When only the main steering gear is power-driven, two separate and independent systems for full followup control must be provided in the pilothouse; except that the steering wheel or lever need not be duplicated. Vessel's control system for full followup has two slices offline. Vessel is to bring control system fully operational to the satisfaction of attending marine inspector within 14 days.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), except as otherwise provided by this subchapter. The #1 main engine has a fuel oil leak and is to be secured until repair. Vessel is to submit root cause analysis, repair, and conduct operational tests to the satisfaction of the attending marine inspector.

Each Vessel must have an illuminated magnetic steering compass, mounted in a binnacle, that can be read at the vessel's main steering stand. Vessel reported magnetic compass still need to be serviced after drydock. Vessel must have tech come out to properly remove air from compass and ensure it is fully functional.

Approved fire extinguishing systems must be installed, as required by Table 76.05-20 on all self-propelled vessels. Vessel conducted removal and cutting of piping joints and fittings in the sprinkler piping system to conduct water jetting during drydock. Complete satisfactory operational test of sprinkler system in the presence of USCG marine inspector.

Where extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the requirements of this part. #1 end reduction gear room hypermist system secured. Failure in solenoid rendered system inoperable. 46CFR76.01-5(a) 16c Deficiency extended to next DDX due to difficulty sourcing parts, Code 705

Doors in “A” Class stairway enclosures and Main Vertical Zone bulkheads, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. The release mechanism shall be so designed that the door will automatically close in the event of disruption to the control system. FSD 25 (double-swing) no fully closing, stairway FSD 30 (sliding) not closing, and MVZ FSD 15 not closing. Keep doors in closed position until repaired and closing as required.

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be, and then enter an attestation statement of the verification in the log book. Vessel's ADIS not operating and no attestation statements have been made attesting to manual draft readings. Provide evidence of ADIS functioning or log entries of attestation entries. 46 CFR 78.17-22

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs. Vessel experienced a soft grounding while underway and is authorized to proceed from Orcas Island to Friday Harbor. Any further vessel movement must receive approval from USCG Marine Inspector. 46 CFR 71.40-1, 60C

Auxiliary machinery vital to the main propulsion system must be provided in duplicate unless the system served is provided in independent duplicate, or otherwise provides continued or restored propulsion capability in the event of a failure or malfunction of any single auxiliary component. Vessel currently has inoperable #1 MDE. 46CFR58.01-35

Rescue boat davit remote brake cable broken. Vessel capacity reduced to half.

Provide proof that each fire/smoke detector installed has been tested annually. 46 CFR 71.25-20, 61.40-6, SOLAS 74 II-2/13

Replace both the No.1 and No.2 heat boiler safety valves. Both valves had excessive bleed-by at the stem. 46 CFR 53.05-1

Provide a means to acknowledge the annunciator for the control air 80 psi alarm. 46 CFR 62.25-20

Prove that the overspeed trip for the E-gen functions within its set points (PSTP AGE-3). 46 CFR 62.35-50

Prove that the No.1 SSDG (vital) lube oil alarm/shutdown functions within its set points (PSTP AGE-1). 46 CFR 62.35-50

Where the emergency source of power is a generator their shall be provided a temporary source of power to supply emergency lighting for 30 min. Inadequate e-lights on temporary power throughout the vessel’s egress points. 46 CFR 112.05-5(a), SOLAS 74 II-1/25.

Prove the proper operation of the GA bell No.23 on the car deck next to the slide. 46 CFR 113.

Prove the operation of the No.1 and No.2 MDE lube oil shut down and alarm (PSTP MDE-3). 46 CFR 62.35-50

Conduct the vessel’s approved PSTP for all items not completed. 46 CFR 61.40-6

Dryer vent line must be connected to an exhaust to the outdoors as per manufactueres installation instructions. 705C: Do not use until rectified.

A certificate called a Passenger Ship Safety Certificate shall be issued after an initial or renewal survey to any ship on an international voyage carrying more than 12 passengers and shall be issued for a period not exceeding 12 months. Vessel does not have a valid Passenger Ship Safety Certificate for international voyages. SOLAS 74 (2020 Cons Ed) Ch.I Reg 12(a)(i) and Reg 14(a)

Companies shall ensure that valid documents are available at all relevant locations, changes to documents are reviewed and approved by authorized personnel, and obsolete documents are promptly removed. Vessel had possession of original/copied documents that were obsolete including older International Air Pollution Prevention Certificates, Security Documentation, Certificates of Class, and Certificates of Inspection. Ensure all statutory surveys/audits are completed. SOLAS 74 (2020 Cons Ed) Ch. IX Reg 3.1; ISM Code 11.2

Reported an oil leak IWO their No. 1 end reduction gear shaft seal between the reduction gear and the propulsion clutch. Vessel to determine root cause and prove proper operation of system prior to the carriage of passengers. 46CFR58.05-1

Call box malfunction disabled fire alarm system. Must repair call box prior to operations.

Vessel has expired line throwing appliance onboard, must replace expired appliance with valid one.

Hydrostatic release for EPIRB expired, replace expired hydrostatic release with valid one. Vessel restricted to 3 NM from shore until hydrostatic release replaced.

Must obtain NOAA Registration for EPIRB. Vessel restricted to 3 NM from shore until registration is completed.

Ensure chart corrections from LNM are completed on a weekly basis.

Malfunctioning Fire Detection System

Provide communication interface and data logging between fire detection system and VDR, and demonstrate proper operation to the satisfaction of the attending marine inspector. SOLAS (14) CH V. 20 IMO Res. A. 861(20) 50C

Prove proper operation of the fire mist extinguishing system without pump faults IAW SOLAS (74)II-2 10.6.1.1.

Provide IAMSAR (International Aeronautical and Maritime Search and Rescue) Manual IAW SOLAS (74) V-21.

Provide proper operation of the 4 inop general alarm bells located at: No. 1 upper, wing, car deck, at frame 42. Port and starboard side, no. 2 lower wing, upper port side, and no. 1 lower wing, frame 42 IAW SOLAS (74) III-6.4.2.

Prove proper operation of public address system IAW SOLAS (74) III-6.5.

Prove proper operation of the scale used to measure the weight of the main CO2 bottles dated 1995. Ensure all the fire servicing personnel are certified by NFPA or local authority. Follow company's CO2 servicing procedures.

Lable the general alarm bell on the no. 2 car tunnel IAW 46 CFR 78.47-7.

Stow and label distress flares on or near the navigation bridge SOLAS (74) III 6.3.

Provide energy efficiency certificate IAW MARPOL Annex VI reg 6.

Determine cause of low lube oil alarm for watertight doors #1, #2, and #6 IAW 46 CFR 170.270 (e).

Provide proof of periodic testing of lifesaving equipment. Daily, weekly, and monthly tests outlined on form No. EMER 8C (05/18)(Deck) are not documented IAW SOLAS (74)20.

Automatic draft indication system is not functioning. Utilize manual verification until system is repaired IAW 33 CFR 164.11.

Lube oil pressure alarm and shutdown occur concurently at 25 psi on all generators. Correct IAW 46 CFR 62.25-20.

Provide fuel samples for not less than the previous 12 months IAW MARPOL Annex VI 18.6.

Remove dead end wire in #2 end stack that leads through bulkhead #1 endstack IAW 46 CFR 111.01-1.

Provide bunker delivery note for inspection. It shall be retained on board for a period of three years IAW MARPOL Annex VI 18.1.3.

WTDs #4 and #5 in #2 end, the local hydraulic hand control detent lever is not operating properly. If not in exact alignment, the door will close on it's own.

Fire control plan does not reflect pfd's for crew IAW SOLAS (74) II-2 2.4.1.

Complete IAPP survey prior to current IAPP's expiration date prior to sailing internationally IAW MARPOL Annex VI

Any automatic water sprinkler system for fire protection required in accordance with Regulation 51 of this Chapter shall be ready for immediate use at any time, and no action on the part of the crew shall be necessary to set it in operation. Where such a system is fitted, it shall be kept charged at the necessary pressure and shall have provision for a continuous supply of water. The vessel reported a pin hole leak in the number 1 zone's hyper mist water suppression system. The vessel shall not carry passengers in the number 1 zone until repairs are made.

Prove proper operation of #1 end rescue boat davit launching arrangements and determine causal analysis of damage caused by earlier event. Vessel is restricted to one half passenger capacity IAW with the vessel's COI which is 600 of the 1200 passengers allowed.

Power operated watertight doors shall operate in accordance with ASTM F1197, incorporated by reference, and in no case shall the closure time be less than 20 seconds and more than 40 seconds. #5 watertight door took greater than 45 seconds to fully close. 46 CFR 111.97-3

The system shall be so designed and arranged that the overhead is effectively sprayed and all portions of the deck are covered. Both sprinkler heads at the bottom of the port side ladder-wells were found with cylinder protective devices that would obstruct spray pattern and prevent system from operating as designed. 46 CFR 76.23-10(a)

Each drill, the date held, description of drill, list of participants, and best practices or lessons learned must be recorded. Vessel currently maintains only the date of the drill and brief description. 33 CFR 104.235(b)

Automobiles shall be stowed within marked lines that indicate vehicle runways and aisle spaces. Vehicle was parked across egress route to a MES. Notified crew member and Chief Mate of discrepancy and corrected on spot.

Drills must, as far as practicable, be conducted as if there were an actual emergency. The fire team responded without appropriate PPE including gloves and going on supplied air and self simulated without proper notifications. Conducted debrief with crew and Chief Mate to address discrepancies. 46 CFR 78.40-1(a)

All doors except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. Fire screen doors #32 and #34 did not completely close. Deficiency rectified during the inspection.

Provide proof that all fire detectors have been tested annually by introducing smoke or heat.

The safety valve on the No.2 heating boiler would not reset. Repair or replace the safety relief valve and demonstrate proper operation to the USCG prior to placing the boiler back into service.

Non-metallic flexible hoses may be used for fluid power systems only where flexibility is required. A hose was installed to replace tubing for the control system on the No.1 WTD. Return the system to original.

Alarms must be independent of each other. Separate the MDE's J/W high temp from manifold air high temp from each other.

Provide a means to silence the low control air alarm. It continuously sounds when activated providing a nuisance to the watchstander in the machinery control room.

Provide a 30 minute automatic transitional power source for the following loads: Machinery spaces and control room, Damage control lockers, Steering gear room, Fire control stations, Passenger egress routes, Internal communication system (PA System), Navigational lighting.

Firemain recirc line has 1/8" hole which is currently mitigated with a soft patch. Semi-permanent repair (doubler) must be completed within 24 hours and a permanent repair must be completed at vessel's December 2018 availability.

On 15 Aug 2018, the CHELAN experienced a loss of steering at the Anacortes ferry terminal. Slice A (of the steering Vessel Control System), a single slice, caused a failure of the entire follow-up system. According to the CHELAN's SMS, "If one system fails, the remaining two systems are able to carry on without losing function or interrupting service". Failure of one slice should not cause a catastrophic failure of the steering system. Provide a report detailing root cause analysis, including how one slice failure shut down the entire system. Provide corrective action plan to prevent recurrence.

Every Company should develop, implement and maintain a safety management system which should follow the functional requirement: procedures for reporting accidents and non-conformities with the provisions of the ISM Code. Objective evidence exists of a major non-conformity and that the vessel is not in compliance with the SMS as follows: The lack of notification for reporting the follow-up steering failure, a hazardous condition; the lack of entries made into deck log/watch handover notes explaining the steering problem; the failure to follow SMS procedure ENGR EMER 0080, ENGR COMM 0020, SMSM REVU 0020, and SMSM CODE 0010 10.1 and 10.2; the lack of any known entry into the SMS for the steering non-conformity; and WSF removing the vessel from service due to the steering non-conformity.

Pin hole leak in fire main along weld seam on recirculation line. Re-weld to satisfaction of CG Marine Inspector. Temporary FRP patch applied from 06May18-07May18; vessel allowed to operate. Temporary patch not allowed after evening of 07May18.

Remove flex hose from the fuel oil pump to the burner nozzle on the #1 Boiler and replace with seamless steel pipe/tubing IAW 46CFR56.50-65.

Repair or replace general emergency red flashing light in engine room #2 IAW 46CFR113.25-10.

Remove dead ended wires in the main cable transit between the #2 engine room and the #2 reduction gear space and seal the cable transits to the equivalency of the bulkhead boundary rating IAW 46CFR111.60-17.

Repair or replace Engine Order Telegraph's loss of power audible alarm in the #1 pilothouse IAW 46CFR113.35-5.

Replace expired ring buoy waterlight batteries.

Ensure fire screen doors #21 and #25 properly secure upon release.

Effect repairs to 03 clogged sprinkler heads. (Zone B, Zone 2, and Zone 4)

crack standoff leading to flange for crankcase negative pressure. Exhaust must be repair at night tie up.

#1 Rescue boat is not operational. Passenger count is limited to 600.

Class 3 WTD alarms do not meet the standards found in ASTM 1197 11.5 as required by 46 CFR 170.270(c)(2). Alarms continue to sound after all doors are closed.

WTD's # 5&6 do not automatically reclose on release of local control handle when in "Doors Closed" mode per ASTM 1197 8.1.2.2(2) as required by 46 CFR 170.270(c)(2).

Automatic bus transfer for emergency generator is not functioning properly. Repair/replace automatic bus transfer. 46 CFR 112.25-10(a)

Witnessed operational testing of all sprinkler zones. Noted clogging of (17) heads that resulted in reduced water flow in the following locations, Zones A, B, C and 3.

Condemn one crew lifejacket.

Inform the Coast Guard when the non-SOLAS approved outboard engine on the rescue boat has been replaced with a SOLAS approved outboard engine. Vessel is restricted to US routes.

The fire door between the EOS and the #1 Engine Room was not shutting completely.

Received notification from [name withheld](Capt.), that the GPS Electronic Positioning System was down.

Replace white quick release lifebuoys with heavier international orange lifebuoys.

Vsl's firemain piping system in way of fire pump discharge recirculating pipe to the # 2 sea chest had a pin hole leak. Crew applied soft patch which appeared to be holding satisfactorily. Required vsl to make permanent repair to the leaky pipe. Crew will conduct temporary repair with a weld bead application to the pin hole.

#1 and #2 engine low jacket water level alarm was found inoperable.

#1 end MIST system pipe was supported by a rubber bungee cord to stop vibrations.

Two pinhole leaks in firemain piping in the #2 end, located above inboard boiler on piping for zone 7, and on pipe elbow under the doorway for the entrance to the engine room.

Numerous sprinkler heads blocked at time of inspection.

Fire screen door did not latch properly.

Replace # 1 Generator main breaker due to faulty tripping mechanism and conducted operation testing with switch gear system.

Label all lighting on emergency circuit and replace burnt out light fixtures.

Re-label fire axes located on 1 and 2 side car deck with vessel name .

Ensure a sign is placed indicating where lifejackets are located on car deck. Passengers should be able to find the lifejacket pull down from placard instructions.

Vessel was carrying old, kaypak lifejackets. All the cotton straps were deteriorated and riped. Vessel carries 1276 adult and 131 child, only 322 adult life jackets passed and 43 children passed. LEGACY COMPONENT DESC: Condition

Lifejackets were spot checked during quarterly inspection and several were found to have no or faded stencil of vessel's name. Talked with WSF operations and decided descrepancy will be fixed at next annual inspection-Nov 26-02 at Eagle harbor. LEGACY COMPONENT DESC: Marking
