Controls [for fixed CO2 systems] shall be installed in an accessible location outside the space protected. Pressure switches were installed inside the protected engine room space. System modification to be addressed IAW decision from Marine Safety Center (MSC) and prior 2026 hull credit. 46 CFR 181.20-10(b)(3)

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H. Doors in bulkheads required to be Class A-60, A-30, or A-15 shall be of hollow steel in equivalent metal construction solidly filled with approved structural insulation capable of meeting the requirements for a class A-15 bulkhead. Engine room door could not be confirmed to meet A-15 fire boundary. Replace door to satisfaction of attending marine inspector and prior to 2026 hull credit. 46 CFR 177.10-5, 46 CFR 72.05-25.

At each initial and subsequent inspection for certifications and reinspection the firefighting equipment of all vessels shall be inspected and all deficiencies found shall be corrected. Complete and simple instructions must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Marine Inspector observed failed pressure switches indicated on inspection report from Fire Equipment Inc dated 29Oct2025. Comments on the report stated pressure switches did not shut down main engines. Owner/operator to conduct additional servicing/testing of the system to verify pressure switches shut down vessel’s main diesel engines to the satisfaction of attending Marine Inspector. Provide system drawings and schematics for on board system. 46 CFR 176.25-25(Old T), 46 CFR 181.20-15(Old T)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its COI giving consideration to provisions for a seaworthy hull. Several areas of localized pitting was found in the #2 void port side forward. O/O to submit a temporary repair proposal to cognizant [name withheld] temp repair within 14 days. Permanent repairs are to then be made prior to next credit hull exam.

Areas of corrosion and wastage were found in the way of the starboard blackwater watertight bulkhead penetration. O/O to submit repair proposal to cognizant [name withheld] repairs are to then be made prior to next credit hull exam. 46 CFR 182.40-1

All vessels shall be provided with a suitable steering apparatus. Vessel’s starboard rudder hydraulic actuator leaking hydraulic fluid. O/O to repair/replace hydraulic actuator prior to receiving next credit hull exam. 46 CFR 182.30-1.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. Vessel’s bilge pick up pipe right after strainer in #6 void and bilge line coupling in #3 void near forward bulkhead was heavily wasted. O/O to repair/replace bilge pick up lines to satisfaction of attending marine inspector prior to receiving credit hull exam. 46 CFR 182.25-1

Penetrations of required watertight decks and bulkheads are prohibited except when each penetration is accomplished using an acceptable metal fitting, welded or otherwise attached to bulkhead by accepted methods, and an acceptable metallic shutoff valve is installed adjacent to the through hull fittings. Blackwater lines penetrating watertight bulkhead between void #6 and lazarette to be fitted with metallic shutoff valves. O/O to submit repair proposal to cognizant OCMI detailing inclusion of metallic shut off valves to the piping system, prior to receiving next credit hull exam. 46 CFR 182.40-1

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly checks on portable fire extinguishers were not completed.

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Operation of the fire main system and checking of the pressure at the most remote and highest outlets. When fire main was energized, the pipe burst while under pressure in the #3 void. Apply permanent repairs to fire main to the satisfaction of the attending marine inspector.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Port forward engine oil pressure gauge at operating station was reading improper pressure level.

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. Vessel's fire control plan is missing all required information except for fire protection/emergency equipment. Update plan to reflect required information/equipment that pertains to vessel.

Pitting was found on the over head of the vessel's number 2 void, including approximately 10 small holes through the deck where the forward part of the superstructure meets the weather deck. Provide repair proposal and make effective repairs to the satisfaction of the attending Marine Inspector. 46 CFR 116.100(a)

Goose-neck for aft ballast tank vent on the starboard side of the aft weather-deck, found with pinhole wastage where the standpipe meets the deck. Repair to the satisfaction of the attending Marine Inspector. 46 CFR 116.100(a)

The owner or managing operator of a vessel shall ensure that the vessel complies with the laws and regulations applicable to the vessel and that the vessel is otherwise satisfactory for the intended service. Excessive pitting was observed in the #6 void, port #4 bulkhead stiffener from centerline. Two substantial pits were observed with a depth of .270 and .210. Vessel representative shall make a final determination through visual inspection of the hull or by other NDT methods to determine plate thickness.

All escape hatches shall be marked on both sides using at least 1" letters "Emergency Exit, Keep Clear". Engine room emergency escape hatch was not marked on either side. Crew mounted placards during the inspection. 46 CFR 185.30-15

All electrical equipment must be clearly marked and identified. The port and starboard steering motor breakers were marked with the wrong amperage. (see pictures)

The application of these requirements to specific vessels shall be as determined by the Officer in Charge, Marine Inspection. Vessel's machinery space on the main deck and on the second deck do not meet the fire control boundary requirements of A-60 boundaries. Submit plans to OCMI for modifications to the spaces to meet structural fire protection requirements IAW: SUBCHAPTER H & NVIC 9-97. 46 CFR 177.10-5

Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gastight throughout its length and shall be properly supported by non-combustible hangers or blocks. Marine inspector noted evidence of exhaust leak in void #6 via visual condition of lagging and through smell while machinery was online. Locate source or exhaust leak and make repairs as nessecary to resolve issue.

Provide mitigation plan to prevent personnel exposure to exhaust hazard in #6 void space to remain in effect until deficiency #1 is corrected.

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: "Emergency Exit, Keep Clear", unless such markings are deemed unnecessary by the Officer in Charge, Marine Inspection. Fwd engine room escape hatch is not properly marked on engine room side. Mark hatch as required.

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. Upper rudder post plate on starboard steering arm is wasted and visually bent due to weakened metal. Replace stbd upper rudder post plate.

Class A bulkheads shall be so insulated with approved structural insulation, bulkhead panels, or deck covering that the average temperatures on the unexposed side would not rise more than 250 °F. Insulation not properly installed in aft stbd corner of engine room. Install missing insulation.

A battery charger intended for connection to a commercial supply voltage shall have an ammeter that is readily visible shall be included in the battery charger circuit. Battery charger on fwd stbd bulkhead in engine room has a non operational ammeter that is not properly mounted to the charger housing. Provide operational ammeter as required.

Each hatch cover must have a securing device. Lazarette hatch cover missing securing device.

Engine exhaust piping shall be gastight throughout its length. Stbd forward exhaust was found with excessive pitting at the point where it meets the side shell. Make gastight. 46CFR182.20-20

At each vessel inspection, all observed hazardous situations must be corrected. In lazarette, stbd quarter has significant inset, above which is a crack in the side shell showing daylight. Make side shell watertight. On port quarter, one cracked weld marked for repair. 46CFR115.830

At each inspection, the marine inspector may require that man overboard/fire drill be held under simulated emergency conditions. Unable to conduct drills due to strong winds.

Vsl is broadcasting no IMO #, not broadcasting a Call Sign, smaller than expected vessel length and no static draft information.

Remove & replace the wasted pipe on the forward generator raw water intake line. Prove proper operation of both generators while operating underway. Conduct sea trials to prove all components are in working order.

Several wasted pipes discovered in exhaust compartment (void #6). Issued requirement to make permanent repairs.

Significant inset was discovered on the port side of the lazerette, issued requirement to make permanent repairs.

Vessel replaced the port outboard shaft and realigned to gear, issued requirement to conduct underway sea trials.

Repair cracked weld on starboard inboard raw water pipe intake for the engine at the forward bulkhead in void #6. Conduct additional rounds every 30 minutes to ensure condition does not worsen.

Troubleshoot and repair stbd inboard MDE over speed alarm.

Repair or renew battery charger in pilothouse.

Provide cover for distribution panel in pilothouse.

Provide platform on MOB ladder.

Crop and renew wasted bulkhead IWO sewage tank.

Repair or replace wasted salt water overboard piping for generators.

Repair or replace wasted fresh water piping in exhaust void.

During MOB drill rescue platform was inadequate. Vessel to provide satisfactory arrangements to the satisfaction of the attending marine inspector.

Engine room door does not latch.

Stair tower doors have carpet installed on the threshold and doors do not self close.

Provide instructions for fixed firefighting arrangements.

Make pressure vessel available for inspection and replace relief valve.

Make permanent repairs to A/C raw water line in fuel void. A/C not to be used until repaired.

Compass light inoperable. Vessel restricted to daylight operations.

Provide current stability letter.

Make exhaust space safe and available for inspection. High levels of CO2 were found indicating an exhaust leak. Repair exhaust leak.

Vessels propulsion controls (pnuematic) inop for outboard starboard engine (locked in forward). Repair and test to the satisfaction of MI.

02 Windows damaged due to collision. Replace to the satisfaction of attending MI.

Crack was found in port fwd engine exhaust in void #6. Repair. 46CFR182.15-20(d)

Have non-conductive mat in front of switchboard. (Corrected on the spot)
