NATIONAL GEOGRAPHIC SEA LION

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with this part. Newly installed bilge/fire pump was not listed on the recently approved one line electrical diagram. It was located in panel 17, slot 14 which is listed as "flow meter". Additionally, the fire pump located in the engine room was not listed on the one line diagram. Update electrical one line diagram and submit to MSC for approval. 46 CFR 120.115

SOLAS 1974 II-2 Regulation 21 Currently, All the crew berthing areas (below the bulkhead deck) have two means of escape. However, the second means of escape are in crewmember berthing rooms above the top bunks. Furthermore, the two means of escape for each berthing area lead to a common space (i.e. Lounge/lounges) above the crew berthing areas. Using the forward crew berth as an example: If a fire were to break out in the forward bar (located in the forward lounge) the crew below would have no escape route. The same scenario for the large aft crew berths under the Galley and main passenger lounge. * Submit plans to Coast Guard Marine Safety Center to correct the deficient means of escape from all three crew berths prior to sailing foreign. Until the means of escape meet SOLAS 1974 II-2 Regulations 21, the vessel is restricted to domestic voyages only.

Taking into consideration the means of escape requirements of SOLAS 1974 II-2 Reg 21), SASH requirements found in 46 USC 3106 (Key Control). On October 19, 2024, and ISM Audit was completed with a Nonconformity issued for SASH policies and Key Control implementation. The Corrective actions were implemented on the vessel; however, the implementation of the key control measures directly conflicts with the two means of escape requirements in SOLAS. Prior to embarking passengers on a foreign voyage, you are required to remove the locks on all three berthing area doors that lead to the second means of escape from the three crew berths below the bulkhead deck. To conform with the SASH requirements, you are required to reassign the crew members from the rooms with the escape hatches to another appropriate berth.

ISM CODE 1.2.2 Safety management objectives of the Company should, inter alia: Provide for safe practices in ship operation and a safe working environment, assess all identified risks to its ships, personnel and the environment and establish appropriate safeguards; and continuously improve safety management skills of personnel ashore and aboard ships, including preparing for emergencies related both to safety and environmental protection. Currently the means of escape routes on the vessel do not meet SOLAS 1974 II-2 Regulation 21 and present a hazard to the crew. If a fire or other emergency were to compromise the lounge areas above the crew quarters, crew members would find themselves without a safe and independent alternative route for egress. This configuration does not provide segregated escape paths for the crew, thereby placing them at increased risk in emergency situations. * Conduct a Risk Assessment covering the emergency response procedures for a fire outbreak in the passenger lounges above the crew quarters. Consider the impact on crew escape routes, and the resultant effects on vessel operations, including firefighting and abandonment of the vessel and its passengers - with special consideration for conducting these operations with a reduced crew.

Ships shall be provided with an efficient bilge pumping plant capable of pumping from and draining any watertight compartment under all practicable conditions after a casualty whether the ship is upright or listed. Ships shall have at least three power pumps connected to the bilge main piping system. Where the criterion numeral is 30 or more, one additional independent power pump shall be provided. Plan review by MSC determined NATIONAL GEOGRAPHIC SEA LION has a criterion numeral greater than 30 requiring one additional independent power pump be provided. Provide plans to MSC for approval for one additional independent power pump. Provide one additional independent power pump on vessel. Vessel restricted from international voyage until plans approved and one additional pump installed and tested in accordance with approved plans. SOLAS (1974) II-1 Reg. 18(b)(i)

46 CFR 115.830 (a) At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Currently there are multiple flammable, explosive and hazardous items being stored in the engine room and not in an approved flammable materials locker. * You are required to install a flammable material locker for the storage of these materials.

46 CFR 115.804 (c) Inspection of all machinery essential to the routine operations of the vessel including generators and cooling systems. (d) External inspection of fuel tanks and inspection of tanks vents, piping and pipe fittings. There was a small fuel leak dripping from the supply hose going to the emergency generator. * Make proper repairs to fix the fuel leak.

The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and/or operational nature of the deficiencies noted above provide objective evidence of a serious failure of the implementation of the ISM Code. A safety management audit satisfactory to the OCMI must be submitted prior to the clearance of this deficiency. SOLAS IX, ISM Code A/10

The emergency generator was found to have clear/plastic fuel hose on the fuel return line and a grease fitting with single hose clamp terminating at the fuel tank. Provide emergency generator fuel return with CG approved fuel hose, proper termination fittings, and double hose clamps. 46 CFR 176.25-10(5)

While testing newly installed bilge/fire pumps #2 and #3, both motor controller overload tripping mechanisms were tripping the circuit and securing the bilge/fire pump. Provide safe electrical overload protection. 46 CFR 177.10-5; 46 111.70-1

Newly installed bilge/fire pumps #2 and #3 were found with unprotected motor to pump couplings. Provide couplings with machinery guard to prevent personnel injury. 46 CFR 182.10-5

Structural fire protection in the engine room overhead (stbd/forward) was found to missing. Restore structural fire protection to A-60 standard. 46 CFR 177.10-5

The aft generator fuel supply piping was found to be missing a support bracket, allowing pipe to move freely and causing a vibration hazard. Repair fuel supply piping bracket. 46 CFR 176.25-10

The engine room to lazarette watertight door indicator switch was found to be inoperable. Make repairs and prove proper operation. 74 SOLAS Reg13, (f) (v)

Fire doors must be capable of closing against an inclination opposing closure. Vessel was not able to demonstrate effective closure of fire doors. 74 SOLAS II-2, Reg 23

Each fire pump motor controller shall have a low-voltage release installed. Prove newly installed fire pump motor controller are equipped with devices.

Protective gratings shall be provided at such locations where persons or gear might come in contact with exhaust pipe. Emergency generator exhaust piping was found to be bare piping and uninsulated. Provide lagging or protective gratings to protect crew and passengers from hot surface. Old T – 46 CFR 182.15-20(g)

At each subsequent inspection the MI shall examine electrical cables to determine they are in a safe condition. Dead-ended cables were found on a junction box centerline in lazarette and in the port side fantail gear locker. Remove dead-ended cables and seal penetrations. Old T – 46 CFR 176.25-15

Receptacle outlets and attached plugs operating at 100 volts or more must have a grounding pole and conductor in the cable. The galley mixer electrical cord was found without a ground terminal on the plug. Repair or replace mixer plug with a three-prong device.

Each lighting fixture globe must have a guard. The lighting fixture inside the emergency generator compartment was found to be unmounted, with an open socket, and no globe or guard. Repair light fixture to satisfaction of attending marine inspector.

At each subsequent inspection the MI shall examine electrical devices to determine they are in a safe condition. The electrical outlet mounted on the day tank outboard the starboard main engine was found to be lose and improperly installed. Repair or replace electrical outlet. Old T – 46 CFR 176.25-15

Each watertight hatch must be attached to the frame by hinges, captive chains, or other devices to prevent its loss. Lazarette escape hatch was found without a retaining device/chain. Prove hatch with retaining device.

In accordance with vessel COI each Zodiac shall have a handheld submersible DSC capable radio. Zodiac's did not have DSC capable feature operational. Make each handheld radio for Zodiac’s DSC capable using each Zodiac name prior to taking passengers.

Emergency lighting modifications made in accordance with MSC letter E2-2500735. Emergency lighting in forward lounge, aft lounge, galley, and forward / lower stateroom passageway was found to be inoperable. Prove proper operation of emergency lighting prior to vessel departing San Diego. 46 CFR 120.432

Bilge and Firemain system modifications made in accordance with MSC letter E1-2500482. Vessel’s bilge piping system is not yet complete. Prove proper operation of new bilge pumps and associated piping. Vessel restricted from international voyage. 46 CFR 119.510

Emergency lighting modifications made in accordance with MSC letter E2-2500735. 50 amp feed breaker was installed for the new inverter panel and the approved plan states 60 amp. Make installation match approved plans or resubmit revised plans to MSC. 46 CFR 120.432

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Port and STBD shut off valves are not functioning as designed. Make valves operator as designed. 46 CFR 119.455(b)(3)

A self-propelled vessel must meet the applicable requirements for main and auxiliary steering apparatus in subchapters F (Marine Engineering) and J (Electrical Engineering) of this chapter. Aft steering/local rudder indicator off more than 5 degrees. Test system to determine which indicator is correct and ensure both indicators indicators are reading the same. 46 CFR 176.25-50

At each inspection for certification the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Observed over 1’ of hydraulic oil in FWD ER bilge. Remove oil from bilge and clean bilge. 46 CFR 115.830(b)

At each inspection for certification all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Forward generator PTO hydraulic PMP hoses were chaffed and top layer ripped. Provide new hoses and tag out system until new hoses are provided. 46 CFR 115.830(a)

Batteries must be mounted in trays lined with, or constructed of, a material that is resistant to damage by the electrolyte and secured to protect against shifting with the roll and pitch of the vessel. Inverter batteries not secured and not in box. Provide box and secure inverter batteries. 46 CFR 120.350(b) and (e)

The owner or managing operator shall prepare an evacuation plan that must: (a) Identify possible casualties involving fires or flooding, including a fire in the largest capacity passenger space in each main vertical zone; (b) Provide procedures for evacuating all affected spaces for each casualty identified as required by paragraph (a) of this section without abandoning the vessel, including— (1) Identify readily accessible areas of refuge for the maximum number of persons allowed aboard the vessel. The capacity for an area of refuge may not exceed the number of persons specified in § 116.438(n)(2) of this part, except that one person may be permitted for each 0.28 square meters (3 square feet) of deck area; and (2) Identify at least two means of escape complying with § 114.400 from the space being evacuated; and (c) Include procedures to evacuate passengers from the vessel using an abandon ship plan, considering the number of passengers and the vessel’s route. The abandon ship plan must identify at least one escape route from each area of refuge to each embarkation station required by § 116.510 of this part. -Provide emergency evacuation plan and identify areas of refuge for all persons on board with 3 sq ft deck space (no furnishings) per Pearson. Evacuation plan must be updated to meet regulation. 46 CFR 116.520

Approved modifications are in progress to the vessel’s bilge system, discrepancies identified between installation and plans are: pumps 2 and 3 share a strainer, overboard check valve and cut-off valve not accurate, cross-connect valve to raw water system incorrectly installed. Make installation match approved plans or resubmit revised plans to MSC. 46 CFR 119.510

Each distribution panel or switchboard must be totally enclosed. Electrical panels (#8 and 15) have openings. Install non-conductive cover/blanks in open spaces. 46 CFR 120.330

At each inspection for certification the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. AC raw water piping has 4 temporary pipe repairs. Tag out system and make permanent repairs. 46 CFR 176.25-50

On a vessel of more than 19.8 meters operating on exposed or partially protected waters, a rescue boat approved under approval series 160.156 is acceptable in meeting the intent of this section. Rescue boat was not found in a list of approved rescue boats. Provide approved rescue boat. 46 CFR 117.210(c)

Master key control system. Vessel needs to meet master key program in accordance with 46 USC 3106. Vessel had some documentation of a master key program in the SMS however, it was not implemented or in practice yet. 46 USC 3106

Requirement to report sexual offenses. Ensure all applicable safer seas act requirements are implemented in the SMS and annual training is completed. 46 USC 10104

In accordance with the COI, the vessel can carry a maximum of 10 - 6-gallon plastic gasoline containers. 14 were found on the lido deck. Remove 04 6-gallon plastic gasoline containers. Conditions of COI

Life jacket placards must be posted in each passenger cabin or stateroom on a vessel of more than 65 feet in length with overnight accommodations for more than 49 passengers. Vessel did not have an instruction placard for the type 1 lifejackets that were in the staterooms. Either change the lifejackets or updates the instructions. Provide instructions and post placard for every type of lifejacket in all staterooms. 46 CFR 122.516

A ship must have a "Discharge of Oil Prohibited" placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space. Placard for “Discharge of Oil Prohibited…” required in OWS space. Also provide line diagram for OWS system. 33 CFR 155.450

Tubular gauge glasses, if fitted to diesel fuel tanks must be of heat-resistant materials, adequately and protected from mechanical damage. Gauging tubes at port F/O were not of heat-resistant materials. Provide heat-resistant gauge tubes. 46 CFR 119.440(a)(7)

Each ring life buoy on a vessel must be approved in accordance with 46 CFR 160.050. Ring buoy top deck portside line frayed, repair or replace. 46 CFR 117.70

Each vessel contracted for before March 11, 1996 may be constructed in accordance with the regulations in effect at the time. However, any alterations or repairs must be done in accordance with § 170.005. Vessel completed dry-dock and made several modifications to the hull. Submit weight added/removed to MSC for review and approval. 46 CFR 170.001(b)/MTN 04-95, CH-2

Unless otherwise provided by this subpart, each vessel required to have a Certificate of Inspection is subject to the annual vessel inspection fee listed in table 2.10-101 for its vessel category. Vessel has five inspected inflatable rafts used to transfer passengers during excursions. COI inspections were recently completed without proof of annual inspection fees paid. Provide proof of annual user fees for all five inflatable rafts. 46 CFR 2.10-101

In a passenger ship the required fire pumps shall be capable of delivering for fire-fighting purposes a quantity of water, at the appropriate pressure prescribed below, not less than two-thirds of the quantity required to be dealt with by the bilge pumps when employed for bilge pumping Relief valves shall be provided in conjunction with all fire pumps if the pumps are capable of developing a pressure exceeding the design pressure of the water service pipes, hydrants or hoses. Could not find pressure of system in plans or during testing to verify pressures or to know if relief valves are necessary. Provide calculations to MSC and update plan to show pressure and if relief valves are needed. SOLAS 74 Ch. II-2 Reg 5(a)(i) and 5(b)(iii).

A-Class decks must be made intact with the main structure of the vessel. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. New inverter power cable penetrate the main deck to lazarette with air gap showing. Provide A-class boundary where inverter cable penetrates deck. 46 CFR 116.415

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge. With changes to emergency evacuation plan, Area of refuge shall be updated on the fire control plan. 46 CFR 116.530

There shall be above the bulkhead deck and outside the machinery casings a self-contained emergency source of electrical power. Emergency generator is not operational. Provide emergency source of power that complies with regulation SOLAS 74 Ch. II-1 Reg 25. Repair prior to sailing foreign.

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] have been made to the electrical system. New motors for bilge/firefighting installed. Provide updated electrical plans to the MSC for approval. 46 CFR 115.700(b)

The owner shall, prior to receiving a Certificate of Inspection, submit for approval to the Marine Safety Center, three copies of the following plans: Ventilation diagrams engine exhaust diagram. Vessel did not have ventilation diagram. Provide diagram for vessel and submit to MSC for review. 46 CFR 116.202(b)

Stiffeners in the sewage tank were found corroded and pulled away from the deck. Submit repair proposal to repair structural integrity.

Area identified on starboard bow in crew berthing area, needle gun to good material and re gauge.

Remove deck plates in engine room and ensure bilges are clean, dry and ready for inspection.

Area with a symmetrical crack identified in a 2x3 foot section on port bow that is inaccessible internally. Remove paint to uncover steel and evaluate condition.

Three valves were found wasted and require replacement, ensure proper replacement in kind on #'s 20, 25, and 41. Inspect all others prior to re installation. Make available for inspection after cleaning and renewing #31 & 33 or replace in kind.

When tanks and void spaces not otherwise inspected are made available, coordinate inspection by USCG and ensure spaces are cleaned, dried and gas freed by a marine chemist in previous 24 hours.

Once a repair proposal is approved, carry out crop and renews in accordance with the repair proposal and ensure fit up, back gouge and final weld NDT is witnessed by OCMI unless otherwise discussed.

Gauge area in A/C room from cropped area up to waterline, forward to bulkhead and aft to the previously installed new insert. Submit repair proposal with gauge report.

Holes found in the A1 tank, needlegun area to determine extent of damage and submit repair proposal.

Pit / hole found in starboard day tank, submit repair proposal.

Questionable areas were found along the side shell. adjacent to the day tank. Conduct gauging in exposed area and forward and aft until good metal is found. Submit repair proposal as necessary. Vessel plans to complete at next drydock.

Test substance near day tank for asbestos and take appropriate measures during repairs.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel suffered failure to the port main diesel engine turbo. Upon further inspection, coolant was identified in the exhaust manifold. Technician onboard is continuing to diagnose further damage to port main diesel engine. Make repairs to the port MDE and complete satisfactory power trials to the satisfaction of attending Coast Guard Marine Inspector prior to the carriage of passengers.

Vessels described by 46 CFR 114.110(f) must submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. 46 CFR 122.410(b)(2)

Each steering-gearing control system must receive its power from the feeder circuit supplying power to its steering-gear power unit. The port and starboard wing controls are inoperable due to a failed circuit board. Make permanent repairs to the satisfaction of the marine inspector prior to carrying passengers.

An audible alarm must activate in the pilothouse upon failure of the electric power to the control system of any steering gear or failure of that power to the power unit of any steering gear. The primary control unit failed to make an audible alarm. Unit was replaced and proved operational.

46CFR58.25-10(b)(2)Main/Aux steering gear shall be capable moving the rudder from 35 degrees on either side to 35 degrees at its deepest loaded line draft. #2 steering pump failed to energize. Multiple hours later MI witnessed sat test of #2 steering pump. Root cause was bad fuses. 46CFR119.600 46CFR58.25

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its COI. MI found multiple plexiglass manhole covers throughout the vessel. 46CFR116.100(a)

Repairs and Alterations: Newly installed steering system is not approved by the Coast Guard. Owners have 07 days to address comments from MSC letter dated 05 Jan 2023. Provide proof of correspondence with MSC. Owners have 07 days to submit QFA/DVTPs to MSC for approval as per CG-CVC policy letters 17-07 and 17-08. Owners have 30 days to test approved DVTPs and to incorporate in preventive maintenance system testing. 46CFR115.700(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its COI. MI found multiple potable water leaks throughout the engine room.46CFR116.100(a)

Each vessel must be provided with a sound powered telephone system, complying with subpart 113.30 of this chapter, to communicate between the pilothouse and steering compartment. Sound powered phone from pilothouse to steering compartment is not functioning as designed.46CFR119.600 46CFR58.25-15

Weekly maintenance and inspections: Each rescue boat must be visually inspected, engine must be run ahead and stern for not less than 3 min, and each battery for rescue boat engine starting must be brought up to full charge at least once each week. MI noted that rescue boat weekly maintenance/inspections were missed on the following dates - Sept 03, 17, and 24. As per the vessels Safety Managements System, create a non-conformity and corrective action plan. NC/CAP shall be submitted for review to OCMI.46CFR122.720

Crew Training: MI noted that 05 crew members crew familiarization was expired. As per the vessels Safety Managements System, create a non-conformity and corrective action plan. NC/CAR shall be submitted for review to OCMI.46CFR122.420

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Emergency escape hatch leading from the lazarette compartment to aft main deck, could not be easily opened by one person in case of an emergency requiring egress from the space. Provide temporary solution by 11AUG2023. 46CFR177.500(h)

When within 12 nautical miles of the nearest land, any discharge of oil or oily mixtures into the sea from a ship other than an oil tanker... The oily-water separating equipment is equipped with a 15 ppm bilge alarm; for U.S. inspected ships, approved under 46 CFR 162.050. On-board oily-water separating equipment (USCG App No: 162.050/9017/0), could not display all dates/times of use. Provide evidence of proper use of equipment. 33CFR151.10(b)(5)

Except as otherwise provided in this section, all vessels shall be provided with not less than two avenues of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed, so located that if one is not available the other may be. At least one of the avenues of escape shall be independent of watertight doors. Crew berthing's secondary means of escape currently has plywood and/or couch cushions sitting on top of escape hatches, obstructing escape. Illustrate an effection means of unobstructed escape.

SOLAS 14 II-2/10.10.4: A minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter's communication shall be carried on board. Ships constructed before 1 July 2014 shall comply with the requirements of this paragraph not later than the first survey after 1 July 2018. Fire teams had no radios meeting the requirements of above during fire drill.

46 CFR 116.435(c)(6)(i) Doors in A-Class fire control boundaries must meet the following additional requirements. A door in a stairtower, stairway, and main vertical zone bulkhead must meet the following additional requirements. A door must be of the self-closing type capable of closing against a 3.5 list of the vessel. 06 fire screen doors would not close properly. Bridge indicating panel was malfunctioning.

PO1: All observed unsafe practices, fire hazards, and other hazardous situations must be corrected. Vessel sustained damage to the port side upper deck knife edge. Submit repair proposal to ensure protection of the passengers and crew in vicinity of damaged knife edge, to satisfaction of Officer in Charge, Marine Inspection.

Where bulwarks on the weather portions of freeboard or superstructure decks form wells, ample provision shall be made for rapidly freeing the deck of water and for draining them. Vessel sustained damage to port side scupper. Submit repair proposal and conduct repairs to satisfaction of Officer in Charge, Marine Inspection, and class.

MI to verify ship's repairs to starboard side fuel transfer line and fire main line. For credit drydock.

THE MEANS PROVIDED FOR PUMPING BILGES SHALL BE TESTED BY OPERATION. ALL SUCTION STRAINERS SHALL BE EXAMINED. BILGE SUCTION TESTS WERE UNABLE TO BE PERFORMED DUE TO CAGE INSTALLATIONS AND ARRANGEMENT. MAKE ALL WATER TIGHT SPACES AVAILABLE FOR OPERATIONAL TEST OF BILGE SUCTION TO THE SATISFACTION OF THE ATTENDING COAST GUARD MARINE INSPECTOR. 46CFR 176.25-10(a)(7)

PENETRATIONS OF REQUIRED WATERTIGHT DECKS AND BULKHEADS BY ANY RIGID PLASTIC PIPE SYSTEMS ARE PROHIBITED, EXCEPT WHEN: EACH SUCH PENETRATION IS ACCOMPLISHED USING AN ACCEPTABLE METALLIC FITTING, WELDED OR OTHERWISE ATTACHED TO THE BULKHEAD OR DECK BY AN ACCEPTED METHOD; AND AN ACCEPTABLE METALLIC SHUTOFF VALVE IS INSTALLED ADJACENT TO THE THROUGH DECK OR THROUGH BULKHEAD FITTING. THIS VALVE SHALL BE OPERABLE FROM THE BULKHEAD DECK. IF TWO VALVES ARE INSTALLED, ONE ON EITHER SIDE OF THE BULKHEAD, THE VALVES NEED NOT BE OPERABLE FROM THE BULKHEAD DECK PROVIDED IMMEDIATE ACCESS TO BOTH IS POSSIBLE. PROVE PROPER INSTALLATION IAW REGULATIONS. 46CFR 182.40-1(a)(1)

EACH VESSEL INSPECTED AND CERTIFICATED UNDER THE PROVISIONS OF THIS SUBCHAPTER MUST, WHEN ANY PASSENGERS ARE ABOARD DURING THE TENURE OF THE CERTIFICATE, BE IN FULL COMPLIANCE WITH THE TERMS OF THE CERTIFICATE. PLAN REVIEW AND OFFICIAL CERTIFICATION FOR DIBS AND ZODIACS AUTHORIZED AS TENDERS ON VESSEL'S COI ARE INCOMPLETE. TENDERS ARE INELLIBIBLE FOR OPERATION AS CERTIFICATED SMALL PASSENGER VESSELS UNTIL COMPLETION OF CERTIFICATION, AND THEREFORE ARE RESTRICTED TO CARRYING NO MORE THAN SIX PASSENGERS IN ACCORDANCE WITH APPLICABLE REGULATIONS FOR UNINSPECTED PASSENGER VESSELS.

Submit plans for repair to V-2 sewage tank. Crop and renew holed area on inboard bulkhead of the V-2 tank on the sewage processing system.

Provide proper fire locker installation.

Propvide proper storage for excess constuction materials.

Confrim all components are completed on the letter from MSC reguarding the UPS bridge navigation equipment dated August 24, 2018 Project # PO11649

Chase and eliminate 120 VAC ground fault located on bridge panel

Replace busted prop on Dib "Leo"

Seal all penetrations on th ebridge with approved SFP

Demontrate proper operation FSD at galley

Provide proper escape route markings to properly match the FCP

Provide proper markings and number for PFD storage.

Demontrate proper operation of the emergency lighting in the aft passageway.

Provide current/up to date CSR

Holed inboard V-2 sewage tank.

Label laz emergency escape hatch in accordance with 46 CFR 122.606

Controls for engine room fixed CO2 system engine shut down shall be moved to a location outside the space protected by an authorized company.
