During the annual inspection, objective evidence revealed that multiple structural and machinery worklist items identified during the 28NOV2025 drydock inspection, that were cleared by the Recognized Organization (RINA) class report and ILLC endorsement survey, were not rectified as identified by RINA report 06FEB2026. The following conditions were found onboard during annual inspection: Wasted steering room vents on stern deck, wasted fuel vents on aft deck, wasted engine room intake vents, wasted port watertight door bulkhead, temporary epoxy repairs on bulkhead deck in berthing, temporary epoxy repairs on bilge piping, wasted bosn locker hatch coaming and hinges, broken engine room porthole, wasted bulwark and gussets, missing watertight door cotter pins, wasted port hull plate behind forward fender rail, starboard side aft deck plate is missing studs and fasteners, port side aft day tank missing studs and fasteners.

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted... (d) The valve must be operable from a safe place outside the space. The remote fuel shut-off valve pull cables for the day tanks snapped during operational testing, completely disabling the ability to secure fuel remotely in an emergency. Repair the broken remote fuel shut-off pull cables to the satisfaction of the MI. 46 CFR 143.260(b) - 17 c.

Equipment must be installed and maintained to minimize the potential for fire and explosions. The starboard main engine was found with significant smoking at the exhaust manifold connections to cylinders no. 5 and no. 6. Repair to the satisfaction of the MI. 46 CFR 143.220(a) - 17 c.

Table 143.245(b) requires Pressure vessel relief valves to be tested twice every 5 years, with no more than 3 years elapsing since last test. No records or test dates for the 03 pressure relief valves associated with the start air receivers. Test or replace with new. 46 CFR 143.245(b) - 17 c.

Each towing vessel certificated under the provisions of this subchapter must be in full compliance with the terms of the COI. Identified deterioration of the hull in way of the starboard exhaust piping which terminates at the transom. Multiple wastage penetrations exist in the exhaust piping which created a noticeable exhaust leak in the steering space and presents a down-flooding risk as piping terminates at the freeboard. Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. The stbd side running light fixture found with broken base separating from bracket. 46 CFR 140.720 - 50 c.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings in such spaces. The engine room's port side exhaust dampener does not close in an event of an emergency. 46 CFR 144.605 - 50 c.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems must be maintained to ensure proper operation. Be suitable for route and service. Have suitable propulsion controls to provide the operator full control at each operating station. The vessel reported bearing failure to the stbd main engine. Demonstrate proper operation of the stbd main engine to the satisfaction of the attending marine inspector.

The reserve power supply must be independent of the propelling power of the ship and of any other electrical system, and must simultaneously energize the radiotelephone transmitter at its required antenna power, the required receivers, the emergency light and the automatic radiotelephone alarm signal generator. The reserve power supply must be available at all times. VHF radio does not have a reserve source of power.

Make repairs to the hydraulic leak discovered on the port duckpeller hydraulic motor to the approval of the attending marine inspector.

A fire pump must be capable of: Being energized remotely from a safe place outside the engine room. Inoperative.

A fixed fire pump must be capable of: delivering water simultaneously from the two highest hydrants at a pitot-tube pressure of at least 50 pounds per square inch. Fire pump was unable to attain required pressure.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention; Alarms must: (1) Be visible and audible at each operating station. The alarm located at the operating station may be a summary alarm. Alarms at the operating stations have no indication that they are machinery related.

Towing vessels must be equipped with the following equipment, as applicable to the area of operation:Illuminated magnetic compass. The compass must be readable from each operating station. Prove operation and submit proof of calibration.

The load line certificate shall certify the correctness of the load line marks assigned to the vessel that the vessel is in compliance with applicable requirements. The load line certificate onboard was expired beyond its expiration and extension dates. Vessel is restricted to operations within the boundary line.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems must be maintained to ensure proper operation. The port main engine low lube oil pressure shutdown switch is not operable.

Electrical components that pose an electrical hazard must be in an enclosure. The main switchboard in the engine room does not have a cover on the back or restricting access for personnel being shocked if touched.

Sufficient lighting suitable for the marine environment must be provided within crew working and living spaces. Found multiple extinguished light bulbs in the engine room.

Towing vessel more than 65 feet in length must carry at least the minimum number of portable fire extinguishers set forth in table 142.230(b). Found wrong type of portable extinguisher in the engine room, fire extinguisher not properly mounted and not serviced since 2001, and fire extinguisher on the bridge that has been discharged.

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead of deck to prevent it from rolling under heavy seas condition. The semi-portable fire extinguisher in the upper level engine room is not properly secured.

All towing vessels must be operated in a manner that minimizes the risk of down flooding and progressive flooding. The access cover for stbd void down in chain locker space was left open and not fastened.

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with 33 CFR Part 84. The port and stbd running lights are obstructed from the pilot platform.

Rails or equivalent protection must be installed near the periphery of all decks accessible to the crew. Equivalent protection must include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. The rail in the port forward of the wheel house is wasted. In addition, the port side engine room upper level is missing rails.

Each towing vessel must be maintained and operated as to minimize fire hazards and to ensure all bilges, voids, spaces are kept free from accumulation of combustible and flammable materials and liquids as practicable. The steering gear room has excessive oil leaks through the entire space making it a unsafe condition for personnel and fire hazard.

A fire pump must be capable of: Being energized remotely from a safe place outside the engine room. Remote pump start/stop controler is inoperable.

Closure devices must be provided for deck house or hull penetrations, which open to the exterior of the vessel and may allow water to enter the vessel. These devices must be suitable for the expected route. The forward quick acting watertight doors main deck are unable to secure and be watertight as required.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. The emergency lighting for crew berthing areas is insufficient for egress.

The following vessels must have a properly installed, operational, type approved AIS: Towing vessels of 26 feet or more in length and more than 600 horsepower, in commercial service. Installed AIS is not operational. Vessel is restricted to daylight operations within Apra Harbor.

The following items were found to be out of compliance: 1. port sea chest vent has an unapproved repair and leaking. 2. Hydraulic pump and motor missing rotating guards. 3. Oil in engine room bilge.

All machinery and equipment that is not in proper working order (including missing or malfunctioning guards or safety devices) must be removed; made safe through marking, tagging, or covering; or otherwise made unusable. The machinery space bench grinder is missing its safety shield.

Towing vessels of more than 65 feet in length must carry at least the minimum number of portable fire extinguishers set forth in table 142.230(b) and one 40-B portable fire extinguisher fitted in the engine room for each 1,000 brake horsepower of the main engines. The aft steering space is missing a portable fire extinguisher.

Each towing vessel must be fitted with a communication system between the pilot house and engine room. The sound power phone between the pilot house and engine room is inoperable.

Closure devices must be provided for deck house or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. The engine room’s overhead portlight glass is cracked and aft steering port side soft patch is leaking.

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. The engine control room alarm panel has 1 extinguished alarm light for #2 generator low oil psi.

Fire detection systems on a towing vessel must be certified and inspected by a professional engineer with experience in fire detection design, by a technician with qualifications NICET lvl IV fire alarm engineering technician, or by a classification society with equivalent experience. Installed fire detection system has not been certified as required.

Pilothouse deck was found to have severe wastage. Crop wasted material and renew plating. Provide gauging report 6 inches outward from affected areas, at 4 inch intervals.

Installed electrical power sources must be capable of carrying the electrical load of the towing vessel under normal operating conditions. The port generator while running has an active alarm/fault. 46 CFR 143.400(b)

Multiple watertight doors on house and deck hatch for engine room compartment with holes and wasted edges. Replace with new and install watertight gaskets found missing on multiple wateright doors.

Missing cylinder head temperature sensor for No. 4 and rpm gauge for port generator.

Replace firehose and fittings in engine room.

Install rubber matting by switchboard.

Hydro test releasing mechanism on liferaft.

Replace retro tape on liferings.

Demonstrate operation of tow lights.
