No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspections. Vessel had unauthorized portable accommodation modules installed on board. There was a 12-person sleeping accommodation and dive control module that were installed without prior approval from Coast Guard or ABS which is the organization that issued the vessel’s stability letter and tonnage certificate in accordance with CG-ENG Policy Letter 01-16.

Electrical installation on vessels must ensure: (3) Maintenance of system integrity through compliance with the applicable system requirements (IEEE, NEC, IEC, etc.) to which plan review has been approved. The onboard generators that are shown in system drawings are inoperable. Portable generators were acquired without notifying the OCMI and without submission of plans to show modification. They were also installed without oversight and no longer meet standards found in subchapter J and F including location, cable supports, cable penetrations, and alarms among other items. The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. The fuel oil purifier located below deck near the fuel tank and pump was installed to drain directly to the bilge. The bilge was modified to hold fuel and approximately 6 inches of fuel was found in a 12’ X 6’ area of the bilge.

The location of the means of escape shall be as remote as practicable so as to minimize the possibility of one incident blocking both escapes. One of the means of escape for the forward section of the unauthorized 12-person sleeper accommodation module was blocked closed and led to nowhere, making it unusable.

Each generator must meet the applicable requirements for construction and testing in section 4-8-3 of the ABS Marine Vessel Rules. The recently installed portable generators used to replace the main generators on board do not have documentation or placarding showing they meet marine standards found in ABS rules, nor have they been tested to ensure compliance. Barges with sleeping accommodations for more than six persons and all self-propelled vessels shall have permanently exhibited for the guidance of the officer in charge of the vessel the following plans: (a) General arrangement plans. The fire control plan on board was not updated to show the new generators and portable accommodations. Additionally, the fire fighting systems and equipment do not account for the modified vessel.

Each vessel must have a general emergency alarm system that meets the requirements of this subpart. The general alarm system for the 12-person portable accommodation was not integrated into the vessel’s general alarm as required in CG-ENG Policy Letter 01-16. Additional cite: 46 CFR 113.25-3

After any survey of the vessel made under 42.09-5 to 42.09-40, inclusive, as applicable, has been completed and deficiencies corrected as provided in 42.09-45, no change shall be made in the vessel’s structure, equipment, arrangement, material, or scantlings as covered by such survey, without the prior specific approval of the assigning and issuing authority. Multiple changes made to vessel without approval of ABS including cables penetrating water/weathertight doors do not allow them to close and additional vessel structure added in the form of portable accommodations. Fuel hose reels have unapproved hoses installed. Must be J-1942 with J-1475 compression fittings or other equivalent standard. 46 CFR 56.60-25(b)

Forward fuel hose reel has multiple electrical installations within four feet of the containment that are properly installed and not suitable for hazardous locations. 46 CFR 111.81-3

Port forward overhead light on main deck under walkway is not permanently installed and currently using a plug in cable. 46 CFR 111.75-5

Multiple sewage pipes on main deck above and near the sewage treatment system are not properly bracketed and are currently relying on zip ties. Additionally expansion joints found cracked & no longer serviceable. 46 CFR 61.15-15

Both port side winch fuel hoses attached to the fuel tank under the winches are no longer serviceable. Replace with J-1942 hoses with J-1475 fittings at lengths no greater than 30”. Any additional lengths need to be hard piped. 46 CFR 56.60-25(b)

Port forward winch’s air receiver has broken gauge with the wrong range of PSI. Gauges upper range should be 1.2 to 2 times the relief valve setting. 46 CFR 61.15-15

Four various oil tanks on the main deck, port side are missing flame screens on the goose necks. 46 CFR 56.50-85(a)(8)

Starboard midship large winch engine has loose wires and wire ties connecting the battery leads to the starter. 46 CFR 111.60-17(b)

Watertight doors below deck have gasket materials gapped in multiple different areas allowing light and water to pass through. 46 CFR 170.270(b)

Watertight cable penetrations above the two watertight doors below deck have stuffing boxes with large amounts of removed material. The stuffing boxes are currently stuffed with lagging that is not approved. 46 CFR 171.111 (c)

Aft starboard on deck fuel tank goose neck vent has been painted over and the flame screen is no longer serviceable. 46 CFR 56.50-85(a)(8)

The fuel tank below deck has a long plastic tube sight glass that is unsupported and the sight glass valves are not self closing. 46 CFR 56.50-10(a)(6)

Groups of electrical wires on the outside of the accommodation space entrances are unsecured and hanging freely. 46 CFR 111.60-5(a)

The ship service generators have open inspection port and cable through fitting around generating unit. 46 CFR 111.01-1(a)(2)

Portable battery charger installed with zip ties and power cord for the spare batteries for the ship service generators. Make permanent installation. 46 CFR 111.15-30

Cummings engine technician was unable to test the high jacket water temperature alarm on both ship service generator’s prime movers. Ensure manufacture approved high jacket water alarms are installed and working properly and allow for inspection by Marine Inspector. 46 CFR 111.12-1(a)

The inspection shall be such as to insure that the vessel, as regards the structure . . . , and their appurtenances, piping, main and auxiliary machinery,. . . and other equipment, is in satisfactory condition and fit for the service for which it is intended. Multiple gussets on I-Beams supporting machinery on deck are wasted. Rectify deficiency prior to issuance of the COI.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material,. . . stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. Port machinery space door has a hole on the bottom. Rectify deficiency prior to issuance of the COI.

All piping, valves, and fittings shall meet the applicable requirements of Subchapter F (Marine Engineering)of this chapter. The piping for the fire main is extremely wasted throughout the topside of the vessel. Rectify deficiency prior to issuance of the COI.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. The structural fire protection below the accommodation spaces was removed for repairs, and was not replaced. Rectify deficiency prior to issuance of the COI.

Exhaust pipe installations must conform to the requirements of ABYC P–1 and part 1, section 23 of NFPA 302 (both incorporated by reference; see 46 CFR 58.03–1). The port generator exhaust has a leak on the outside of the machinery space.

If a main transverse watertight bulkhead is penetrated, the penetration must be made watertight. Multiple watertight cable penetrations through accommodation space bulkheads did not maintain watertight integrity. Rectify deficiency prior to the vessel's departure. 46 CFR 93.01-1 46 CFR 171.111(c)

Piping, including valves, pipe fittings and flanges, conveying vapors, gases or liquids whose temperature exceeds 150 degrees F., shall be suitably insulated where necessary to preclude injury to personnel. The frequency of the tests and inspections required by this section must be: For vessels, annually or as part of the biennial and mid-period inspections. Vessel representatives were unable to provide proof of hydrostatic testing. Rectify deficiency prior to issuance of the COI. 33 CFR 156.170(f)(3)

Along with service, each hose assembly used for transferring oil or hazardous material must be marked with the following: Maximum allowable working pressure; date of manufacturer; date of latest test required by 33 CFR 156.170. Vessel transfer hoses did not containing any applicable marking listed above. Rectify deficiency prior to issuance of COI. 33 CFR 154.500(f)

Each helicopter deck must: Have a wind direction indicator located in an unobstructed area readily visible to helicopter pilots approaching the deck; Be fitted around the perimeter with yellow and blue lights in alternate order, not more than 3 meters (10 ft.) apart; and be marked with: The unit's identification; A continuous line 40 centimeters (16 in.) wide on the perimeter; and Aiming circles. Vessel's helicopter pad was missing signage and lighting. Rectify deficiency prior to issuance of the COI. 46 CFR 108.241

Vessel did not have an approved SOPEP onboard. Vessel reps will submit to MSC for approval.

Several fire dampners did not open and close all the way. Third party personnel are working on proper operation.

Vessel needs to conduct drills once vessel has hired properly credentialed AB.

Vessels Type II MSD was found to have holes in the side of tank.

Vessel MSD pressure vessel safety relief valve was inoperable. Vessel has new tested safety relief valve in route.

Vessels fire control plan is inaccurate and incorrect; it shows extra firemans outfits and shows a open winch space on the deck being protected by a fixed CO2 system. Vessel shall create and provide an up to date accurate plan to USCG inspectors for approval/stamping.

CRANE. The cranes on board the vessel do not have the current certifications; require vessel to have cranes certified by a CG approved crane certifying authority prior to operation of the cranes; to the satsifaction of a USCG marine inspector.

Emergency exit markings in workshops were missing.

Each vsl must have posters or signs displayed in the vicinity of each survival craft's launching controls that illustrate the procedures for operating the launching device.

Nonmetallic flexible hose were > than 30 inches in length. 46 CFR 56.60-25(b)(2).

Switch board was not fitted with a dripshield. 46 CFR 111.30-5(b)

Bilge Placards were missing. 33 CFR 155.450

Provide proper expansion joints in the ballast & wing tanks as per 46 CFR 56.30-35 IAW ABS & MSC requirements.

Provide documentation that pressure relief valves on the vsl's P/V have been tested. P/V relief valves need to be tested annually.

Renew the Port/ Aft fire hose nozzle.

Loud metallic sound was coming from #2 Genset while attempting overspeed testing. Repair #2 Genset and provide satisfactory testing of Genset's overspeed safety device.

The SCBA is not rated for use in fire fighting by the NFPA. The SCBA & bottles are to be stenciled "RESCUE ONLY" and removed from the container containing the fire man's outfit.
