Each drain installed in a vessel's small discharge containment must be fitted with a mechanical closure. The small discharge containment fitted to serve the vessel's port side fuel tank deck fill pipe was found to have a metal plug that was wasted and incapable of making a seal in the small discharge containment coaming. 33 CFR 156.120(o)

The owner, managing operator, or a qualified servicing facility as applicable shall test...time delays, alarms, and ventilation shutdowns as stated in the system manufacturer's instruction manual. Fire damper for fixed pre-engineered system in furnace room was not tested at last attendance by a fire service technician. Furnace is not to be used until ventilation shutdown is tested satisfactorily, to include the fusible link. 46 CFR 115.810(b)(2)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Port sidelight and stern light bulbs were inoperable/missing. 46 CFR 120.420

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. The painter on the port aft IBA was incorrectly attached to the float-free link. 46 CFR 117.130(a)(1)

Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles on bridge and in both heads were plastic. 46 CFR 116.405(i)

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. The vessel sustained damage to STBD pulpit in 3 locations. The largest area of damage is about 1 foot long and 6 inches wide. The second area is about 10 inches long and inches 5. The third area is about 5 inches long and an inch and a half wide. O/O must repair stbd pulpit damage to the satisfaction of the attending marine inspector before issuance of next drydock credit.

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. Starboard IBA hydrostatic release's painter was affixed to the side of the hydrostatic release unit that attaches to the deck and not to the weak link. 46 CFR 117.130(a)(3)

A fire damper must close against the draft in the duct and be accessible for periodic inspection by means of a hinged or bolted plate in the duct and surrounding bulkhead or deck, if fitted. 3 out of 6 total manual pull station's duct closures for both engine room's CO2 fire suppression system did not close all the way. 46 CFR 116.610(g)(2)

A hatch cover must: Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Port engine room escape scuttle was not attached to neither the frame nor the coaming. 46 CFR 116.1160(b)(2)

All cable and wire must: Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Multiple spaces had hanging, unsupported electrical cables in overhead. 46 CFR 120.340(b)(4)

All passenger accommodations must be arranged and equipped to provide for the safety of the passengers in consideration of the route, modes of operation, and speed of the vessel. Starboard aft passenger bench had loose hardware and was not securely affixed to the deck. 46 CFR 116.800(a)

Leading to the upper passenger deck, port aft stairwell's uppermost bolts were missing/loose with the exception of one. 46 CFR 116.800(a)

A portable or temporary electrical cord or cable must be constructed and used in compliance with the requirements of 46 CFR 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. A flexible electric cable is permanently installed at the main distribution panel for an unknown circuit. Identify circuit and renew flexible cable with cable that complies with 46 CFR 120.340(d) to the satisfaction of the attending marine inspector. 46 CFR 120.340(r)

A fire damper must close against the draft in the duct and be accessible for periodic inspection by means of a hinged or bolted plate in the duct and surrounding bulkhead or deck, if fitted. One of the Port engine room's fire damper did not release at all using the manual pull station. 46 CFR 116.610(g)(2)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arrestors. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 x 30 mesh. Flame screen for port fuel tank goose vent was wasted and had numerous holes compromising it's ability to prevent flame/spark from entering the fuel tank. Goose neck vent float ball was also wasted and needs replacement.

Each accommodation space must be fitted with a manual alarm system that meets requirements in 46CFR76. The activation of any manual pull station must cause an audible and visual fire detection alarm signal at the control panel. None of the pull stations activated an audible alarm in the pilothouse. Vessel to prove proper operation of fire detection panel and pull stations. Vessel to maintain at least one deckhand on each passenger deck during operations and ensure constant communication with pilothouse in event of fire.

Where subject to moisture, a storage cylinder must be installed to provide a space of at least 2 inches between the deck and the bottom of the storage cylinder. All four CO2 bottles were set directly on the deck with no space underneath.

Fuel lines must be protected from mechanical injury and effectively secured against excessive movement and vibration. The 1" fuel injector suction lines on all four main engines were worn. Replace wasted lines and install protection to prevent future occurrences.

Electrical equipment must be maintained to protect crew. Marine inspector noticed at least 1" of fluid in the port forward battery box. Vessel to remove fluid and determine cause of fluid in box.

Nonmetallic flexible hoses must be in accordance with SAE J1942. The Parker 421 hoses that are installed are overbuilt for the system parameters, however it does not meet the specifications required as per SAE J1942 testing requirements. All Parker 421 fuel hoses in the engine room shall be replaced with a USCG approved marine-use hose as per SAE J1942-1.

Doors in A-Class fire control boundaries must meet the following additional requirements: A door must not have vent grilles or louvers; The oil-fired furnace located on the port side, aft on the main deck has vents located on the lower portion of the fire door.***RESTRICTED FROM OPERATING OIL-FIRED FURNACE UNTIL CLASS A' FIRE DOOR IS COMPLETED.*** 31 OCT 2020

Fire dampers must be capable of manual operation from outside the space served, be fitted with an indicator showing whether the damper is open or closed, and be marked with red letters of at least (0.5inches) in height stating ‘‘VENTILATION FIRE DAMPER’’ Two of the three fire dampers located on the port side engine room failed to close completely. 46 CFR 118.610

Controls and valves for operation of fixed gas fire extinguishing system must be: (i) Located outside the space protected by the system; and (ii) Not located in a space that might be inaccessible in the event of fire in the space protected by the system. Fixed CO2 system engine shutdown pressure control switches are located inside of the space protected by the vessel fixed fire extinguishing system.

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. The plan does not have the hazard classifications for each of the spaces identified, the oil-fired furnace is not properly identified as a A60 fire-boundary, the emergency equipment (IBA's) are not identified and escape routes are not marked.

Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. During the DVTP testing of the vessel's propulsion micro processors the "override" feature after "locking" the controls failed.

At least one ring buoy must be fitted with a floating waterlight, unless the vessel is limited to daytime operation, in that case no floating waterlight is required. The floating waterlight located on the starboard, second deck, aft side housing is cracked into the battery compartment.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld]/stress cracks found in the stbd overhead jet room. Determine extent of material defect and crop and renew to good metal as original on overhead of the stbd jet room to the satisfaction of attending marine inspector 705: Vessels is restricted to a Lakes, bays and sounds route in Boston Harbor and adjacent waters defined as: Inside of a line drawn between Deer Island and Point Allerton. This restriction will remain in effect until the completion of required repairs.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] extent of material defect and submit repair proposal to Marine Inspector for all identified crack on the port and stbd jet room overheads.

A hatch exposed to the weather must be watertight. Hatch gaskets to the stbd jet room and stbd engine room are missing or damaged. Replace gaskets on stbd jet room and stbd engine room hatches.

An exposed hazard, such as gears or rotating machinery, must be protected by a cover, guard, or rail. Generator alternator in the stbd engine room missing pulley guard. Provide guard for alternator pulley.

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. Vessels Fire control plan missing extinguishers and fire hose locations. Update Fire control plan with all required information.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Open electrical enclosures with current carrying wires found in stbd engine room on the port bulkhead forward and on the stbd bulkhead aft. Vessel rep stated that boxes were for previously removed engine control panels. Provide covers for open electrical enclosures.

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of §111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. Numerous portable cords found permanently installed and daisy chained underneath the main control console at the vessels operating station. Remove portable cords from location.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached. Port side IBA was not properly stowed with the painter attached to the float free link. Attach painter as required.

Vessel must comply with the structural design requirements. Port and STBD jet rooms have cracks in the overhead.

Cable and wire for power and lighting circuits must meet requirements in 46 CFR 111.60. Port and STBD engine rooms have extension cords in use for the security cameras.

When a flexible fuel fill pipe section is necessary, the metallic sections of the fill pipe must be joined by a conductor. Port and STBD fuel fill pipes are not bonded.

Vsl is broadcasting invalid IMO # and no static draft information

Prove proper operation of E/R ventilation fire dampers both port/STBD.

Provide proof of servicing for pre-engineered F/F system.

Mount portable extinguishers- 2 on second deck/1 on main deck.

All of the vessel's inflatable buoyant apparatus have been removed for annual servicing. The vessel is limited to operations on Lakes, Bays & Sounds not more than one nautical mile from shore until all IBA have been re-installed to the satisfaction of an attending marine inspector.

ST01- Bilge strainer in STBD void #3 found wasted. Replace strainer.

THE VESSEL'S INFLATABLE BUOYANT APPARATUS HAVE BEEN REMOVED FOR ANNUAL SERVICING. THE VESSEL'S ROUTE IS LIMITED TO LAKES, BAYS & SOUNDS AND NOT MORE THAN ONE NAUTICAL MILE FROM SHORE UNTIL THE IBA HAVE BEEN RE-INSTALLED TO THE SATISFACTION OF AN ATTENDING MARINE INSPECTOR.

Clean clogged fuel oil vent flame screens, port and stbd side.

Repair/renew wasted electrical stuffing tubes at top of #2 void.

Replace broken fan cover on back side of fir/bilge pump motor.

Repair sea water leak at hose clamp on top of starboard generator.

Make operable starboard engine room, center fire damper.

Make operable, CO2 pressure switch that shuts down port generator when system is activated.

Replace the two worn out aft life rings.

Prior to carring passenger beyond 20 miles from a safe harbor provode and install lights on all of the life jackets.

Insulate fuel oil supply piping bracket in tank compartment.

Renew fire pump pressure gauge in starboard pontoon engine room.

Repair transmission lube oil heat exchanger leaks for the aft main engine in the port pontoon.

Repair fuel oil leak (coupling) for port fuel tank.

Repair fuel leak to starboard generator supply piping.

Renew deck cable penetrations in the # 3 void space starboard.

Prior to the completion of the next credit dry dock examination, upon receiving a stamped approved repair proposal from the local OCMI, crop out temporary repaired PORT bow area and renew following approved plans. Follow all local OCMI repair inspection sequences.

Re-locate center inflatable life raft up for crew deployment as discussed.
