The marine employer must have a sufficient number of alcohol testing devices readily available on board to determine presence of alcohol. Alcohol test strips were expired. 46 CFR 185.212 46 CFR 4.06-15(a)

Where the length of the compartment is less than 12 feet, one vertical means of escape will be acceptable. Engine room is greater than 12 feet requiring more than one vertical means of escape. Marine Inspectors observed seized emergency escape hatch for engine room. 46 CFR 177.15-1(b)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks. During the Internal Structural Exam conducted on 24JAN2024, Marine Inspectors identified cracking on the watertight bulkhead separating the rudder room from the machinery space. Cracking is concentrated around the STBD main engine exhaust bulkhead fitting. Submit repair proposal to [email withheld] and once USCG approval is granted, complete repairs during next scheduled dry docking.

Electrical installations on vessels shall be such that (i) services essential for safety will be maintained under various emergency conditions, and (ii) the safety of passengers, crew, and vessel from electrical hazards will be assured. MI observed 9 connections at each battery terminal under the console servicing navigation equipment. Effect repairs reducing the electrical hazard to the satisfaction of the MI.

EACH RING LIFE BUOY ON A VESSEL MUST BE APPROVED IN ACCORDANCE WITH 46 CFR 160.050 IN SUBCHAPTER Q. VESSEL HAD 02 LIFE BUOYS THAT WERE WATER LOGGED. VESSEL MUST REPLACE 02 LIFE BUOYS TO THE SATISFACTION OF THE MARINE INSPECTOR.

THE MEANS USED TO ATTACH THE FLOAT-FREE LINK TO THE VESSEL MUST BE RESISTANT TO DETORIORATION FROM THE ULTRAVOILET LIGHT. 01 LIFE-FLOATS LINES WERE FOUND TO BE DETRIOATED AND MUST BE REPLACED TO THE SATISFACTION OF THE MARINE INSPECTOR.

REMOTE FUEL SHUTOFF STATIONS MUST BE MARKED IN CLEARLY LEGIBLE LETTERS AT LEAST 1 INCH HIGH INDICATING PURPOSE OF THE VALVE AND DIRECTION OF OPERATION.

A DURABLE PLACARD MUST BE POSTED NEXT TO ALL RADIOTELEPHONE INSTALLATIONS WITH THE EMERGENCY BROADCAST INSTRUCTIONS AND INFORMATION SPECIFIC TO THE INDIVIDUAL VESSEL. THE ONBOARD INSTRUCTIONS WERE NOT SPECIFIC TO THE VESSEL AND MUST INCLUDE VESSEL SPECIFICS TO THE SATISFACTION OF THE MARINE INSPECTOR.

INTERNAL COMBUSTION ENGINE EXHAUST AND SIMILAR SOURCES OF IGNITION MUST BE KEPY CLEAR OF AND SUITABLY INSULATED FROM COMBUSTIBLE MATERIAL. BOTH ENGINE EXHAUST INSULATION MUST BE RE-APPLIED TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

EACH LADDER USED AS A MEANS OF ESCAPE MUST BE MOUNTED AT LEAST 7 INCHES FROM THE NEAREST PERMANENT OBJECT IN BACK OF THE LADDER. THE ESCAPE LADDER IN THE MALE HEAD WAS NOT SECURELY FASTENED. VESSEL MUST SECURE THE ESCAPE LADDER TO THE SATISFACTION OF THE MARINE INSPECTOR.

FLEXIBLE HOSE RUNS MUST BE VISIBLE, EASILY ACCESSIBLE , PROTECTED FROM MECHANICAL DAMAGE. CHAFFING WAS NOTED IN THE HYDRAULIC LINES. VESSEL MUST SECURE THE HYDRAULIC LINES TO THE SATISFACTION OF THE MARINE INSPECTOR.

EACH POTABLE WATER TANK SHALL BE PROVIDED WITH A MEANS TO PREVENT ENTRANCE INTO THE TANK OF ANY CONTAMINATING SUBSTANCE. VESSEL MUST REPLACE THE FRESH WATER SCREEN TO THE SATISFACTION OF THE MARINE INSPECTOR. 21 CFR 1250.82(c)

Repair holes in starboard side ventilation enclosures preventing vents from closing.

Repair FCC-approved RADAR.

Replace junction box on lifefloat deck.

Remove dead-end wires in center compartment or provide suitable enclosure.

Prove proper operation of fire pump.

Port engine exhaust leak

Life jacket lights did not work.

Misssing nav rules

Engine room bilge under port and starboard shafts under shaft seal area has corrosion. Require crop and renew or provide gauging report.

Aft house bulk head new main deck has a couple of hole in bulk head. Required POC to remove panalling inside house to get a better look at how much metal is wasted. Cut out damage area and weld in new plate.

Bulk head penatrations for port side engine throttle cables on forward engineroom bulk head need replacing.

2 welds in the forward peak tank were fractured. Required POC to gouge out and reweld

Replace painter line on starboard, top life float.

Make all strainer openings capable of taking suction by increasing opening size to three times pipe diameter opening.

Terminate dead end wire in steering component in port overhead.

Crop and renew bottom plating where excessive pitting has occured.

Provide means of attaching all hatch covers according to 46 CFR 179.360 (a)(2).

Label both sides of emergency hatch in way of engineroom according to 46CFR185.606.

Provide means to prevent chafing of throttle cable where cable passes through forward machinery space, aft bulkhead.

Provide on board a Compass Deviation Card.

Provide an Employee Assistance Program and associated training for the crewas required by 46 CFR subchapter A, part 16.

Provide on board a valid FCC Radio Safety Certificate.

GUAGE PLATE IN WAY OF PORT AND STARBOARD A/C UNITS, AND REPLACE AS REQUIREDBY LOCAL OCMI OR ATTENDING INSPECTOR.

MAKE DECK IN WAY OF PORT AND STARBOARD HEADS ACCESSIBLE FOR INSPECTION OR PROVIDE METHOD TO DETERMINE SOUNDNESS OF DECK PLATING.

REPLACE ALL HATCH COVER GASKETS ON MAIN DECK.

PROVIDE PFD DONNING INSTRUCTIONS.

REPLACE BATTERY ON EPIRB.

PROVIDE COMPASS DEVIATION TABLE.
