Exhaust gas must not leak from the piping or any connections. Exhaust on STBD generator was leaking into the engine room during the inspection.

Each ring bouy with a floating waterlight must have a lanyard; Each ring life buoy must be approved IAW 160.050. Ring buoy on main deck, STBD side, had a water light lanyard degrading. Ring buoy on 2nd deck had a grab line degrading.

All cable and wire must be installed with metal supports. Cable runs in both Void 2's and the PORT jet drive compartment were installed with zip ties.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles in the bridge and bathrooms were made of plastic. O/O is to remove plastic trashcans or furnish noncombustible trashcans to the satisfaction of attending marine inspector.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Exhaust fans in jet room/sewage tank void were missing protective blade covers. O/O is to install means to prevent an accident caused by rotating machinery to the satisfaction of attending marine inspector.

Jet drive hydraulic tubing in jet room was found to have a pinhole leak causing a constant stream of hydraulic fluid to leak from it creating a possible fire hazard. A hydraulic clamp cover was used for a temporary repair to make the system intact. O/O is to submit repair proposal to OCMI for approval and make permanent system repairs to the satisfaction of attending marine inspector.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. 2 junction boxes in the port jet room were found to be have broken covers exposing electrical connections. O/O is to repair electrical junction boxes to the satisfaction of attending marine inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. The deck area directly aft of the stairwell to the upper passenger deck, and the upper passenger deck in vicinity of the inflatable buoyant apparatuses were found to have broken deck support stiffeners causing the deck to buckle and pop up. O/O is to submit a repair proposal to the OCMI and make permanent repairs prior to vessel's next credit hull examination.

Each lighting fixture and lampholder must be fixed. The light fixture in the port engine room was held up by a temporary repair. The plastic cable tie holding the light fixture broke after a touch and go grounding.

Penetrations in A-class fire boundaries for electrical cables must be constructed to prevent the passage of flame and smoke for one hour. The cable penetrations in both the port and starboard jet rooms are deteriorated, affecting the integrity of the fire boundary.

Materials used in piping systems must meet the requirements of this subpart & be acceptable to the OCMI. The bilge pipe for the port side #3 void had a pinhole leak near the coupling in the engine room.

Controls and valves for operation of a fixed gas fire extinguishing system must be located outside the space protected by the system; and not located in a space that might be inaccessible in the event of fire in the space protected by the system. System engine shutdown pressure switch is located inside of the protected space on both hulls. Submit plan to OCMI for relocation of engine shutdown pressure switches outside of the protected space. Upon approval relocate switches to a location outside of the protected space.

Whenever damage to hull plating that may affect the seaworthiness of a vessel is discovered, the OCMI may conduct internal structure examination and required the vessel to be taken out of service to asses extent of damage and effect permanent repairs. Stbd laz outboard hull mid section has pin hole about two inches from water line.

Vsl is broadcasting no static draft information

All of the vessel's IBAs have been removed for annual servicing. The vessel is limited to a Lakes, Bays, and Sounds route not more than one nautical mile from shore until the IBAs have been reinstalled to the satisfaction of an attending marine inspector.

Conduct repairs to popped welds on rub rail starboard quarter stanchion along deck appx. 4’ – 5’ above water line and rub rail 1.5’ above water line at next dry-dock or before 31MAY19; 46CFR176.700.

Repair leaking fuel line return on #4 engine prior to carrying passengers, 46 CFR 119.435.

Vessel is restricted to three miles from shore until EPIRB is returned from servicing.
