Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Stbd inner MDE experienced a punctured block due to a failed connecting rod. Vessel is allowed to operate on the three other engines not to exceed 12 knots so as to not overwork the remaining engines. Repair or replace engine in kind or to the satisfaction of the cognizant OCMI.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space, including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Attending MI found that the welds on bulkhead stiffeners on transom in lazarette did not have adequate tie in/penetration where previous repair occurred aft of the wet exhaust piping. Permanent repairs to be complete at the credit drydock in 2026. 46 CFR 115.600(d)

The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float and buoyant apparatus. This number must: ( 1) Be the number of persons the device is equipped for; and (2) Not be greater than the number the device is approved for as shown on the data on the nameplate. Persons capacity was not marked on the life floats and the nameplates were no longer legible due to being painted over.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 65 feet in length must also have navigation lights that meet UL 1104 "Marine Navigation Lights," or other standard specified by the Commandant. Vessel has LED navigation lights that are not marked with UL listing. Prove installed lights meet UL 1104 or install navigation lights that meet UL 1104.

Each battery used in an item of lifesaving equipment, must be replaced during the annual inspection if the expiration date of the battery has passed... Except for storage battery, each battery without an expiration date indicated on it or for which the the owner or managing operator does not have a record of the expiration date, used in an item of lifesaving equipment, must be replaced during the annual inspection. Batteries in the life float and ring buoy lights have expired or are not marked with expiration dates. Renew all batteries with approved batteries as indicated by the light manufacturer.

Where liquid level indicating gages are installed,they shall be of heat resistant materials, adequately protected from mechanical damage and provided at the tank connections with devices which will automatically close in the event of rupture of the gage or gage lines. Vessel has a gate valve which does not automatically close in the event of rupture of the gage or gage lines. Cite: 46 CFR 182.20-25(1)(5) Old T

Pitting found in lazarette hull plating starboard side in way of rudder post and forward corner at the bulkhead. 04 pits to be repaired by clad welding; 02 pits in Bay #4 and 02 pits near rudder post not protected by external doubler. All other pits greater than 0.16" deep to be cleaned to good metal and filled with a suitable marine epoxy for corrosion resistance. Both areas are to be cropped and renewed prior to credit drydock in 2026. Until renewed, high water alarm in lazarette shall be tested and logged daily.

Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads. Small hole developed in way of the STBD quarter at the transom, approximately 4' above the waterline. Operator is permitted to install a temporary patch to continue operations until permanent repairs are completed as part of the next credit hull inspection. Vessel is to operate on a Lakes Bays and Sounds route only during this period, the Limited Coastwise route is deactivated until permanent repairs are complete. Conduct rounds of the lazarette at each stop on the ferry run.

(a) At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather desks. During Annual Exam, a crack in the starboard side shell near the transom. Repair the crack to the satisfaction of the attending marine inspector.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. The vessel's starboard outboard main diesel engine was found to have a fuel leak on the piping to the engine's fuel strainer isolation valve. Repair to the satisfaction of the attending marine inspector.

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. The lighting fixture cover in the lazarette was found to be incorrectly affixed. Repair to the satisfaction of the attending marine inspector.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Fuel supply/return lines on generator engine/s were found to be deteriorated. Replace fuel lines on port and starboard generators.

Each lighting fixture, globe, lens or diffuser must have a guard or be made of high strength material. Florescent light fixture was found to have a broken clip that secures the light cover. Replace clip/cover to meet the requirements in 46CFR120.410.

Employers must ensure that they and their crewmembers meet the requirements of this part. Vessel was unable to provide enrollment for the DAPI program.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Vessel limited to 3mn operation due to expired Hydrostatic release.

The minimum capacity of the fire pump shall be not less than 60 pounds per square inch at the pump outlet. The fire pump only makes 30 psi at the pump outlet. 46 CFR 181.10-1(c)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. One of the forward, port lifejacket bins is missing the number of lifejackets stored inside. Add a label for the number of lifejackets stored in that container. 46 CFR 122.604(f)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Lifejackets are stored on the second deck, but there are no donning instructions posted. 46 CFR 122.516(a)

All escape hatches used as a means of escape must be marked on both sides in clearly legible letters at least 2 inches high "EMERGENCY EXIT, KEEP CLEAR". The emergency escape hatch on the stbd side near the bar on the main deck is not adequately marked. 46 CFR 122.606

Machinery spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. The foam gasket under the door leading from the accommodation space on the main deck to the engine room on the stbd side is wasted. 46 CFR 116.405

All electrical equipment, including distribution panels, shall be dripproof protected. Breaker panel in the engine room on the port side aft does not latch closed. 46 CFR 183.10-10(a)

All electrical installations shall be to the satisfaction of the Officer in Charge of Marine Inspection. The bilge alarm junction box in the aft steering void near the centerline on the transom is not watertight as designed (missing stuffing tube). 46 CFR 183.01-15(a)

Port outboard propulsion engine raw water hose dry rotted. Hose is located on the onboard side at the expansion tank outlet. Replace hose in kind.

Aft emergency escape hatches on port and starboard of engine room not properly marked. Mark emergency escape hatches as required.

Starboard generator failed to start during operational testing. Starting battery replaced. Generator op test sat.

No visible draft marks on vessel. Provide draft marks as required. 46CFR122.602(b)(1) NLT next DD on 2020

Access control markings on starboard door into pilot house missing restricted access marking. Provide markings as required.

Where subject to moisture, a storage cylinder must be installed to provide a space of at least 2 inches between the deck and the bottom of the storage cylinder. Storage cylinders on the MASSACHUSETTS are currently resting directly on the deck in an area subject to moisture.

Owner/Operator shall conduct tests to determine that fixed gas fire extinguishing equipment, including controls, are in operating condition. Cylinder control heads linked to engine room rate of rise heat sensors for automatic deployment of fixed CO2 system are inoperable. System is currently capable of manual operation only.

Vsl is broadcasting NAME with Vsl type precursor (NAME must not include Vsl type precursors, e.g. F/V, M/V) and no static draft information

Provide vessel with approved DVTP's and PSTP's.

Make permenent repairs to STBD M/E. Prove proper operation of main propulsion.

Multiple wires were damaged due to a fault in the engine control system. Replace in kind all damagaed wires.

Conduct operational test of all propulsion equipment to the satisfaction of attending marine inspector.
