Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge (in a readily visible location) and with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. MI found the pressure vessels located in the engine room are stamped with a MAWP of 200 psi and fitted with a 250 psi relief valve, exceeding the 10 percent allowable maximum pressure. 46CFR143.300(a)

Installed electrical power source(s) must be capable of carrying the electrical load of the towing vessel under normal operating conditions. Port and Stbd generator windings found heavily oiled and with dirt/debris. Provide insulation resistance test with a megohmmeter conducted in accordance with manufacturer instructions. Test results must include the resistance measured between each winding and from each winding to ground. Include recommendations on whether windings shall be cleaned, rewound, or if they are within manufacturer insulation resistance specifications. MI identified two extension cords daisy chained together with two multi outlet surge protectors at the main operating station. The breaker for the use of this was incorrectly identified on the breaker panel. The 110 V conductor for this installation was found jumped at the double pole breaker identified for the 220V outlet in the pilot house. MI identified dust and dirt in the breaker panel when disassembled by the crew. The crew cleaned the breaker panel box and removed the 110 V conductor from the 220 V double pole breaker. 46CFR143.400(a)
