All machinery shall be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. At the time of inspection, the vessel was found to have exhaust leaks on each of the vessel's four Main Diesel Engines. Operator to rectify within 30 days to the satisfaction of the attending Marine Inspector. 46CFR182.01-5

Rudder angle indicator has not functioned in several months. Repair rudder angle indicator or provide an equivalent level of safety prior to the carriage of passengers.

The steering pump indicator lights on the bridge have been covered with electrical tape impeding the visibility of the lights. Remove tape (if lights are too bright, replace with less intensity bulbs or cover with a darker lens) prior to the carriage of passengers.

Portable fuel systems, including portable tanks and related fuel lines and accessories, are prohibited except where used for portable dewatering pumps or outboard motor installations. Portable red gasoline tank filled with diesel observed stored in crew storage closet in lower-deck passenger accommodation, and not intended for portable dewatering pumps or outboard motors. 46 CFR 119.458

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. The bilge pick-up pipe in the forepeak has a hole in it. Repair pipe to the satisfaction of the attending marine inspector.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The vessel is equipped with an EPIRB, but the vessel's Safety Radiotelephony Certificate does not have the EPIRB box checked. Have a FCC licensed technician re-issued certificate.

For semiportable and fixed gas fire extinguishing systems, the inspections and tests required by Table 115.810(b). The owner or managing operator shall provide satisfactory evidence of the required servicing to the marine inspector. While witnessing the annual servicing of the vessel's fixed gas fire extinguishing system (FFS) by a certified technician, the main stop valve failed twice to operate both through the time delay and manually, preventing the testing agent (carbon dioxide) to flow into it's designated space (engine room). The system must be repaired, re-tested, and documents provided showing that the repair were conducted IAW manufacturers manual. Vessel is authorized to conduct a 01 time transit from Long Wharf gate 4 to Cashman Yard in Quincy to conduct/enact repairs. 46 CFR 115.810(b)(2)

System components must be listed and labeled by an independent laboratory. A component from a different system, even if from the same manufacturer, must not be used unless included in the approval of the installed system. During annual servicing of FFS, it was observed that the system charged indicator did not operator for 02 tests. Indicator must be addressed & re-tested. 46 CFR 118.410(a)(2)

For semiportable and fixed gas fire extinguishing systems, the inspections and tests required by Table 115.810(b). Halon-1301, recharge or replace with if weight loss exceeds 5 percent. Fire technician stated the system had lost more than 5 percent of cylinder weight and did not contain enough agent to extinguish an engine room fire. Refill tank or replace with an approved system to the satisfaction of the attending marine inspector. If refilled the following conditions must be met: 1. Provide latest servicing report from 25Oct23. 2. Provide information on company providing the agent and refilling the bottle. 3. Replace tank fittings and pressure gauge. 4. System service must be conducted after initial refill to ensure no leaks and serviceable condition by a qualified technician in the presence of marine inspector. 5. After initial refill, system must be serviced once every three months by a qualified technician in the presence of a marine inspector or until system is replaced.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Outboard skin of port rudder was peeled back due to weld failure. Renew plate and test weld.

Each floating water light must have a lanyard of at least 3' but not more than 6' securing the waterlight around the body of the ring life buoy. Water light is not properly attached.

Vessel must have permanently marked draft marks at each end of the vessel. Vessel has no visible draft marks. 46 CFR 185.30-3(b) Due NLT next DD. 31JAN2022.

Machinery spaces shall be separated from other spaces by a watertight bulkhead. Small hole on aft watertight bulkhead of engine room. Temp repair approved. permanent repair NLT next DD 31JAN2022.

All piping, valves, and fittings shall be in accordance with good marine practice and suitable for the purpose intended. In the #2 void, the fire main leading to the forward fire station has a severe leak at the coupling at the deck penetration. Make permanent repairs to the fire main.

Each length of hose must have a nozzle that has a ½” solid stream orifice or a combination nozzle approved by the Commandant in accordance with 46 CFR 162.027-6. The fire hose nozzle at the aft station does not have the ability to spray a straight stream, and is in poor condition.

In all ventilation systems, manually operated dampers shall be provided in accessible locations, outside the spaces served by the system for shutting off the passage of air in the event of fire. The port and starboard aft dampers would not close using the manual pull.

Recharge or replace if weight loss exceeds 5% of the weight of the charge or if cylinder has a pressure gauge, recharge cylinder if pressure loss exceeds 10%, adjusted for temperature. The gauge on the halon cylinder is below 10% and does not match what is recorded on the last servicing certificate from November 2019.

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place in the space where the extinguishing agent cylinders are stored. The instructions posted at the halon station are insufficient as the valves are not numbered or identified.

All escape hatches and other emergency exits used as means of escape must be marked on both sides. The emergency escape from the engine room was not marked.

A durable placard must be posted next to all radio telephone installations with the emergency broadcast instructions and information. Vessel did not have placard posted.

The stability letter must be posted under glass or other suitable transparent material at the operating station. The current stability letter was not posted.

Each EPIRB must be tested monthly and shall be logged or otherwise documented. EPIRB testing was not logged.

The movement of a vessel shall be under the direction and control of the master or a licensed mate at all times. The vessel’s COI allows for a licensed mate to be substituted by a senior deckhand that is designed in writing. The senior deckhand was not designated in writing upon start of inspection.

All vessels must be provided with bilge pumps in accordance with Table 182.25-10(a). The primary bilge pump was inoperable, but was repaired during the inspection.

Engine room means of escape needs to be labeled IAW CRF. Label means of escape from engine room.

Engine room means of escape needs to have emergency light IAW CRF. Provide emergency light for means of escape from engine room.

Upper deck max pax should be labeled at all access points. No sign indicating max pax on upper deck aft access.

Fixed fire ext. system must be hydrostaticly tested as required. No date of last hydro available.

Vessel required to have draft marks. No draft marks are permanently affixed on vessel. Permanently mark draft marks as required NLT next DD 31JAN2020

DC12 panel in PH breaker to VHF, Flux, radio and GPS do not match the approved drawing. make correction and get approval from MSC

120VAC panel in ER has breaker to sub panel in PH new installation that does not match the approved drawing. Make corrections and get approval from MSC

Spec sheets for new instalation of equipment for propulsion control need final approval from MSC. submit and get approval from MSC

DRW 320-03 Rev C needs revision to show installation of disconect switch prior to 10A fuse to MDE stops. Make corrections and get approval from MSC

Each vessel inspected and certificated under the provisions of Subchapter K must be in full compliance with the terms of the certificate. Vessel has removed its IBA for servicing and is limited to a route 1nm from shore until the raft is onboard and properly installed.

Crop and renew hull in way of puntured area, void #3, port side.

#4 bilge piping failed to pump water from compartment. Prove proper operation of bilge system.

Discovered wasted piping on supply water and discharge line for the starboard head. Repair holed piping.

Numerous failures of structural fire protection insulation throughout the engine room. Make repairs with approved materials.

Halon discharge instructions were damaged and unreadable. Provide Halon discharge instructions.

Cracked welds were found on the air receiver where the compressor mounts. Make repairs and re-weld.

Extinguisher was found un-serviced. conduct annual servicing.

Steering compartment hatch was found without a gasket installed. Intall gasket and make hatch water tight. 46 CFR 116.1160

Bridge STBD Aft propulsion engine's lube oil pressure gauge was inoperable. Prove proper opeeration of guage. 46 CFR 182.20-5

The vessel's 100-person IBA has been removed for annual servicing. The vessel is limited to operations not more than one nautical mile from shore until the IBA has been re-installed to the satisfaction of an attending marine inspector.

Make pressure vessel available for internal inspection.

Vessel is restricted to three miles from shore until EPIRB is returned from servicing.
