Vessel experienced smoke coming from the #1 prop shaft clutch. Upon investigation, it was found that a reduction gear shaft seal was leaking on the clutch. Vessel to repair clutch and reduction gear shaft seal to manufacturer specifications. Deficiencies must be corrected to the satisfaction of the attending marine inspector before the vessel will be considered for service. 46 CFR 70.20-1 46 CFR 58.05-1

Doors, other than those which are normally locked, such as from staterooms, fan rooms, lockers, etc., shall be of the self-closing type capable of closing against a 31/2 degree list, and such doors shall be numbered in accordance with § 78.47-35 of this subchapter. 46 CFR § 199.160 Rescue boat embarkation, launching and recovery arrangements... The rescue boat must meet the embarkation and launching arrangement requirements of §§ 199.110 (e) and (g), 199.150, 199.155, and if the launching arrangement uses falls and a winch, § 199.153. All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features and interlocks. Approved PSTP has low lube oil pressure shutdown as the expected result on both MDEs at 12 psi. Amend PSTP to relfect system function of no shutdown or make system follow PSTP. Requirement due by next annual inspection. 46 cfr 61.40-1

Illumination of passenger and crew spaces. Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Multiple Emergency lights throughout the vessel are extinguished and not maked as "Emergency Lighting" 46 CFR 111.75-15

The applicable standards established by the ABS Steel Vessel Rules may be used as the standard for the design, construction and testing of main and auxilary machinery; 46 CFR 58.01-5 & 46 CFR 62.35-50. #2 SSDG low lube oil alarm did not actuate IAW approved PSTP. ABS Rules 4-9-4 Tabel 8.

Each general emergency alarm signal must be an electrically-operated bell, klaxon, or other warning device capable of producing a signal or tone distinct from any other audible signal on the vessel. (04) general alarms inoperable. inoperable general alarms located in fire zone: C, 3, 4, and 7.

CPP system on #1 end of vessel is inop. System needs to be back in operation. Rectified within 7 days. 46CFR77.03-1

Repair or replace leaking fire plug valve at station 23.

Replace the multiple cables into a single compression terminal on the gyro compass battery installation.

Replace the corroded and compromised radio/radar emergency battery cable.

Renew the deteriorated retro-reflective tape and faded markings of both rescue boats IAW 46 CFR 199.176.

Replace and repair compromised or removed A-60 boundary insulation and restore class B boundary encapsulating EOS.

Verify breaker size against approved plan and provide breaker data placard to include breaker size (amp) and corresponding system served.

Repair loose hatch on the No.1 steering gear room.

Qualify base metal under the holed No.2 STBD cleat to ensure min scantling at the vessel next hull inspection 2022.

Hole in fire main piping in the #1 void space. Effect repairs prior to carriage of passengers.

In all ventilation systems, manually operated dampers or other suitable means shall be provided in accessible locations, outside the spaces served by the system, for shutting off the passage of air in the event of a fire. 1. Prove proper operation of pilot house #1 and #2 fan room ventilation damper and provide label identifying open and closed positions. 2. Install wing nut on #2 crew quarters supply damper open/close mechanism. 3. Provide securing devices in #1 and #2 crews quarters and toilet exhaust(on house top) cover. 4. Prove fire damper modifications in fan room B and C were part of approved fire control plan modifications. If modifications were approved, submit updated fire control plan to OCMI for approval. 5. Maintenance fire damper in fan room to ensure a complete air seal.

As appropriate for the intended voyage, all vessels except barges, ferry boats, and vessels operating exclusively on rivers, must carry adequate and up-to-date (e) local notice to mariners. Publications were not up-to-date. Provide evidence of updated publications.

Ceilings and linings and any furring incidental to their erection shall be of approved incombustible materials. Structural fire protection in ceiling is unsecured and falling onto ceiling light. Secure materials as originally installed.

Each space used by passengers or crew must be fitted with lighting that provides a safe habitable and working environment under normal conditions. Multiple lights in passageway and water closet were extinguished. Fix light fixtures.

Each fire hydrant must have at least one length of fire hose, a spanner, and a hose rack or other device for stowing the hose. #13 and #20 hoses failed during operational testing. #23, #24 and #26 fire hydrants require servicing. Replace hoses, and service hydrants.

Each vessel must carry a life-jacket for each person on board and each life-jacket must be marked in block capital letters with the name of the vessel; and with type 1 retro-reflective material. Eight life-jackets were condemned. Ensure vessel meets COI requirements for the number of life-jackets required.

11126 - #2 end rescue boat davit suffered casualty and is inoperative. Manual launch and recovery is still operational, however powered mechanism is inoperable. Vessl crew has identified issue exists within the power supply unit. You must provide findings and repair proposal to the Coast Guard and conduct a 100% weight test to be witnessed by Coast Guard Marine Inspectors. - 46 CFR 199.45(c) - 705C (Rectify deficiencies w/in 7 days).

WSF contracted divers found oil droplets leaking from the #2 end stern tube of vessel. Approximately 8 gallons of stern tube lubrication oil was reported missing from the head tank over the course of a week by the vessel's chief engineer. Effec reparis to the stern tube and provide report of findings.
