If, during an internal structural examination, damage to the hull plating, structural members is discovered, the OCMI may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. During ISE, found the following hull / structural member damage: #3P wing void- 3/4"W x 18-20”L breach 3’ below gunwhale (behind rub rail), impairing seaworthiness. Effect permanent repairs. #1S wing void- multiple excessive longitudinal “hump channel” insets in bottom plating under cargo tanks. Stbd Bow Rake - 2 cracked intermittent weld sections between #2 half-channel and side shell. Effect permanent repairs. Port Bow Rake - 4 cracked stitch welds Btwn #2 half- channel and side shell & 3 excessive insets in corner wraps. Effect permanent repairs. Repairs must be inspected by a USCG Marine Inspector.

46 CFR 56.50-29(a) Each nonrising-stem valve where, because of design, the position of the disc or closure mechanism is not obvious must be fitted with an indicator to show whether the valve is opened or closed. Fwd fuel tank supply valve for cargo pump prime movers did not indicate position. Install valve at fuel tank supply line that indicates position, prior to cargo operations.

Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine (such as by cutting off the intake air). Found emergency shutdown for aft-stbd pump engine did not secure engine. Repair mechanism to ensure shutdown of engine.

Once barge is returned to Stone Oil (MM96.7) conduct damage survey to ensure no damage to the hull of the barge. Once completing this contact the MSO New Orleans Inspection dept. so the barge can be cleared to the satisifaction of the attending CG Marine Inspector.

REPLACE BRAKE ON BOTH AFT DECK WINCHES.

CLEAN ALL PRODUCT OUT OF ALL CONTAINMENT SUMPS.

GAS FREE BARGE FOR INSPECTION TO DETERMINE CAUSE OF LEAKAGE. NO CARGO IS TO BE CARRIED UNTIL IF HAS BEEN INSPECTED BY USCG MARINE INSPECTOR.
