Each survival craft must be stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Inflatable survival craft were ashore for servicing and not available for inspection. Limited to 1 mile from shore.

All microprocessor or computer based systems must meet the requirements of part 62 in subchapter F of this chapter. The vessel's approved DVTPs for the Reversing bucket system were in correcntly subbmitted for approval. Re-submit the proper DVTP for the reversing system to the Marine Safety Center.

Carbon dioxide cylinders forming part of a fixed fire extinguishing system must be maintained as follows: cylinders must be retested every 12 years. The fixed fire CO2 cylinders were last hydro tested in 2011. Schedule and have serviced hydro test for the CO2 cylinders.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the OCMI. The starboard side engine room portable fire extinguisher bracket was broken. Repair/replace bracket to OCMI satisfaction.

waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacle on the bridge was made of combustible material. O/O is to remove or replace with proper style non-combustible waste receptacle.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical outlet receptacle outside the port electrical closet in main deck passenger space had a loose top outlet showing an open ground. O/O is to repair or replace to satisfaction of attending marine inspector.

Each battery used in an item of lifesaving equipment must be replaced during the annual inspection if the expiration date of the battery has passed. The battery for the ring life buoy water light was expired. Crew member replaced battery during the inspection.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Vessel’s STBD side bilge pump had a cracked male threaded pipe adjoining the bilge pump to the bilge suction line. O/O to repair or replace threaded pipe to the satisfaction of attending marine inspector.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Vessel’s port side #2 void bilge suction line pipe union was severely deteriorated and leaking. O/O is to repair or replace piping union to satisfaction of attending marine inspector.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. O/O is to repair or replace all overhead and vertical cable support hangers with metal supports/retention devices spaced no more than 48 inches apart prior to next credit hull/drydock exam.

A vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Aluminum hull vessels: (1) Rules and Regulations for the Classification of Yachts and Small Craft, Lloyd's; or (i) For a vessel of more than 30.5 meters (100 feet) in length—Rules for Building and Classing Aluminum Vessels, ABS. Vessel allided with the pier after disembarking passengers in Provincetown, MA. Found vessel's starboard quarter external structure inset approximately 2 inches by 1 foot, with a popped butt weld between the top rail and topmost longitudinal external frame. Structure comprised of external sheet aluminum, framing, and welded tube which protects the jet drive and scoop, aft of each hull. No penetration witnessed in the watertight envelope of starboard hull upon internal and external inspection. Repairs shall be made to return vessel to original design, to the satisfaction of attending Marine Inspector.

Immediately after addressing resultant safety concerns, the owner, agent, master, or person in charge of a vessel involved in a marine casualty shall notify the nearest sector office, marine inspection office, or Coast guard group office, whenever a vessel is involved in a marine casualty consisting of an occurrence materially and adversely affecting the vessel's seaworthiness or fitness for service or route. Vessel allided with the pier while mooring in Provincetown, MA causing damage to the vessel and the pier. Vessel is required to conduct a damage survey and any necessary repairs to the satisfaction of the attending USCG marine inspector. 46 CFR 122.202

Except as provided in paragraph (d) of this section, this subpart applies to each self-propelled vessel that (1) Carries oil of any kind as fuel for propulsion; (2) Is not a tank vessel; (3) Operates upon the navigable waters of the United States; and (4) is 400 gross tons or more as measured under the convention measurement system. Vessel does not have a Nontank Vessel Response Plan (NTVRP). Provide copy of NTVRP.

A pull cable used to activate the system controls must be enclosed in conduit. Cable pulley box in Port Side CO2 system cover is disconnected and deteriorated.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel suffered a fire onboard in starboard engine room due to an overheated PTO hydraulic pump on #3 MDE. Vessel is restricted from carrying passengers until repairs are made to the satisfaction of the attending marine inspector.

Controls and valves for operation of a fixed gas fire extinguishing system must be located outside the space protected by the system; and not located in a space that might be inaccessible in the event of fire in the space protected by the system. System engine shutdown pressure switched located inside of the protected space on both hulls. Submit plan to OCMI for relocation of engine shutdown pressure switches outside of the protected space. Upon approval relocate switches to a location outside of the protected space.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. 04 cracked longitudinal welds marked in the port and stbd forepeak. Repair weld to the satisfaction of attending marine inspector.

Vsl is broadcasting invalid IMO # and no static draft information

Piping for stbd CO2 system is wasted. Have fire servicing technician attest to integrity of system or renew.

Ensure fire control plan is posted.

Replace 3 temporary repair patches (approx 4x6 inches) with proper full inserts of new plate in way of scrape damage on starboard quarter.

Prior to carrying passengers, provide temporary repairs to 3 holes in hull, stbd aft above waterline, and provide repair for gouges to the satisfaction of the attending marine inspector.

All of the vessel's inflatable buoyant apparatus have been removed for annual servicing. The vessel is limited to a Lakes, Bays, and Sounds route not more than one nautical mile from shore until the IBAs have been re-installed to the satisfaction of an attending marine inspector.

THE VESSEL'S INFLATABLE BUOYANT APPARATUS HAVE BEEN REMOVED FOR ANNUAL SERVICING. THE VESSEL'S ROUTE IS LIMITED TO LAKES, BAYS & SOUNDS AND NOT MORE THAN ONE NAUTICAL MILE FROM SHORE UNTIL THE IBA HAVE BEEN RE-INSTALLED TO THE SATISFACTION OF AN ATTENDING MARINE INSPECTOR.
