The construction and arrangement of a vessel must allow the safe operation of the vessel in accoredance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. On both rudder posts, the packing flange bolts are loose and bent. Provide evidence that the bolts are properly torqued prior to 2025 Hull Exam credit.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry— (1) Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. During inspection, vessel flares onboard had just recently expired.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Operational test of the means provided for pumping bilges. During inspection, inspector was able to witness satisfactory bilge suction in forepeak void, however, the rest of the bilge suction points throughout the vessel required additional arrangements in order to test due to tight proximity of the suction point in way of the hull. Crew shall make arrangements necessary to conduct bilge suction test to the satisfaction of the attending marine inspector.

During the internal structural exam, several cracked welds were discovered in the forepeak void and up forward on the starboard side. Starboard bow railing is still structurally sound, however, the vertical support structures have cracked welds that have begun to separate from the deck. In the forepeak void, on the upper starboard side near the hatch, there are also cracked welds along the transverse beams meeting with the starboard side shell. Crew shall monitor for crack progression until permanent repairs are made prior to next drydock credit (due by 30SEP2025).

Each vessel constructed of a material other than wood certificated to operate on a coastwise route must be provided with life floats of an aggregate capacity that will accommodate at least 100% total number of persons permitted in board. At the time of inspection, vessel was missing one of two 25 person liferafts listed on the vessel's COI. Vessel currently has one 25 person liferaft. Vessel is limited to carrying no more than 25 total persons on board until second liferaft is onboard and installed.

A fire hose with the nozzle must be attached to each fire hydrant at all times. While testing the fire pump it was identified that the hose and nozzle on board were not compatible and could not be connected. Replace hose or hose with an approved type and prove proper operation of hose and nozzle.

Each life jacket must be marked with Type 1 retroreflective material approved under 164.018. 46CFR180.25-25 Life jackets were found with failing retroreflective tape.

Vessels shall carry sufficient life floats for all persons onboard that comply with 160.027. 03 Life Floats were unsatisfactory condition and deemed unfit for service. 46CFR180.10-5

A vessel 20 meters or more must be provided with a bell and whistle. Whistle was not operational.

At each inspection all machinery essential to operation including cooling systems shall be examined. Port and STBD main propulsion engines had coolant leaks. STBD generator also had a coolant leak.

A placard must be posted next to all radios with emergency broadcast instructions specific to the vessel. The required emergency broadcast placard was missing.

A bilge high level visual and audible alarm must be provided at the operating station. The audible alarm was not load enough to be heard over machinery.

When operating on a body of water where the discharge of sewage is prohibited the overboard valve must be secured The sewage overboard valve was not secured in the closed position.

Crew accommodation and work spaces must be adequately constructed to provide for safe accommodation and protection of the crew. A hole was found in the galley floor that must be securely covered.

Electrical equipment must be installed and maintained to protect passengers and crew from electrical hazards to include fire and electrical shock. Multiple power strips were observed onboard the vessel and emergency lighting in the engine room was hanging by the wire.

Doors shall be provided with heavy duty hinges and locking devices to prevent accidental opening in heavy seas. The oven and refrigerator do not have a means to lock the door/doors.

Vessel is limited to operating with a max of 12 persons until the full compliment of life floats IAW the COI, are onboard.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Starboard diesel generator and all three main diesel engines missing guards for exposed rotating machinery on forward area.

Every vessel must be fitted with a suitable magnetic compass. Except on a vessel limited to day time operations, the compass must be illuminated. Compass was not iluminated. deviation table missing.

A vessel must be provided with a steering gear that is capable of steering the vessel at all service speeds. Steering was hunting/jerking when moving to STBD.

Each vessel mus the equipped with two portable battery lights, one at the operating station and one at the propulsion room entrance. Vessel had no portable lights onboard.

The general construction of the vessel as to minimize fire hazards. The dry vent hose had holes and lint trap was behind a bulkhead where it could not be cleaned.

Vsl hit a submerged log and bent the port and center propellors

Inset in side shell plating, port side where side shell meets forward engine room bulkhead. Bulk head and side shell inset beyond allowable limits.

A hole through the side shell plate was improperly repaired by clad welding. Hole is located in the rudder room, starboard side approx. 3" below the main deck.

Life float stowage, Life Floats were not in good working order and ready for immediate use.

Starboard side Life Float had damaged, deteriorated webbing material and was not in serviceable condition.

All Life Float painters were not installed correctly IAW 46 CFR 180.137

Painter for port side group of Life Floats was deteriorated and in un serviceable condition.

Provide a light cover for the light in the engine Room on the port side, as per 46 CFR 183.410.

Replace the hydraulic line and repair the hydraulic leak on the clutch on the Stbd MDE, as per 46 CFR 182.200.

Vessel has heavy pitting on hull. Vessel is cropping and renewing keel and other pitted areas.

Emergency fuel shutoff is broken.

Need to replace links.

Need to replace 4 life jackets.

Replace gasket in WTD IWO E/R.

Replace ring around Escape scuttle in E/R.

Repair/replace lantern in passage way.

Silicone was used instead of approved gaskets on water tight scuttles.

Mechanically fasten all loose wiring in E/R.

Remove all dead end wiring IWO port and stbd generators.

Secure washer and dryer.

Stencil emergency exits.

Label all emergency lights.

Crop and renew pin holes in deck iwo fuel fill.

Properly ground fuel fill piping.

Log monthly EPIRB tests in ships log.

Prove proper operation of fire pump and make vessel ready for Coast Guard Inspection.

Crop and renew port and starboard sides of water tank located in lazzerette due to fractures and leaks.

Prior to vessel departing Houston/Galveston COTP zone, make vessel availible for post casualty inspection by local OCMI

Prior to vessel movement, make proper repairs to port generator fuel oil supply line

Proceed directly to Baron's shipways, Freeport, TX and make vessel available for drydock inspection for attending CG inspector.

Vessel sailing short 1 person. COI required 1 master and 2 deckhands. Vessel had 2 masters and no deckhand.
