No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspections. Vessel had unauthorized portable accommodation modules installed on board. There was a 12-person sleeping accommodation and dive control module that were installed without prior approval from Coast Guard or ABS which is the organization that issued the vessel’s stability letter and tonnage certificate in accordance with CG-ENG Policy Letter 01-16.

Electrical installation on vessels must ensure: (3) Maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. The onboard generators that are shown in system drawings are inoperable. Portable generators were acquired without notifying the OCMI and without submission of plans to show modification. They were also installed without oversight and no longer meet standards found in subchapter J and F including location, cable supports, cable penetrations, and alarms among other items. The fuel oil purifier located below deck near the fuel tank and pump was installed to drain directly to the bilge. The bilge was modified to hold fuel and approximately 6 inches of fuel was found in a 12’ X 6’ area of the bilge.
