graywater system; and # 20Aand #19A, both of which are 2 inch gate valves on the port side for the black water overboard.

2 transverse frames in the engine room were found weakened by large irregular holes cut in them in order to run pipes through. These transverse frames are located beneath the port generator and aft of the starboard bow thruster pump.

Longitudinal stringer in void space in accommodation Rm #107 was found to have a 12 inch area where it is corroded through at the forward end of the void and a 36 inch area where it is corroded through where the stringer meets the aft bulkhead.

The crew noted a small hole in the hull plating at the aft crew quarters. Upon examination, excessive wastage was found on the deck and bulkheads. The issue will be corrected with two deck inserts, a bulkhead insert and a side shell insert.
