Each survival craft required on a vessel by this part must meet one of the following: (1) For an inflatable liferaft—Approved under approval series 160.151 or other standard specified by the Commandant, with the applicable equipment pack, as determined by the cognizant [name withheld] liferafts were removed for servicing. Vessel is limited to no more than 1 nautical mile from shore.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. Vessel did not have a valid Bridge-to-Bridge certification nor a valid Safety Radio Certification. 47 CFR 80.903

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment,and electrical shock. MI discovered numerous openings/missing breakers in both electrial panels on board. 46 CFR 183.200

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Each rescue boat required in accordance with § 180.210 of this chapter must be launched with its assigned crew aboard and maneuvered in the water as if during an actual man overboard situation. Vessel is required to conduct a successful Man Overboard Drill, including the launching of the rescue boat, prior to carriage of passengers.

Vessel does not have any primary lifesaving devices onboard. Vessel is limited to one NM from shore until such devices are re-installed and inspected.

Oceans, coastwise, limited coastwise, and Great Lakes routes. A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry—(1) Six hand red flare distress signals approved in accordance with § 160.021. Distress signals onboard were expired.

A pre-engineered fixed gas fire extinguishing system must: Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Generator did not secure when pre-engineered system activated.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessel did not have sails rigged for the inspection.

Each nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. There was no working nozzle on the vessel.

Each distribution panel or switchboard must be totally enclosed and of the dead front type. 02 panels in the engine room entry had open breaker holes and 01 breaker box cover was not attached to the housing.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Bilge pick-up in foc'sle is missing strainer.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Waterlight failed to blink.

An adult life jacket must be provided for each person carried on board a vessel. Vessel was missing 05 life jackets. Total persons on board is reduced to 125 until the missing lifejackets have been replaced and inspected.

Waste receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles on board were made of plastic.

A vessel must be fitted with a general marine radar system for surface navigation if on Limited Coastwise Route. Vessel unable to test radar during inspection. To be tested during the full sail inspection.

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The vessel missed its annual inspection. Vessel must undergo an inspection for certification prior to operations. 46 CFR 176.500(a)(1)

All electrical equipment and circuits must be clearly marked and identified. One circuit breaker on the electrical panel was not clearly marked. Appropriately mark breaker.

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of § 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. Flexible electric cord was spliced in forecastle of ship to power anchor light. Make connection permanent or remove from service.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging,blocks, fittings, and sails. Starboard side forward chain plate on main mast is showing signs of wear. Make piece available for further inspection.

Jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station.

Written Maintenance Plan: All inspected sailing vessel owner/operators should have a written Preventive Maintenance Plan (PMP) that specifically addresses their vessel’s sail rigging equipment and components. See NVIC 02-16 for details on what the plan should include. Provide plan and proof of implementation.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. EPIRB should be listed as inspected, under the Radiotelephony Safety Certificate.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Bilge pump hose on discharge of PTO pump is not in serviceable condition. Replace in kind.

Each distribution panel or switchboard must be totally enclosed and of the dead front type. Panel in engine room not enclosed.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh.

Double hose clamping is required. Both generators fuel lines not hose clamped.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Fuel leak on small generator poses a potential fire hazard. Make repairs.

Vessel was granted extension for DD. Route is limited to lakes bays and sounds untill credit hull exam is completed. NLT 31OCT2021

A vessel must be fitted with a Federal Communications Commission (FCC) type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station if: The vessel is self-propelled; The vessel has an oceans, coastwise, limited coastwise, or Great Lakes route. Radar installed but not operational. Provide operational radar. 705: vessel limited to Lakes Bays Sounds route only.

Each ladder used as a means of escape must be mounted at least 180 millimeters (7 inches) from the nearest permanent object in back of the ladder. Rungs must be: (1) At least 405 millimeters (16 inches) in width; and (2) Not more than 305 millimeters (12 inches) apart, and uniformly spaced for the length of the ladder with at least 114 millimeters (4.5 inches) clearance above each rung. In vessels salon, rope escape ladder provided is not suitable for use as a means of escape. Provide alternative ladder arrangement. 705: No passengers in the salon

A vessel of more than 19.8 meters (65 feet) in length must carry a minimum of three life buoys of not less than 610 millimeters (24 inches) in diameter. Ring buoy on operating station had frayed lifelines and its body was worn beyond repair. Provide new ring buoy.

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. In the forecastle the bilge piping strainer has numerous wasted areas. Replace strainer.

The owner, charterer, master, or managing operator of a vessel carrying overnight passengers shall have a suitable number of watchmen patrol throughout the vessel during the nighttime, whether or not the vessel is underway, to guard against, and give alarm in case of, a fire, man overboard, or other dangerous situation. Vessel master stated that vessels crew did not comply with the regulation while moored at the pier. Provide written policy that implements this requirement. 705: no overnight passengers

The generator circuit breakers must be interlocked to prevent the generators from being simultaneously connected to the switchboard. Interlock does not prevent the shore power and “little generator” breakers from being energized simultaneously. Provide a interlock that prevents any two breakers from being simultaneously energized.

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of §111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. Air conditioner in the stbd lazarette/crew area is powered by an extension cord and plug strip. Remove temporary wiring.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Cables penetrating watertight bulkhead in the port lazarette liquor cabinet are not watertight. Make cable penetrations watertight.

U.S. Vessels, in commercial service, that are bound for ports or places within the navigable waters of the United States are required to submit a notice of arrival (NOA). LIBERTY CLIPPER did not submit a NOA IAW 33CFR160.212.

Each officer employed on any vessel subject to this subchapter must have his or her license or merchant mariner credential onboard and available for examination at all times when the vessel is operating. Vessel master does not have his MMC onboard. Provide MMC as required by regulation.

All wiring and cables shall be suitable for marine service and for the particular installation used. Non marine grade electrical cable is permanently installed and powering multiple power receptacles in the vessel overnight accommodations space forward of the salon. Remove non marine grade cable.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places. Blades for exhaust/supply fan in over head of engine space vestibule are missing their guard. Provide guard for fan blades.

Each overnight accommodation space on a vessel with overnight accommodations for passengers must be fitted with an independent modular smoke detection and alarm unit that is UL217 approved. Port and stbd aft most overnight accommodations spaces do not have UL217 smoke detectors installed. Install smoke detectors as required. ***Prior to carrying overnight pax

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Port aft most overnight accommodations space has a light fixture mounted to the overhead that is powered by a permanently installed extension cord. Remove permanently installed extension cord.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Port aft most overnight accommodations space has a red light fixture on ceiling missing lamp and terminal covers. Provide lamp and terminal covers for light.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Central accommodations space immediately forward of the salon, inside cabin 6, has an electrical connection box with exposed electrical terminals. Connection box is at ground level stbd of the entrance door behind the sink. Provide cover for connection box.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Central accommodations space immediately forward of the salon, inside cabin 6, has a fan powered by a permanently installed and spliced extension cord. Remove permanently installed extension cord.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. A electrical cable splice outside of an enclosure located Inside forward most accommodations space along the upper edge of the stbd aft bulkhead. Make electrical connections inside of an approved enclosure.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Lights found hanging by wires in the salon along the aft bulkhead amidships. Install lights as designed.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Two box fans found powered by a permanently installed extension cord in the salon overhead. Remove permanently installed extension cord.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Dead ended electrical cable found on port side of engine space vestibule. Remove dead ended cable or insulate conductors and terminate inside of an approved electrical enclosure.

Electrical installations and equipment shall be maintained to result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Correct the following discrepancy prior to carrying passengers overnight. Openings to live electrical components found inside lower port side breaker panel inside the engine space vestibule. Provide adequate covers for all openings in breaker panel.

Openings in Watertight Bulkheads must have means to make them watertight. MTWB between E/R and Captain's Qtrs has multiple areas where corrosion/wastage has holed the bulkhead. Make written proposal for permanent repair to OCMI, and upon approval, complete work to satisfaction of attending [name withheld] must be satisfactorily completed prior to annual voyage south, and no later than 05OCT2019. ****In the interim, vessel is restricted to protected waters only, and no more than 1 NM from shore.****

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protection space. Automatic shutdown does not work.

EACH VESSEL THAT OPERATES ON THE HIGH SEAS, OR THAT OPERATES BEYOND THREE MILES FROM THE COASTLINE OF THE GREAT LAKES, MUST HAVE ON BOARD A FCC TYPE ACCEPTED CATEGORY 1, 406 MHZ EPIRB, INSTALLED TO AUTOMATICALLY FLOAT FREE AND ACTIVATE. HYDROSTATIC RELEASE ON EPIRB IS EXPIRED.

A HATCH EXPOSED TO THE WEATHER MUST BE WATERTIGHT. CHAINLOCKER HATCH GASKET IS WASTED.

A PORTABLE OR TEMP ELECTRIC CORD MUST BE CONSTRUCTED AND USED IN COMPLIANCE WITH REQUIREMENTS OF 111.60-13 IN SUBCHAPTER J FOR A FLEXIBLE ELECTRIC CORD. LARGE NUMBER OF NON-COMPLIANT EXTENSION CORDS USED THROUGHOUT ACCOMODATION SPACES.

ELECTRICAL EQUIPMENT ON A VESSEL MUST BE INSTALLED AND MAINTAINED TO PROTECT PASSENGERS, CREW, OTHER PERSONS, AND THE VESSEL FROM ELECTRICAL HAZARDS, INCLUDING FIRE, CAUSED BY OR ORIGINATING IN ELECTRICAL EQUIPMENT , AND ELECTRICAL SHOCK. LIGHT FIXTURE IN FWD COMPARTMENT DOES NOT HAVE A COVER, AND HAS EXPOSED WIRE SPLICES.

FOR PORTABLE FIRE EXTINGUISHERS... THE OWNER/MANAGING OPERATOR SHALL PROVIDE SATISFACTORY EVIDENCE OF THE REQUIRED SERVICING TO THE MARINE INSPECTOR. PORTABLE CO2 EXTINGUISHER IN STBD AMIDSHIPS COMPARTMENT HAS A WASTED HOSE.

A VOLTMETER, AMMETER, AND MEANS FOR MEASURING FREQUENCY SHALL BE PROVIDED FOR A GENERATOR THAT IS IN OPERATION. PROPER FUNCTIONING GAUGES NOT PROVIDED FOR EACH GENERATOR 46 CFR 183.10-5(g)

ALL PIPING, VALVES, AND FITTINGS SHALL BE IN ACCORDANCE WITH GOOD MARINE PRACTICE AND SUITABLE FOR THE PURPOSE INTENDED. FIRE MAIN HAS AN UNAUTHORIZED TEMPORARY REPAIR IN THE PORT SIDE MACHINERY SPACE. 46 CFR 181.15-5 (c)

ALL VESSELS OF 26 FEET IN LENGTH AND OVER SHALL BE PROVIDED WITH INDIVIDUAL BILGE LINES AND BILGE SUCTIONS FOR EACH WATERTIGHT COMPARTMENT. BILGE SUCTION ARRANGEMENT UNDER THE CAPTAINS QUARTERS DOES NOT MEET THE REQUIREMENTS OF 46 CFR 182.25-5(a).

AN ADULT LIFE JACKET MUST BE PROVIDED FOR EACH PERSON CARRIED ON BOARD A VESSEL. VESSEL IS RESTRICTED TO CARRYING 127 PASSENGERS, INCLUDING CREW, UNTIL ADDITIONAL LIFE JACKETS ARE PROVIDED.

THE COGNIZANT OCMI MAY REQUIRE THAT A VESSEL AND ITS EQUIPMENT UNDERGO ANY ADDITIONAL TEST OR INSPECTION DEEMED REASONABLE AND NECESSARY TO DETERMINE THAT THE VESSEL AND ITS EQUIPMENT ARE SUITABLE FOR THE SERVICE IN WHICH THEY ARE TO BE EMPLOYED. CONDITIONS OF STANDING RIGGING NOTED DURING ANNUAL INSPECTION ON 26SEP2017 WHICH WERE INDICATED BY MASTER TO HAVE BEEN IDENTIFIED FOR RENEWAL DURING 2018 DRYDOCK, HAVE NOT BEEN ADDRESSED. ITEMS DURING 2017 INSPECTION NOTED AS FOLLOWS: (1) Foremast starboard mid shroud has a bottomed-out turnbuckle (2) Lower main mast port/stbd fore shrouds: wire rope eyes slightly "bird caged" in thimbles IWO turnbuckles (3) Upper main mast stbd shroud: shackle connection to chain plate has improperly sized pin and turnbuckle is bowed. (4) Main sheet traveler end gussets corroded to about 50% of 1" original thickness. IN ADDITION, DURING CURRENT ANNUAL INSPECTION, MAIN TOPMAST SHROUD CHAINPLATE ON P/S WAS OBSERVED TO BE DEPLETED FROM CORROSION, AND TWISTED/DEFORMED. SUBMIT WRITTEN PLAN WITH TIMELINE TO SECTOR BOSTON TO ADDRESS THESE ISSUES AND ENSURE OVERALL INTEGRITY OF RIG.

Prove proper operation of pre-engineered fire extinguishing system. Ensure automatic shutdown of generator.

Properly mark and affix retro-relective tape on stbd ring buoy.

Provide functional lights on 03 life preservers.

Make penetrations in aft bulkhead of forecastle, stbd side, watertight.

Make proper connections/terminations of emergency lighting conductor stbd midships.

Two fire extinguishers found inoperable in Forecastle (dry chem - discharged) and Nav Shack (CO2 - hose worn/cracked). Provide 02 fire extinguishers.

Mount light switch in receptacle in forward compartment

Place spliced electrical wires in j-box with cover in port midships compartment.

Replace/repair distribution panel cover in port midships compartment

Make all engines that draw air from within space capable of shut down with Fireboy activation.

Grabline was wasted. Vsl had three other serviceable RLBs. Renew/remove from vsl.

EPIRB NOAA registration was expired. Vsl restricted to <3 NM from shore. Also required to mark date on hydrostatic release.

Prove proper operation of pre-engineered system.

Provide 02 liferafts. VSL restricted to 03 miles from shore.

Replace EPIRB battery. VSL restricted to 03 miles from shore.

Install retro reflective tape on 02 lifebouys.

Replace STBD midships #3 fire extinguisher.
