MAJESTIC PRINCESS

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every two years. Vessel Dry dock period lapsed on 28FEB2026. Complete credit dry dock prior to carriage of passengers.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Audible alarm is inoperable for all spaces. 46 CFR 119.530(a)

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. Fire pump gage is missing a needle and inaccurate. 46 CFR 181.10-1 (c) "OLD-T"

Vessels contracted for on or after July 1, 1961 which carry more than 150 passengers shall meet the requirements of subpart 72.01 of Subchapter H (Passenger Vessels) of this chapter. Fire Damper serving the Engine Room located on the aft E/R bulkhead, Port side does not completely secure. 46 CFR 177.10-5 (a) "OLD-T"

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Both PORT and STBD Main Engines contain multiple wasted areas on the cooling water/heat exchanger piping, elbows, and attachment points. Additionally, the PORT Main Engine contains crushed/dry-rotted piping serving the heat exchanger. Replace all affected piping sections to satisfaction of Marine Inspector on both main engines. 46 CFR 182.05-1 (a) "OLD-T"

Installation of diesel engine exhaust pipe shall be in accordance with 182.15-20. STBD ENG wet exhaust contains 02 injections in engine room, just FWD of the aft engine room bulkhead which are missing proper double-hose clamps. 46 CFR 182.20-20 (a) "OLD T"

Installation of diesel engine exhaust pipe shall be in accordance with 182.15-20. STBD ENG wet exhaust in lazarette is missing double hose clamps at both ends of the flexible section. 46 CFR 182.20-20 (a) "OLD T"

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Piping serving the sink drain on port side of the vessel in the fuel void contains a heavily corroded seacock which is missing a handle, unable to prevent water ingress in an emergency. 46 CFR 176.25-50

All unfired pressure vessels shall be Coast Guard approved and installed to the satisfaction of the Officer in Charge, Marine Inspection. Both Port and Stbd air receivers serving the main engines for starting air are rated at 250PSI MAWP. The gauges only range to 200PSI. 46 CFR 182.10-5 (d) "OLD-T"

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonable safe from electrical shock of personnel. Bar area on main deck contained a power strip with charred power receptacles. 46 CFR 183.01-5 (a) "OLD-T"

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended: (2) Overload or circuit protective devices. Distribution/Breaker panel in pilothouse and in fuel tank void are missing blanking devices where there are no breakers installed. 46 CFR 176.25-15 (a) (2)

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Multiple open plastic trash cans were observed throughout all areas of the vessel. 46 CFR 116.405 (i)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. No instructions for donning lifejackets were posted onboard the vessel. 46 CFR 122.516 (a)

A vessel at anchor shall exhibit where it can best be seen an all-round white light. Vessels anchor light was inoperable COLREGS Rule 30

Suitable covers, guards or rails shall be installed in way of all exposed and hazardous places such as gears, machinery etc. There is no cover on the starboard generator and starboard main engine which both have moving belts posing a safety risk to the crew and those working in the space. 46 CFR 177.35-15 "OLD-T"

Gages to indicate engine cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Oil pressure gauges/sensor is inoperable on the starboard engine. 46 CFR 182.20-5(b)

Fuel supply piping shall be of copper, nickel copper, or copper nickel…except that piping of other material such as seamless steel pipe or tubing which provides equivalent safety may be used. Existing valves serving both port and stbd fuel tanks are improperly terminated with non-metallic caps. 46 CFR 182.20-40(a) “Old-T”

Auxiliary machinery shall be suitable in type and design for the purpose intended. PORT generator is inoperable due to a failed starter. 46 CFR 182.10-1 "OLD-T"

Propulsion machinery shall be suitable in type and design. Starboard wet exhaust leak found near aft bulkhead. 46 CFR 182.05-1

Propulsion machinery shall be suitable in type and design. The starboard shaft seal has leak and packing gland has failed. 46 CFR 182.05-1

Propulsion machinery shall be suitable in type and design for the intended purpose of vessel. Observed outboard port propulsion engine s/w leak. 46 CFR 182.05-1 (Old T)

Vessels constructed after July 1, 1961, which carry more than 150 passengers, shall meet the requirement of Subpart 72.05 of Subchapter H. Structural Fire Protection Plan does not indicate the main deck's aft wooden bulkhead; the fire door to galley below the main deck does not adequately self-close and secure. Additionally, the fire door needs to open and close in the full range of motion as designed. 46 CFR 177.10-5 (Old T)

A vessel of more than 19.8 meters (65 ft) must carry a minimum of three life buoys... Upper deck life buoy not in serviceable condition. 46 CFR 117.70(a)(3)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Install machinery guards on engines & acquire safer gangway for vessel. 46 CFR 115.830(a)

The regulations contained in this part are substantially in accord with, and in general are based upon established codes and recognized marine standards and practices. Breaker panel aft and outboard port side of the galley needs to have a breaker directory that is up-to-date IAW good marine standards. Currently, there are three locations labeling breakers on this panel that are not alike. 46 183.01-15(b)

A vessel must carry either a first-aid kit that meets the requirements in46 CFR 199.175(b)(10)or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Vessel did not carry a first aid kit that was USCG approved. 46 CFR 121.710

(a) As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible; USCG MI observed electronic navigation system onboard vessel, however, the operator failed to demonstrate the "Declaration of Conformity" wrt the system's commercial and regulatory compliance and the last update to system was in 2019. Additionally, there were no up-to-date charts present. 46 CFR 121.420(a)(1)

Repair cracks along STBD rub rail.

A door must not be undercut more than 12.7 millimeters (0.5 inches) above the door sill or deck covering. Rugs and carpets must not pass through doorways, but linoleum and similar deck covering may. Vessel's plan is to cut the carpet under the pilot house door, in the "A" class boundary, and secure a threshold plate to the deck. (46 CFR 116.435(c)(5))

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] hatch located in the forward main deck dinning area was covered by a table and chairs, and was not marked. (46 CFR 122.606)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the cognizant OCMI. The frame or support of each fire extinguisher must be welded or otherwise permanently attached to a bulkhead or deck. (46 CFR 118.520)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. The connection for the bilge alarm in the lazarette does not meet the above condition. 46 CFR 120.340(g)

There is excessive pitting in the tank space compartment, located forward of engine room. Crop and renew bottom plating in forward three-quarters of tank space where bottom hull plating is 3/8" thick. Must be completed prior to next drydock credit, due 01Jan2024.

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. A crack was found on the weld of the second from top railing on the aft starboard quarter, main deck. 46 CFR 116.900

Deck rails (and bulwarks) must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Two cracks were found on welds on the starboard bow bulwark, main deck.

The storage or use of compressed natural gas (CNG) containers within the accommodation area, machinery spaces, bilges, or other enclosed spaces is prohibited. During deficiency check inspection, observed six portable butane stoves and several butane gas canisters presumably used as cooking devices in the vessel’s galley located below the main deck. Butane is considered a compressed natural gas and the portable stoves were not compliant with NFPA 302. The owner/operator verified the butane canisters and stoves would be removed from the vessel prior to passengers. In order to properly document and avoid future occurrences; this CG-835 will be uploaded into the vessel's documents record via MISLE CASE NUMBER:7452129”

The wire connections for the bilge sensor, for the void before the engine room was not made in a box but twisted and taped in the bilge.

The bilge alarm in the void before the engine room compartment was inoperable.

Pr-employment testing is required for any individual to serve as crew member unless the individual has passed a chemical test within the previous 6 months OR was subject to random drug testing within the past 185 days. Vessel representative informed attending Coast Guard that 02 members on board where newly hired and were subject to preemployment/random testing. Provide evidence of such testing. Additional, when the individual's exceed this time frame they must be added to the current company's drug and alcohol program. Cite:46 CFR 16.210 (b)

Employers must maintain records of chemical tests and must make these records available to Coast Guard officials upon request. Vessel did not have drug and alcohol records on board ready for inspection. Provide a maintained consolidated program on board at all times for inspection. Cite:46 CFR 16.260 (a)

At each initial and subsequent inspection for certification the marine inspector shall examine and test unfired pressure vessels to the extent necessary to determine that their condition is satisfactory. Two PVs in enginre room are overdue for internal visual exam. Cite:46 CFR "OLD" T 176.25-30

At each inspection the marine inspector shall require that all observed unsafe practices and hazardous situations are corrected. Witnessed on most forward starboard side bulkhead a large 4' x 4' section of insulation and hydraulic steering lines exposed to the elements of the galley. Considering its adjacent location to the deep fat fryers and galley stove/oven being a significant ignition source, the existing paneling that was removed to protect the insulation and hydraulic fluid lines shall be replaced to maintain structural fire protection. The master or person in charge of a ship shall ensure that the ship is not operated unless a garbage management plan meeting this section is on the ship and that each person handling garbage follows the plan. (b) Each garbage management plan must be in writing and describe procedures for minimizing, collecting, processing, storing and discharging garbage. As well as designate the person who is in charge of carrying out the plan. No garbage management plan was on board. Cite: 33 CFR 151.57

First aid kits on board must include the following contents that were missing or expired during inspection: (1) Tourniquet, (1) Wire splint, (10) ammonia inhalants Cite:46 CFR 160.041-4 (b)

Vessel's fire damper on the port aft side did not operate properly. Ensure port aft fire damper closes satisfactorily. Cite: 46CFR116.610

Vessel's emergency lightting continuously trips the breaker. Ensure emergency lighting operates as intended. Cite: 46CFR120.432

Vessel's FCC Safety and Operator Certificates are expired. Renew FCC certificates. Vessel must operate within 1000ft from shore. Cite: 47CFRPart13

Prior to departure from the pier submit a dive report to the attending MI.

Conduct sea trials with the approval of the attending MI.

High level bilge alarm in E/R must be placed in junction box.
