All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. In addition to the emergency power sources required under part 112 of this chapter, each self-propelled vessel and each mobile offshore drilling unit must have at least two electric generating sources.46 CFR § 111.10-3 The master reported a of loss of power on the starboard generator caused by mechanical failure of it’s water pump. 46 CFR § 90.25-1

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. In addition to the emergency power sources required under part 112 of this chapter, each self-propelled vessel and each mobile offshore drilling unit must have at least two electric generating sources. The port generator is not operational. The port generator voltage regulator conductor was found melted/damaged, and the crew took preventative actions when the electrical system began to fail by shutting down the generator. The generator windings were tested and found damaged after the incident. 46 CFR § 90.25-1 and 46 CFR § 111.10-3

Except as provided in paragraph (d) of this section, after loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master or person in charge shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. The vessel may not depart until it is in compliance with these requirements. When determining compliance with applicable stability requirements the vessel's draft, trim, and stability must be determined as necessary. The master was unfamiliar with using the information in the trim and stability book while loading cargo. The operator uses a third party to verify cargo loading and vessel stability. The master is unfamiliar with essential shipboard duties related to cargo loading and vessel stability requirements outlined in the trim and stability book and stability letter.

Each vessel must have an amplifier-type announcing system that will supplement the general emergency alarm. This system must provide for the transmission of orders and information throughout the vessel by means of microphones and loudspeakers connected through an amplifier. The system may be arranged to allow broadcasting separately to, or to any combination of, various areas on the vessel. Each electrical subsystem in a weather location must be watertight or in a watertight enclosure and must meet Type 4 or 4X of NEMA 250 or IP 56 of IEC 60529:2013. Loudspeakers must be located to provide intelligible and audible one-way communication throughout the vessel. Weather deck loudspeakers must be watertight and suitably protected from the effects of the wind and seas. There must be a sufficient number of loudspeakers throughout the vessel. The public address system must be installed with regard to acoustically marginal conditions and not require any action from the addressee. The vessel does not have a PA system loudspeaker in any internal spaces. The vessel has two loudspeakers for the PA system externally on the main deck. The forward PA speaker was not operational and the aft PA speaker does not create a sound at 80 dB(A) IAW 46 CFR 113.50-15(c)(2).

Except as provided in paragraph (d) of this section, after loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master or person in charge shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be. The vessel's stability letter requires all weather tight doors to be closed underway. The doors were open while underway for drills.

The emergency power source must meet Table 1 to 112.05–5 and have the capacity to supply all loads, at a unity (1.0) service factor, that are simultaneously connected to it, except a load on a bus-tie to the main switchboard or non-required loads that are connected in accordance with § 112.05–1(c). The emergency lighting requirements of § 112.15–1 (b), (c), (f), and (g) must be met. § 112.15–1 (b) Enough lights throughout machinery spaces to allow essential operations and observations under emergency conditions and to allow restoration of service. Emergency lighting was not operational when tested in the stbd side machinery space, switchboard, aft tunnel, mid tunnel, and it is not labeled in the stairwell to the bridge.

Each panelboard must meet Section 9.10.1 of IEEE 45.1–2017 (incorporated by reference; see § 110.10–1 of this subchapter). Each outgoing circuit should have a suitable nameplate. The breakers in the 220V distribution panel located in the tunnel are not labeled.

Barges with sleeping accommodations for more than six persons and all self-propelled vessels shall have permanently exhibited for the guidance of the officer in charge of the vessel the following plans: General arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads, together with particulars of the fire alarms, detecting systems, the sprinkler installation (if any), the fire extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating systems including particulars of the master fan controls, the positions of dampers, the location of the remote means of stopping fans, and identification numbers of the ventilating fans serving each section. For vessels constructed on or after September 30, 1997 or for existing vessels which have their plans redrawn, the symbols used to identify the aforementioned details shall be in accordance with IMO Assembly resolution A.654(16). These identical symbols can also be found in ASTM Adjunct F 1626. The aforementioned information shall be kept up-to-date, any alteration being recorded in the applicable medium as soon as practicable. The Fire Control Plan was found with several applicable safety items that are no longer located in the areas identified on the plans or removed from the vessel. Update the Fire Control Plan to match the location of the safety equipment on the vessel.

The operator of a vessel with a capacity of 250 or more barrels of oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154 shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter for transferring—(a) To or from the vessel; and (b) From tank to tank within the vessel. The transfer procedures required by § 155.720 must contain all contents found in 33 CFR 155.750. The transfer procedures on-board do not include all contents from 155.750.

Each operator or agent of a vessel with a capacity of 250 or more barrels of fuel oil, cargo oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154, or each person who arranges for and hires a person to be in charge of a transfer of fuel oil, of a transfer of liquid cargo in bulk, or of cargo-tank cleaning, shall designate, either by name or by position in the crew, the person in charge (PIC) of each transfer to or from the vessel and of each tank-cleaning. The PIC for fuel transfers is not designated in writing.

For each watertight door which is in a required subdivision bulkhead, an indicator light must be installed in the pilothouse and at each other vessel operating station from which the door is not visible. The light must show whether the door is open or closed. The port side e/r WT door light is not operational.

The fuel supply piping to engines shall be of seamless steel, annealed seamless copper or brass pipe or tubing, or of nickel copper or copper nickel alloy meeting the requirements of subpart 56.60 for materials and § 56.50–70(a)(2) for thickness. The installation shall comply with § 56.50–70(b). MI identified a diesel fuel leak on the starboard generator at the piping connection to the fuel filter.

The fuel supply piping to engines shall be of seamless steel, annealed seamless copper or brass pipe or tubing, or of nickel copper or copper nickel alloy meeting the requirements of subpart 56.60 for materials and § 56.50–70(a)(2) for thickness. The installation shall comply with § 56.50–70(b). All fuel pipes, pipe connections, and accessories shall be readily accessible. The piping shall run in sight wherever practicable, protected against mechanical injury, and effectively secured against excessive movement and vibration by the use of soft nonferrous metal liners or straps without sharp edges. Fuel delivery piping to the manifold on the starboard generator is installed without proper supports to prevent movement or vibration.

Each cable installation must meet Sections 6, of IEEE 45.8–2016. Metallic cable retention devices should be installed not less than every 24 inches. MI identified unsupported jacket water temp sensors on the port/stbd main engines and generators.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. The fire pump piping and reduction flange were found leaking under load. The piping was removed and the company hired a third party to repair. Provide a repair proposal prior to repairs.

The general alarm system must be tested weekly and logged.

Where fitted, it shall be the duty of the master to see that the emergency lighting and power systems are operated and inspected at least once in each week that the vessel is navigated to be assured that the system is proper operating condition. Storage batteries for emergency lighting and power systems shall be tested at least once each 6-month period that the vessel is navigated to demonstrate the ability of the storage battery to supply the emergency loads for the period of time specified in Table 112.05–5(a) of this chapter. The date of the tests and the condition and performance of the apparatus shall be noted in the official log book. The master failed to appropriately log the tests required by 46 CFR 97.15-30.

The following conditions were found as a result of the Two (2) vessels making contact with each other as described in Finding 191 of this work order: Due May 26, 2024: Side Shell - Sideshell was found variously set in, approximately 6" in way of Fr Nos. 38 - 47 dissipating both fore and aft. Plating in way of Fr. 41 suffered the brunt of the impact above the internal longitudinal sideshell stringer. The affected area is approximately 16' in length and 6' in height. Internal Structures -Fr. Each machinery space must be ventilated to ensure that, when machinery operating at full power in all weather including heavy weather, an adequate supply of air is maintained for the operation of the machinery and for the safety, efficiency, and comfort of the crew. Repair & install P&S wasted/rusted motor fan for engine room ventilation.

The compressed air starting system must provide the cranking torque and engine starting RPM recommended by the engine manufacturer. Repair inoperable air compressors.

If twist-on type of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Remove or make proper wire connections for dead end wires above STBD engine room batteries and wires above fire station #6.

The routine maintenance of the oily water separating equipment and the bilge alarm must be clearly defined by the manufacturer in the associated operating and maintenance manuals. All routine and repair maintenance must be recorded. Provide proof of maintenance and demonstrate proper operation.

Non-conducting deck coverings, such as non-conducting mats or gratings, suitable for the specific switchboard voltage must be installed for personnel protection at the front and rear of the switchboard and must extend the entire length of, and be of sufficient width to suit, the operating space. Install missing mat in front of main switchboard.

Approved fire extinguishing systems must be maintained in good condition to the satisfaction of the OCMI. The flexible hoses between each high pressure bottle do not have proof of the required pressure tests at the intervals required. The hoses were manufactured in 2014 and have not been tested since original installation. Two flexible hoses were found gouged and require replacement. Provide proof of pressure test services conducted on the fixed CO2 system flexible hoses or replace hoses. A COI will not be issued until the CO2 system is found to be in compliance with regulations.

Propulsion diesel engines must have engine protection: over-speed trip, low oil pressure alarm/shutdown, high temp. Demonstrate alarms and shutdowns are operational. The high temperature alarms were verified on March 14, 2024. The vessel must install overspeed alarms no later than June 2024.

To maintain a valid Certificate of Inspection, you must complete your annual and periodic inspections within the periods specified in § 91.27-1 (a) and (b) and your Certificate of Inspection must be endorsed. The vessel's fourth annual is required to be completed on or before June 15, 2023. The vessel operator failed to notify the Coast Guard for an annual inspection. The vessel is permitted to depart dry dock and return to Saipan for a COI re-inspection. Submit an CG application for inspection to MSD Saipan in order to schedule. Code 705: Prior to returning to cargo service. 46 CFR 91.27-5

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. The port main engine sea water cooling pump was found wasted and leaking during a marine inspector's attendance. 46 CFR 96.03-1 46 CFR 58.05-1

Where Table 91.40-3(a) indicates a 2.5 year examination interval, it means a vessel must undergo two examinations within any five year period. No more than three years may elapse between any two examinations. Dry dock and ISE overdue as of 28FEB2023. Prior to carrying cargo/persons for hire, vessel must complete required dry dock and ISE for credit to the satisfaction of the attending USCG MI.

Propulsion machinery, including main engines, reductions gears, shafting, bearings, and electrical equipment and systems must be maintained to ensure proper operation; be suitable for route and service, and have suitable propulsion controls to provide the operator full control at each operator station. The port main engine has a air leak from the inboard turbo charger pipe to the after cooler.
