All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. All tubing and pipe materials must be suitable for handling the hydraulic fluid used and must be of such chemical and physical properties as to remain ductile at the lowest operating temperature. There was a on oil leak observed from the stern thruster. Vessel must determine the cause of the leak, and conduct permanent repairs to the satisfaction of Class / USCG. 46 CFR 90.20-1(a) 46 CFR 58.30-15(b)

Each auxiliary boiler must be available for examination at intervals specified by 46CFR61.05-10. Outboard Aux boiler SerNo CHI-79-8 shall be tagged out until safety valve SerNo SB72506 is adjusted to relieve at not more than 45psi. Submit service report detailing valve's adjusted set point or perform safety valve relief test in the presence of USCG Marine Inspector. Due prior to operation of boiler.

Each panelboard must have a circuit directory that has: circuit designation of each circuit, description of the load of each circuit; and the rating of the overcurrent protective device for each circuit. Vessel shall install panelboards legends that have ampacity rating for each circuit.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel's starboard engine has experienced a loss of power. Provide class or tech report of repair prior to departure from next port.

The certificate of inspection (COI) issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. Vessel master reported consent based departure of 2 A/E. Replacement must report within 14 days. 46 CFR 15.501(a) 16c

Each boiler including superheater, reheater, economizer, auxiliary boiler, low-pressure heating boiler and unfired steam boiler must be available for examination. Both vessel auxiliary boilers are opened/tagged out of service and not ready for light off or testing. Perform approved PSTP procedures and relief device tests and break-glass ventilation for both boilers, prior to operation, with local OCMI.

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. In port side cargo tunnel, across cargo hold #5 at gate #52 there is a weeping weld on the ballast tank. Make repairs by next vessel seasonal layup period.

Test and inspections of the survival and lifesaving equipment shall be carried out during each inspection. Vessel needs to service and mainttain all immersion suites as per designated manufacturers manuals, in absence of those NAVIC 01-08 may be refered to as a guide and good marine practice.

All piping, valves, and fittings shall meet the applicable requirements of Subchapter F (Marine Engineering) of this chapter. A fire main expansion joint on port side is leaking. Make permanent repairs to fire main exapansion joint.

Cargo vessels must meet the requirements in subpart D of 46 CFR 199. Per Coast Guard letter dated March 24, 2025, the vessel was approved an alternative lifesaving arrangement with the caveat that an approved rescue boat and davit under 46 CFR 160.156 also be installed in place of the open lifeboat. Plans for installation of a Coast Guard approved rescue boat shall be submitted to the OCMI within 30 days, and installation shall be completed prior to completion of winter layup 2026.

Where fire extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the applicable requirements. A new fire detection system was installed to replace the existing broken system. Upon commissioning the new system, minor errors were noted with the fire arrangement. Make the necessary revisions to the fire arrangements and fire control plan.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. New and existing multi-cable penetrations installed in the accommodation spaces do not maintain the applicable fire boundaries. Conduct necessary repairs to the multi-cable penetrations throughout accommodation spaces.

Vessels is currently operating on an Extended Load line certificate that expires 19DEC24. Vessel has been issued a Single voyage load line authorization and has been cleared to transit from Indiana Harbor, IN to Sturgeon Bay, WI for dry dock. Vessel is not authorized to carry any cargo during this transit and is not to carry any cargo until LL survey/DD is complete. Any deviation of the plan is to be communicated to the nearest OCMI.

An industrial system is accepted if the inspector finds there are guards, shields, insulation or similar devices for protection for personnel. 40' air hose on cargo boom slewing system has wasted. Renew with approved hose.

46 CFR 15.501(a) Certificate of inspection(COI) issued by the Officer In Charge Marine Inspection (OCMI, to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew neccesary for the safe operation of the vessel. Vessel operating with a crew shortage of 01 AB. OCMI requires documentation that proves company is actively seeking a suitable replacement.

Vessel reported loss of bow thruster function, it is believed there is something lodged in the thruster tunnel preventing its full utilization. The vessel is was fully loaded at the time of the report and the water clarity is such that divers are unlikely to be able to address the issue. The vessel has plans to evaluate the situation when they arrive back in Burns Harbor, IN. Vessel is authorized to transit from Duluth, MN to Burns Harbor, IN with assist-tug required in all restricted waters (to include: departure from Duluth, MN, St. Mary's River system, and approaches to Burns Harbor, IN; and anywhere else that the Captain deems necessary). Vessel Captain to provide technician's report to MSU Duluth upon satisfactory bow thruster repairs/test (to be completed prior to departing from Burns Harbor, IN.

The Master made notification to [name withheld], stating that the vessel's AB watchman was terminated by the company on 12OCT2024. The vessel master requested a temporary reduction in manning until the company is able to find a suitable replacement. The vessel is authorized to sail with one less AB for 07 days until 25 October 2024. Three person watch system and work/rest hours must be maintained in accordance with applicable laws and vacated station billet positions shall be evaluated and filled to the satisfaction of the Master. The vessel/company shall make reasonable efforts at each port to find a replacement. Prior to the expiration of the deficiency, the vessel/company is required to provide proof of adequate effort to find crew replacements.

Except as provided by § 15.725 of this part, no vessel may be navigated unless it has in its service and onboard the crew compl ement required by the COI. Return to full crew complement as required by COI prior to departure from Sturgeon Bay.

Inspections shall be such as to insure that the vessel as regards to main and auxiliary machinery, electrical installations, lifesaving, fire detection and extinguishing equipment, pilot boarding, pollution prevention equipment and other equipment is in satisfactory condition and fit for service for which it is intended. Multiple pieces of machinery associated with cargo unloading system were found poorly maintained. - Demonstrate proper operation of all machinery associated with cargo handling equipment prior to cargo operations to the satisfaction of Class.

Inspections shall be such as to insure that the vessel as regards to main and auxiliary machinery, electrical installations, lifesaving, fire detection and extinguishing equipment, pilot boarding, pollution prevention equipment and other equipment is in satisfactory condition and fit for service for which it is intended. Multiple pieces of machinery associated with cargo unloading system were found poorly maintained. - Provide plans to MSU Sturgeon Bay for permanent repairs to cargo boom slewing machinery within two weeks.

Inspections shall be such as to insure that the vessel as regards to main and auxiliary machinery, electrical installations, lifesaving, fire detection and extinguishing equipment, pilot boarding, pollution prevention equipment and other equipment is in satisfactory condition and fit for service for which it is intended. Multiple pieces of machinery associated with cargo unloading system were found poorly maintained. - Permament repairs to cargo boom slewing machinery must be completed prior to December 20, 2024 to align with vessel drydock.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. Vessel reported unintentionally touching bottom on the port side while loading at BNSF #5. Prior to departing Duluth/Superior, make associate ballast tanks available for inspection by Coast Guard Marine inspectors.

In general, compliance with the structural standards established by the American Bureau of Shipping will be considered as satisfactory evidence of the structural efficiency of the vessel. Vessel is aground after attempting to depart BNSF #5. Utilizing the vessel's non-tank vessel response plan, conduct assessment of structural stability and submit salvage plan from salvage provider for Coast Guard review and approval. After CG approval and execution of salvage plan, conduct hull and bottom survey to identify any damage to the vessel.

Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection, and shall include demonstration of 1)Proper condition and operation of the survival craft and rescue boat launching appliances at loads ranging from light load to full load. 6)The ability of crew to effectively carry out abandon-ship procedures. Lifeboat and rescue boat release gear must be operationally tested under a load of 1.1 times the total mass of the lifeboat when loaded with its full complement of persons and equipment whenever overhauled or at least once every 5 years. Lifeboats needs to be launched at light load, full load, and 1.1 times the total of full complement with lifeboat. A man overboard drill to accompany this testing.

Both auxiliary boilers have been found to have bolting materials that are not in line with the requirements in 46 CFR part 56. Bolts or all thread stock are to be of A193 grade B7 as per ASME B18.2.1, they shall bare the mark of B7 and be of the heavy hex head type. Nuts are to be of A194 grade 2H as per ASME B18.2.2, they shall bare the mark of 2H and be of the heavy hex type. ALL BOLTING materials on each boiler or piping attachment shall be as prescribed and not mixed and matched as found on board. Also, bolts or all thread stock shall extend at least all the way through nuts. Some approved materials were found but the bolts or all thread stock in some cases were found only halfway through the nuts. The boilers are to be immediately taken offline and shutdown so these repairs can be made.

Each vessel must carry a life jacket for each person on-board in accordance with the Certificate Of Inspection. 07 Life jackets were found to be in unsafe condition and required to be cleaned and or replaced.

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. 03 lifejackets were found missing lights.

Each lifebouys must be marked in block letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel under 67.123 of part 67 of this chapter. Multiple lifebouys were found excessively dirty and required marking excessively faded. Clean and remark or replace required lifebouys to the satisfaction of the attending marine inspector.

During preparation, launching, and recovery, each survival craft and rescue boat, its launching appliance, and the area of water into which it is to be launched or recovered must be adequately illuminated by lighting supplied from the emergency power source. Starboard side #3 liferaft emergency lighting was found inoperable.

Liferafts shall be sufficiently ready for use so that 2 crew members can complete preparations for launching in less then 5 minutes. Port side 25 man was found in the wrong location with the FWD 16 man liferaft. Store all liferaft in their proper locations.

In the inspection of hulls, boilers, and machinery of vessels, the standards established by the American Bureau of Shipping, see part 90, subpart 90.35 of this chapter, respecting material and inspection of hulls, boilers, and machinery, and the certificate of classification referring thereto. All Class certificates on board are expired at this time. All Class certifictes are to renewed and or extended prior to departure.

Vessels that operate in fresh water at least six months in every 12 month period since the last drydock examination must be examined in accordance with the intervals set forth in Table 91.40-3(b). Vessel is past due on Internal Structural Examination (ISE) due date; make vessel ready for ISE within 30 days.

Inboard and outboard auxiliary boilers failed hydro tests (due to pressure loss) as completed on 16NOV23. Cause of failure was deemed to be valve leak-by versus pressure vessel or tube issues. Locate and rectify source of pressure losses and subsequently demonstrate satisfactory hydro tests for each boiler.

Crown plate of outboard auxiliary boiler exhibits numerous welded crack repairs. Crop and renew crown plate as originally constructed. Repairs to be completed prior to end of 2024 winter-work / fit-out period.

Discovered an approximately 3/4" gap between the refractory casing (upper section) and the inner combustion chamber wall on the inboard boiler. Re-caulk gap with appropriate material as approved by manufacturer (Johnston Boiler). Repair must be comleted to the satisfaction of the USCG before returning boiler to normal service.

Master made notification to [name withheld], of crew shortage due to consent based discharge of AB [name withheld], MMC [withheld], who departed the vessel on 7/12/2023. Vessel is authorized to operate with one less AB until 7/26/2023. The vessel/company is required to continue to make reasonable efforts to attain a replacement at each port.

Each lifeboat must be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months during an abandon-ship drill. The lifeboat is unable to be launched and operated due to the vessel being moored starboard side where the lifeboat is located, deploy and operate lifeboat to the satisfaction of attending Marine Inspector.

Cables should be adequately protected where exposed to damage. Multiple cables should be supported in metal hangers or trays. Multiple unsupported cables/wires were found in the gyro room under the bridge. Additionally, multiple splices and dead end cables/wires were discovered in the same location.

Where fire extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the requirements of this part. A fire detection system has been installed onboard but installation has not been verified.

Steam lines and registers in non-accommodation and non-public spaces must be suitably located and/or shielded to minimize hazards to any personnel within the space. Steam line near boilers was missing shielding. Replace shielding.

The general structural strength of the hull must be sufficient for the draught corresponding to the freeboard assigned and must be approved by the Commandant. Water seeping out of weld at bottom of ballast tank/deck at gate #37 welds are corroded for approximately 40 plus feet and will need crop and renew at next winter layup.

Water leaking from unauthorized weld repair between gates 56 and 57. Weld had approx. 8-10 weld beads going down the side of the ballast tank instead of cropping and renewing plate. The respective piping systems installed on ships shall have the necessary valves for safe and efficient operation of the vessel. The globe valve was found leaking internally and out of the suction strainer in gate #2 stbd side. Repair valve at next winter layup to prevent the vessel’s watertight integrity from being compromised.

The valve at aft starboard tunnel next to bulkhead in the overhead/centerline of ship leaking water from packing and was missing bolt on the port side flange at the top. Repair valve to prevent the vessel’s watertight integrity from being compromised.

At each time necessary, the inspector will determine that all fire extinguishing equipment is in suitable condition and may require any tests necessary to determine the condition of the equipment. Two firemain water leaks were found at the pipe connection at the packing glands on the main deck, port side. Make repairs to leaks to allow system to be fully functional.

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010 lifebuoy lights on the aft main deck are not capable of being sealed and the attachment line is not 3’ to 6’ in length.

A vessel at anchor shall exhibit where it can best be seen an all-round white. The anchor light is currently inoperable.

Means shall be provided for stopping all fans in ventilation systems serving machinery and cargo spaces and for closing all doorways, ventilators and annular spaces around funnels and other openings to such spaces, from outside these spaces, in case of fire. The vent closures on the booth stacks IVO bridge have missing nuts and bolts which are damaged and prohibit proper closure of the vents.

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. EAP program material and hotline information was not posted.

Each pump must be capable of delivering water simultaneously from the two highest outlets at a Pitot tube pressure of approximately 50 p.s.i. A substantial leak was discovered on the fire main expansion joint located amidship.

Vital systems that are automatically or remotely controlled must be provided with instrumentation to monitor system parameters necessary for the safe and effective operation of the system. The following gauges on the vessel were inoperable: air intake manometers, STBD clutch air gauge, STBD outboard crank case pressure tube disconnected. Replace or repair gauges.

Electrical installations on vessels must ensure protection of the passengers, crew, other persons, and the vessel from electrical hazards. STBD aft general alarm globe was broken. Replace globe.

All emergency lights shall be marked with a letter “E” at least 1/2 inch high. One emergency light located in the belt tunnel portside was not labeled as required.

The material, design, construction, of the main propulsion machinery must be at least equivalent to the standards established by ABS Steel Vessel Rules. Copper line to airline box on STBD outboard MDE was kinked. Replace copper line.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. The following issues were discovered with the vessel’s sea strainers: foot strainer at gate #73 stbd side is clogged and requires cleaning, stainer at aft end of the port side tunnel leaking water, suction strainer at gate #2 stbd side is clogged. Make necessary repairs to said strainers.

Each fixture globe, lens or diffuser must have a high strength guard. The following lights were missing covers: bulb at gate #2 stbd side, bulb at gate #28 port side. Install high strength guards for lights.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Found light flickering at gate #62 stbd side. Make necessary repair to ensure light is fully functional.

Manholes must have permanently attached covers, unless the cover is secured by closely spaced bolts around its entire perimeter. Manhole in stbd side tunnel has missing bolt near gate #56/57.

Make necessary repairs to prevent the vessel’s watertight integrity to be compromised.

Electrical installation on a vessel must ensure protection of the passengers, crew, other persons, and the vessel from electrical hazards. Dead end wiring found in the following location: IWO the OWS in the machinery space and the pilot house equipment room. Remove all unused wiring.

Renew or repair and install the starboard shaft generator and test the starboard shaft generator to the satisfaction of Coast Guard Marine Inspector and Class Surveyor.

Port inboard main engine experianced a fire in the air box. Vessel may proceed to Duluth/Superior to further troubleshoot and effect perminent repairs.

Boarding Ladder on the starboard side found with both hand rails crushed and welds fractured. Repair and straighten.

Vessel unable to launch lifeboat during annual fitout inspection due to hazardous ice conditions. Conduct MOB recovery drill and demonstrate launch, maneuvering, and recovery of the lifeboat. Drill to be witnessed by USCG Marine Inspector; or, provide date-stamped video of drill showing all aspects of evolution, to include operation of davit system components and on-water operations. Remove mold, rust scale and correct loose plumbing and low shower pressure in crew staterooms. Separate list provided to vessel rep.

Provide secondary means of determining the water level in the auxiliary heating boilers. Replace missing gauge cock valves on the boiler shells.

Vessel reported failure of the emergency generator switchboard automatic transfer switch/breaker. Emergency generator is still fully functional and will still take all emergency loads; however, the load transfer must be switched manually since the automatic switch/breaker is not functioning. Restore emergency generator switchboard to normal operating condition. Until the breaker is repaired/replaced the on-duty engineering watch-stander must be on stand-by for immediate recall for the purpose of manually transferring to emergency power if necessary (within 45 seconds). Communications between the bridge, engine control room, and the engineering watch-stander must be maintained at all times. Upon completion of repair; provide service technician's report, and [name withheld] statement, certifying satisfactory repair and post repair testing, to USCG MSU Duluth.

Firetube boilers fitted with fusible plugs must use fusible plugs stamped with the manufacturer's name on the the casing and "ASME Std." stamped on the water end of the fusible plug. The fusible plugs installed are not approved for installation in the boilers. Replace the existing fusible plugs with approved fusible plugs.

When alterations or repairs of boilers, pressure vessels, machinery, safety valves or piping systems become necessary, the work shall be done under the cognizance of the [name withheld] repair to affected engine and provide technical report to the satisfaction of CG prior to operating engine.

Two separate and independent systems for full followup control must be provided in the pilothouse. The port rudder is misaligned when operating on #1 steering pump in follow up mode. Repair steering system to the satisfaction of class and the cognizant Officer in Charge, Marine Inspections.

Before the vessel leaves and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. While launching the life boat, the aft gripe failed to release, preventing the lifeboat from being launched.

Vessels two most inboard palm bolts on the port and starboard rudders show excessive wear. Replace bolts and make available for inspection by CG Marine Inspector prior to departure from Winter Layup.

Outboard and Inboard Auxiliary Boilers hydrostatic test is overdue. Boilers must be made available for examination by a CG Marine Inspector prior to use of the boilers. Submit request for CG attendance prior to departing next port.

Vsl is broadcasting: Invalid MMSI and invalid Call Sign.

Vessel reported loss of one MDE while underway in the vicinity of the Apostle Islands, Lake Superior. Vessel has three functional MDEs. Vessel shall make permanent repairs to the damaged MDE to the satisfaction of the attending CG Marine Inspector prior to departing the next port of call.

Determine source and effect repairs to hydraulic fluid leaks on port and stbd steering system pumps.

Prove proper operation of the starboard inboard steering gear motor overload C phase as required by 46 CFR 111.70

Conduct weight test

Conduct MOB at first available port of call

Verify all accommodation ventilation dampers are properly operating

Properly install (Rox Blocks) in all bulkhead locations that are missing IAW manufacturers instructions.

Make Boiler Fireside tubes and sheets clean and open for inspection as required by 46 CFR 61.05-5(a)
