Provision shall be made by means of plugs, covers, etc. to percent admission of air into the space. Ventilation louvers would not shut for both engine room spaces. 46 CFR 181.20-35 (a)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Port side mid-compartment and port side engine room bilge alarm is not operational. 46 CFR 182.530(a)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position or perform the duties and functions of a position, required by the vessel's COI. Marine employers may form or otherwise use sponsoring organizations, or may use contractors to conduct the random chemical testing programs. Vessel's O/O is developing a chemical testing program to include random-testing by third party, but has not completed the program. Provide satisfactory evidence of a chemical testing program. 46 CFR 16.230(a)

Where life preservers are stowed so that they are not readily visible to passengers, the containers in which they are stowed shall be marked "life preservers" and with the number contained therein, in at least 1-inch letters and figures. This legends shall indicate the separate stowage of children's life preservers. Missing port and starboard bench seating stowage markings for adult and child life preservers. 46 CFR 180.25-15(b)

A submersible electric bilge pump may be used as a power bilge pump only on vessels of not more than 65ft in length carrying not more than 49 passengers, provided that: The pump is permanently mounted. Bilge pumps and floats must be permanently mounted. 46 CFR 182.520(e)(3)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Engine room bilges were full of debris and rags. 46 CFR 176.830(b)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The bilge pipe on the port side mid-compartment was not properly secured. 46 CFR 176.830(a)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The battery banks located in both engine compartments have overloaded terminals. Provide an additional terminal post so no more than four cables/wires are on each post, an ensure locking nut is fully threaded on. 46 CFR 176.800(a)

If your vessel is not operated on international voyages and does not meet the conditions in paragraph (d) of this section, it must undergo a drydock and internal structural examination as follows unless it has been approved to undergo an underwater survey (UWILD) per 176.615 of this part: (1) A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo drydock examination and an internal structural at least once every two years. Vessel's drydocking due date of June 29, 2024 has lapsed and vessel is overdue for credit drydocking. Vessel is not to operate with passengers onboard or conduct any dry docking repairs unless approved by the attending marine inspector. 46 CFR 176.600(c)(1)

An adult life jacket must be provided for each person carried on board a vessel. Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. 1 Life jacket was ripped and unserviceable. 14 of required 140 life jackets onboard had wasted or missing retroreflective material. Replace 1 unserviceable life jacket. Install retroreflective material on 14 life jackets. Vessel passenger count reduced from 128 passengers to 113 passengers until deficiency is corrected. 46 CFR 180.71 46 CFR 185.604

Endorsements on Certificates of Documentation are valid for one year. Vessel's Certificate of Documentation expired on April 30, 2023. Provide proof that vessel owner has applied for renewal and received endorsement for a valid Certificate of Documentation. 46 CFR 67.163

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician once every five years. Vessel's FCC Safety Radiotelephony certificate is expired. Provide proof of a valid FCC Safety Radiotelephony certificate to attending marine inspector. 47 CFR 80.903

The Certificate of Inspection must be posted under glass or other suitable transparent material. Vessel's valid COI is not posted or kept onboard. Post vessel's valid COI in appropriate location. 46 CFR 176.302

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmemeber assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Community service hot-line number and employer's EAP policy are not posted or kept onboard. Display employer's EAP policy and hot-line number onboard to the satisfaction of the marine inspector. 46 CFR 16.401

Vessels less than 65' carrying more than 49 passengers shall be outfitted with one fixed, power, bilge pump that is permanently connected to the bilge main. Both of the provided power bilge pumps were unable to dewater any of the compartments served by the bilge main. Vessel passenger count limited to 49 until the vessel is provided with a fixed, power, bilge pump capable of dewater all watertight compartments on the vessel. 46 CFR 185 2.25-10

An adult life jacket must be provided for each person on board a vessel. 12 Adult life jackets were either missing or un-serviceable. Vessel is restricted to 116 passengers until all life jackets required by the COI are provided. 46 CFR 180.71(a)

Emergency exits used as a means of escape must be marked in legible letters "EMERGENCY EXIT, KEEP CLEAR". Markings provided were worn an illegible. Provide required markings. 46 CFR 185.606

An audible and visual bilge high level alarm is required at the operating station for normally unmanned spaces with a non-watertight closure on the main deck. The audible and visual high water indicator for the port fuel tank space was inoperable. Provide the required alarm. 46 CFR 182.530

Deck rails must be designed and constructed to withstand a point load of 200 pounds. The starboard forward stanchion, second from outboard and the starboard, aft stanchion for the forward ladder railings were not secure to the deck. Repair and secure to the satisfaction of the attending marine inspector. 46 CFR 176.25-1 46 CFR 177.900

A vessel must comply with the structural design requirements of one of the standards listed. The fiberglass in way of the strut mount for the port and stbd hulls were cracked and presented indications of water intrusion and delamination. Grind back for further inspection. Hull material found wasted, crop and renew per approved plans. 46 CFR 177.300

The owner or operator shall make a vessel available for drydock examinations. Vessels drydock was due 3/31/2020. Vessel must complete credit drydock with attending Marine Inspector prior to carrying passengers.

Each hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices to prevent its loss; vessels hatch covers does not have any device to prevent its loss. Attach hinges, captive chains or another device to prevent loss of hatch cover.-(OLD)46 CFR 171.124(b)(2)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Vessel's starboard fuel tank void and port side forward void visual indicators are inoperable. Repair visual indicator for each void.-46 CFR 182.530(c)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating; Vessel's port midship void visual indicator indicates port midship void, port engine room, and port steering room is operating when only port midship void bilge pump is operating. Vessel must repair visual indicators to indicate when the correct bilge pump is operating.

Only one pre-engineered fixed gas fire extinguishing system is allowed to be installed in each space protected by such a system. Cross space unites port and starboard engine rooms, thus creating one space. Secure cross space to separate engine rooms-46 CFR 181.420(c)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system; Closure devices for port and starboard engine room spaces does not properly close to secure ventilation. Repair closure devices.

A vessel is inspected for compliance with the standards required by this subchapter. Repairs and alterations that affect the safety of the vessel must not be made without the approval of the cognizant OCMI; Vessel's swimming platform starboard support bracket is cracked at welding. Repair cracked welding on support bracket. Passengers are not allowed on swimming platform until repaired.-46 CFR 176.700(a)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment under all practicable conditions. Submersible electric power pumps in port side steering void, starboard mid void, and piping for fixed power hand pumps are not secure. Secure submersible electric pumps and piping for fixed power pumps - 46 CFR 182.500(a)

Repairs and alterations that affect the safety of the vessel must not be made without the approval of the cognizant OCMI; Vessel's forward port cleat has loosened and cracked the surrounding deck. Make surrounding deck around the cleat watertight.-46 CFR 176.700(a)

Deck rails must be designed and constructed to withstand a point load of 200 applied at any point in any direction. Forward deck rails are loose from deck. Tighten stanchions.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct test and have vessel ready for inspection of the hull structure, electrical, and machinery. Vessel was not prepared for annual inspection. Vessel is restricted from operating with passengers for hire until a satisfactory annual inspection is completed as required by 46 CFR 176.500.

Port and Stbd ventilation shutdowns for engine room are inoperable. Renew ventilation shutdowns to create vapor tight seal in space for fire suppression system.

Make entire engine room space vapor tight for fixed fire suppression system in port and stbd engine space. Multiple holes found in the overhead and forward bulkhead.

Passengers are restricted from using aft swim step ladder until the proper repairs are made to the following: cracked fiberglass stairs on the stbd transom, cracked fiberglass transom where swim step support detached, disconnected railing on port side and under wooden swim step.

Witnessed excessive movement at the tiller arm connection to the hydraulic arm on the port and starboard rudder. Operator will disconnect for further inspection of linkage.

Repair seven inch long cracked and softened passenger deck located starboard side, five feet forward of the doorway leading to the forward passenger deck.

Replace or renew severed grounding wire on the port and starboard fill pipe for the fuel tank.

Replace cracked flexible hose connected to the thru hull valve in the starboard hull used to provide sea water to the MSD tank.

Provide written weather limitations and in-water briefing instructions for the crew to comply with reduced manning for snorkel operations IAW Sector Hono CID work instruction 31.

Replace ring life buoy and water light battery with USCG approved ring life buoy and battery for water light.

A/C generator positive conductor ring terminal connection hole is larger than the nut used to fasten it. Replace the nut or ring terminal connection with appropriate size.

Provide double hose clamps on generator exhaust discharge piping located in the port engine compartment.

Provide over current protection IAW 46 CFR 183.380.

Replace all electrical terminal connections and provide covers for terminal connections in the port and starboard machinery space.

Overcurrent protection fuse found severely corroded in the D/C circuit. Replace overcurrent protection IAW 46 CFR 183.380.

Battery storage box located in the starboard machinery space found is constructed of wood. Wood rot found under the box and on both sides. Wood supports for the battery storage box found disconnected from the battery storage box due to wood rot.

Flexible hoses on raw water cooling system for port and starboard main engine found with soft spots, kinks, cracks, and holes. Hose clamps fell off of flexible hoses when touched by hand. Replace raw water cooling hoses in kind for port and starboard main engines.

Port main engine exhaust flexible piping is kinked at the thru hull penetration due to poor support for the entire exhaust piping. Starboard main engine exhaust is supported by nylon straps previously used on lifejackets. Provide adequate support for exhaust piping on the port and starboard main engines.

Port main engine fuel delivery line found kinked just before the primary fuel filter in the port engine compartment. Replace fuel delivery line IAW 46 CFR 182.20-40.

Repair exhaust leak on the starboard main engine exhaust. Leak detected at the thru hull penetration.

Repair in accordance with approved plan.

Grind back the following identified areas of wood rot to good wood and submit repair proposals for: line locker hatch hinge seats, and hatch cover starboard side, second void aft from bow starboard side ring frame in the vicinity of the keel and forward bulkhead beam, starboard forepeak, two aft overhead frames. Repair in accordance with approved plan.

Grind crack on port forepeak deck for further inspection.

Starboard side, third void aft from bow, renew gasket, provide means of dogging hatch down, and attach line from vessel to hatch cover.
