Resolution: Received, reviewed and accepted class report from [name withheld] stating the deficiency was rectified. Teflon package was damaged and replaced immediately. During class survey the valve was visually inspected and quick closing device was tested.

Resolution: Received, reviewed and accepted class report LLoyd Register stating the deficiency was rectified. Crew replaced O ring on Cylinder NO.4 Fuel Oil pump connection pipe Aux engine no.1 Port. Fuel pump connections were visually inspected by class, no leaks.
