A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station:A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. Controls failed to override the shutoff and restart the engines.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. STBD steering linkage bolt was loose.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Port shut off valve spun fully around.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel did not have required contents of first aid kit. Vessel's first aid kit was missing multiple required items.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The lazarette bilge high level alarm did not provide visual alarm at operating.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. Starboard fuel tank vent flame screen was damaged.

Each fire hydrant must have a valve installed to allow the firehose to be removed while the fire main is under pressure. Fire hydrant closure valve on deck near operating station ladder was leaking while fire main was energized.

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. The port passenger rail stanchion 4th forward from the stern was cracked at the base. Repair to the satisfaction of the attending marine inspector.

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. The passenger embarkation gate was missing a securing nut on the lower hinge bolt and the eye hole for the gate lock pin was excessively worn. Repair to the satisfaction of the attending marine inspector.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Aluminum hull vessels: ABS High Speed Craft. Marine inspectors found pitting in the second void space along the port and starboard sides of the hull adjacent to the keelson. Pit gauge measurements revealed individual pits and pit clusters with measured wastage up to 3/16". Submit repair plan proposal to OCMI for approval prior to conducting repairs. 46 CFR 177.300(d)(2)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 12 inches into the machinery space. The cable connected to the port fuel shutoff valve detached during valve closure testing. Crew was able to fix the cable linkage and successfully test the fuel shutoff valve prior to the marine inspector departing the vessel. 46 CFR 182.455(b)(4)

The flame screens must consist of a single screen of corrosion resistant wire of at least 30 x 30 mesh. The port side fuel vent was missing the removable flame screen. Install flame screen to the satisfaction of the attending marine inspector. 46 CFR 182.450(e)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the OCMI. The portable fire extinguisher located at the operating station was not bracketed in place to as to be secured in its stored position. Properly secure portable fire extinguisher to the satisfaction of the attending marine inspector. 46 CFR 181.520

A vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Aluminum hull vessels: ABS High Speed Craft. The third void space port hull sideshell had three holes where bolts had previously secured a metal plate to the hull. The holes are located above the waterline but below the weather deck. Holes to be repaired to restore watertight integrity to hull sideshell to the satisfaction of the marine inspector. 46 CFR 177.300(d)(2)

Except for a storage battery used in a rescue boat, each battery without an expiration date indicated on it or for which the owner or managing operator does not have a record of the expiration date, used in an item of lifesaving equipment, must be replaced during the annual inspection. The water light batteries for the life floats and ring buoys did not have expiration dates marked on the batteries. Renew water light batteries to the satisfaction of the attending marine inspector. 46 CFR 185.726(e)

Each vessel must be equipped with at least two operable portable battery lights. Vessel was missing one portable battery light. Prior to MI departing crew provided two functioning portable lights. 46 CFR 183.430

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with 47 CFR 80.59 once every five years and be issued a Communications Act Safety Radiotelephony Certificate. The Safety Radiotelephony Certificate was expired. Vessel is restricted to operating no further than 1,000 feet from shore while in service until issued a Safety Radiotelephony Certificate. 47 CFR 80.903

Rail courses or the equivalent must be installed between a top rail required by paragraph (a) of this section, and the deck so that no open space exists that is more than 12 inches high except on passenger decks of a vessel on an excursion trip the following must be installed... bars, slats, rail courses, or an equivalent spaced at intervals of not more than 4 inches. Vessel's aft deck section has an opening between the deck and the stern bench exceeding 12 inches in height without a rail course of not more than 4 inch intervals. Submit repair proposal to OCMI for approval prior to repair work. 46 CFR 177.900(g)(1)(iii)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Light fixture in engine room was missing guard and light bulb. 46 CFR 183.410(a) 701c

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Submersible pump in lazarette does not engage when automatic switch is activated.46 CFR 182.100 16c

All machinery and equipment must be maintained in such a manner as to afford adequate protection from machinery failure. Vessel experienced a loss of maneuverability while transiting inbound to dock. Vessel to identify cause of machinery failure, affect repairs and conduct satisfactory sea trial in presence of attending Marine Inspector.

Vessel to identify cause of machinery failure, affect repairs and conduct satisfactory sea trial in presence of attending Marine Inspector.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. The fire extinguisher located at the operation station was not stored in permanent bracket or in a manner that would prevent damage to extinguisher while underway.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a space with a through-hull fitting such as a lazarette. Vessel's engine room high water alarm did not have a working visual indicator at operating station and vessel's lazarette audible and visual high water alarm were completely inoperable.

As appropriate for the intended voyage, a vessel must carry adequate and up to date charts of large enough scale to make safe navigation possible. Navigational charts that were on board the vessel were from 2014 and 2017 respectively and were not corrected.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personal injury. Vessel's port main diesel engine had a significant oil leak on aft end of engine underneath turbo charger posing a possible fire hazard.

Penetrations in watertight bulkheads must be made to be watertight. The watertight bulkhead between the tank and engine room has penetration from an old electrical cable. Make the penetration watertight.

Connections must be made to battery terminals with permanent type connectors. The connectors are using temporary wingnuts. Replace the wingnuts with permanent connectors.

Except when individual pumps are provided for separate spaces, individual bilge suction lines must be led to a central control point or manifold and provided with a stop valve at the control point or manifold and a check valve at some accessible point in the bilge line. A stop-check valve located at a control point or manifold will meet the requirements for both a stop valve and a check valve. The bilge suction lines did not have a check valve.

A bilge pipe in a vessel of not more than 19.8 meters (65 feet) in length must be not less than 25 millimeters (1 inch) nominal pipe size. A bilge pipe in a vessel of more than 19.8 meters (65 feet) in length must be not less than 40 millimeters (1.5 inches) nominal pipe size. A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. The bilge strainer on the Lazarette and the two spaces forward did not meet the strainer size.

Electrical equipment on a vessel must be installed and maintained to:Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Expose wires at the operation station, and the electrical switch panel was empty and not blanked.

A submersible electric bilge pump may be used as a power bilge pump, provided that the pump is permanently mounted. Engine room electric submersible pump was not permanently mounted.

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. Engine room electric submersible pump light was inoperable.

A power driven vessel underway shall exhibit sidelights. The vessel sidelight on the STBD was not exhibiting an unbroken light arc of the horizon of 112.5 degree. Demonstrate proper operation of the navigation light. Vessel restricted to day operation.

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, the minimum capacity of the fire pump must be 50 gallons per minute at a pressure of not less than 60 psi at the pump outlet. The pump outlet must be fitted with a pressure gauge. The pressure gauge on the pump did not read 60 psi. After changing the gauge the PSI was satisfactory.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. The markings for the fuel shutoff were not present.

Ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Ring Buoy was missing the retroreflective tape.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the recognized OCMI... The shore side installation does not match the approved drawing approved by MSC. Operator or owner must submit drawings for approval, until approved the circuit must not be utilized.

During a drydock examination, all accessible parts of the vessel's underwater body will be examined. The turnbuckles and bolts in vicinity of port and starboard aft jet drives are corroded.

A hatch cover must be attached to the hatch frame or coaming by hinges, captive chains, or other device of substantial strength to prevent its loss. #4 void watertight hatch cover is inadequately attached to the vessel.

A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds. Vessel's steering rod is deformed and not of acceptable strength for normal operation.

On a vessel of not more than 65 feet carrying more than 49 passengers, the minimum capacity of the fire pump must be 50 gallons per minute and not less than 60 psi. Vessel's fire pump reached a maximum output of 30 psi.

On a vessel of at least 26 feet, a visual and audible indicator must be provided at the operating station to indicate a high water level. Indicator light for high water level alarm was not operable. Vessel is given 7 days to rectify from the date of Inspection.

The shutoff valve at the tank must be manually operable from outside the compartment. Starboard fuel shutoff valve inoperable. (Prior to carrying passengers)

At each subsequent inspection the portable fire extinguishers must be inspected and maintained in accordance with NFPA 10. The portable fire extinguisher located at the operating station and the portable fire extinguisher located at the bottom of the stairs are low.

03:Each item of lifesaving equipment determined by the Marine Inspector to not be in serviceable condition must be repaired or replaced. The life ring grab line is deteriorated.

Propulsion machinery must be suitable in type design for propulsion requirements and capable of operating at constant machine load under such requirements. Vessel was experiencing intermittent engine shut down. Vessel required to conduct sea trials & prove proper engine operations under normal conditions.

Remove and replace cracked nozzle on the port engine. Conduct sea trials to prove proper operation after repairs.

Multiple battery terminals are broke. Replace terminals.

Pressure guage missing on firepump. Install gauge.

Pre-Engineered system means to reset device is inoperable. Prove proper operation of reset device.

Conduct underway drills.

Port fwd handrails are cracked. Port aft handrails are cracked. Repair or replace.

Damage caused by collision to both port and starboard aft sections of the vessel. Repair and make watertight to the satisfaction of attending Marine Inspector prior to carrying passengers.

Stantion #6 and #7 on port side are cracked. Grind and weld.

STBD side seat row #3, #3, #4, and rows #2, and #3 on port side have cracked welds. Grind and weld.

Port and STBD side hand rail courses missing. Install courses.
