Reduction in propulsion reported, while vessel underway, due to fuel oil leak on No.6 cylinder fuel pump on Stbd Main Engine and Abnormal Stbd M/E alarm received on bridge. During sea trials and inspection of reported fuel oil leaks on Stbd Main Engine, Marine Inspector identified substantial and consistent leaks from No.2 and No.5 cylinder's high pressure fuel pumps. 46 CRR 143.205(a) - 17 c.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Illuminated magnetic compass. The compass or swing-meter must be readable from each operating station. The compass is currently unreadable from the operator's station.

On or about 20OCT2024, during a pier side testing of the starboard throttle control, the operator was unable to engage the starboard main propulsion engine. Upon further troubleshooting, the crew discovered that the circuit board for the governor air system had malfunctioned. Effect permanent repairs in accordance with OEM instruction to the satisfaction of the attending USCG Marine Inspector.

The following systems must be equipped with gauges at the machinery location. The port main engine’s fuel oil pressure gauge, cooling water pressure gauge, and RPM meter are not functioning. The starboard main engine’s cooling water pressure gauge is not functioning.

To stop flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies directly to a propulsion engine or generator prime mover. The fuel shut off remote cable for #3 stbd fuel tank broke.

There must be an installed or portable bilge pump for emergency dewatering. The installed bilge transfer pump is not functioning.

The fire main must have a sufficient number of fire hydrants with attached hose to allow a stream of water to reach any part of the machinery space using a single length of fire hose. The engine room’s fire station valve is broken and inoperable.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems must be maintained to ensure proper operations; be suitable for route and service; and have suitable propulsion control to provide operator full control at each operating station. Vessel lost generator power while underway and was towed back to F2. Make repairs to the affected generator(s), report findings, and demonstrate proper operations to the satisfaction of the attending Marine Inspector prior to movement.

Sufficient lighting suitable for the marine environment must be provided within crew working and living spaces. Several lights are extinguished in the ships machinery spaces.

Electrical conductors passing through watertight bulkheads must be installed so that the bulkhead remains watertight. Wire runs passing through watertight bulkheads between the engine room and engine control room are not watertight.

Electrical components that pose an electrical hazard must be in an enclosure. The battery charger cabling and electrical supply conductors are not properly installed.

The following items were found to be out of compliance: 1. Saltwater supply pump has excessive leaking at the packing gland, 2. Starboard main engine fuel gauge is inoperable.

Pipes and machinery components that exceed 428*F, including fittings, flanges, valves, exhaust manifolds, and turbochargers, must be insulated. Starboard main engine turbo exhaust and port main engine exhaust are missing heat shielding.

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crewmembers are capable of performing the duties expected for emergencies. This includes abandoning the vessel, recovering persons from the water, responding to onboard fires and flooding, or responding to other threats to life, property, or the environment. Current recovery net and life sling installed are not capable of recovering a unconscious person from the water.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Multiple emergency lights are extinguished in the engine room.

Each integral fuel tank must have a vent that connects to the highest point of the tank, discharges on a weather deck through a bend of 180 degrees, and is fitted with a 30 X 30-mesh corrosion resistant flame screen. Fuel oil tank vents are not fitted with 30 X 30 corrosion resistant flame screens.

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. Aft main deck starboard side engine room ventilation dampener is unable to close.

EACH Cargo vessel of 150 gross tonnage and upwards but less than 500 gross tonnage must have installed a bridge navigation watch alarm system onboard not later than the first survey after 1 January 2018. Vessel is not outfitted with an operational BNWAS. Vessel is restricted to domestic routes until BNWAS is installed and tested.

A towing vessel with an existing radar must meet the applicable requirements of paragraphs (a)(1), (i) through (iv) of this section by August 2, 1998. The radar is currently inoperable with the mast in the stowed position.

Except as provided by 33 CFR 164.01(b), each towing vessel must be equipped with the following navigational-safety equipment: A searchlight directable from the vessel's main steering station and capable of illuminating objects at a distance of at least two times the length of the tow. The forward searchlight is currently inoperable with the mast in the stowed position.

Each towing vessel must be equipped with navigation lights, shapes and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The stern navigation light is not visible with the mast in the stowed position.

The fire main must have a sufficient number of hydrants with attached hose to allow a stream of water to reach any part of the machinery space using a single length of hose. The starboard hydrant station in the engine room is corroded to the point that the hose cannot be attached.

Towing machinery used to connect the towing vessel to the tow must be suitable for its intended service. It must be capable of withstanding exposure to the marine environment, likely mechanical damage, static and dynamic loads expected during intended service, the towing vessel's horsepower, and arrangement of the tow. The vessels aft tow winch was not operational at the time of the inspection.

Except as provided by 33 CFR 164.01(b), each towing vessel must be equipped with the following navigational safety equipment: An illuminated card-type magnetic steering compass readable from the vessel's main steering station. The installed magnetic compass' glass was severely cracked rendering the compass unreadable.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. crew member on board was unable to provide proof of a valid medical certificate.

Vessels subject to initial and periodic surveys shall have annual surveys within 3 months either way of the certificate's anniversary date. The annual surveys shall be made by and prove satisfactory to the assigning and issuing authority prior to executing the required annual endorsements on load line certificates. The scope shall be as defined in 46 CFR 42.09-40 and such as to ensure that the applicable load line marks are found to be correct for the condition the vessel is then in. Vessel's intermediate annual survey expired on 15APR2021. Pending completion of its annual survey to the satisfaction of the attending Class Surveyor or upon an additional extension granted, HURAO is restricted to operating within Apra Harbor Guam. This restriction will take affect on 16APR2021 at 0001hrs.
