If a vessel operates on exposed or partially protected waters, an opening port light is not permitted below the weather deck unless it has a hinged dead cover. There is one portlight in the pump room, port side, and one port light in the engine compartment, port side missing dead cover. Replace the same.

Stern light is not operational.

Starboard side aft seating area in the overhead lighting circuit junction box, conductors are pulling out make repairs to satifaction of OCMI.

Bridge power panel double pole circuit breakers 24/26 and 23/25 need loads labeled.

Main distribution panel double pole circuit breaker 25/27 needs to be a 30 amp breaker vice 50 amp breaker per MSC approved plans on or before 13MAY2024.

Emergency proceedures and Emergency broadcast proceedures are not posted. Post proceedures.

Ring Buoy waterlight is cracked. replace waterlight.

Provide record of MIS report submission 2023.

Child size life jacket stowage area needs to be marked with quantity of jackets.

Port and Starboard propulsion engines dry exhaust needs lagging from the water cooled engine manifold aft.

Fire Extinguishers in Galley and Pump room need approved capture brackets installed.

Repairs to machinery that affect the safety of the vesssel must not be made without the approval of the cognizant OCMI. The vessel suffered a loss of propulsion on 28Sep23. Determine the cause of teh casualty, make repairs, and conduct a sea-trial in the presence of a Coast Guard Marine Inspector.

Repairs or Alterations the effect the safety of the vessel must be approved by the OCMI including alterations to the electrical system. During the course of the vessels annual inspection Marine Inspectors identified several heaters and heat trace tape installed. Vessels operator must revise and submit electrical one line to MSC for approval. All loads not noted on the vessels approved plans must remain disconnected until installation is approved.

Every vessel must be equipped with a suitable magentic compass at the operating station. Provide a magnetic compass.

Properly support electrical wires with metal supports every 24".

Properly install heat tape throughout vessel to manufacturers instructions, including GFCI outlets.

The vessel is required to operate w/ 1-Master and 2-Deckhands at all times when passengers are onboard until it can be proven that a man overboard can be recovered with only 2 crew members.

The hailing port of the vessel must be clearly visible on exterior part of the stern of the vessel.

Both fire stations do not have a spanner wrench provided for 1.5 inch diameter fire hose. Provide a spanner wrench at each fire station.

There is no evidence that new pressure vessel system is designed and constructed as per requirements in Section VIII of the ASME and must be stamped with the ASME "U" symbol. Class II and III pressure vessels with a net internal volume of less than 5 cubic feet and don't carry hazmat material must be stamped either "U" or "UM". Provide evidence that current system meets ASME standard or replace system with approved one through local OCMI.

Electrical shore power feed is 60 amps, 3 phase 208, with a neutral (208/120 VAC). The engine rooms existing electrical shore power tie selector switch a 3 pole; with shore power neutral electrically connected directly to the 3 phase 208 volt generator neutral wire. Confirmation with USCG MSC electrical engineer ([name withheld]) concurs with CG MI on a small passenger vessels the shore tie neutral wire must connected and disconnected when on shore power. IAW Old T 46 CFR 183.01-15(b), to prevent potential current feedback from the shore tie neutral wire, system must be redesigned to incorporate a four pole selector switch, whereby making and breaking all hots and neutral shore power wires, with 4 pole disconnect switch rated at that of the generator.

Observed neutral wire is absent in the electrical 208/120 VAC, 60 amp shore tie, shore power breaker box. Observed bare ground conductor connected to the 3 pole breaker, neutral bar, whereby is not compliant with the National Electrical Code. IAW Owner/operator contact a shore side electrician or electrical firm to consult and to install a property sized neutral wire to the shore side 60 amp breaker box and to the vessels shore side electrical shore tie receptacle. IAW Old T 46 CFR 183.01-15(b)

Vessel lacks a switchboard, observed one panel (120 VAC) stbd side engine room and 3 phase/208 power supplying the steering motor & fire pump. Wires supplying power to each piece of equipment are undersized. IAW Old T 46 CFR 183.10-35, remove and install electrical wiring rated that of the generators current and or reconfigure 120 VAC power to be supplied from the pilot house panel.

Prior to making electrical shore power modifications, owner/operator shall submit revised electrical plans and or one line electrical drawings, with equipment details to the USCG Marine Safety Center for review and approval. When approved modifications shall be made and verified to the satisfaction of an attending CG Marine Inspector.

Flange for removed fire monitor is warped and leaking. Provide watertight seal on the forward fire monitor stanchion.

The employee supervisor has not completed the Employee Assistance Program one hour of training per 46 CFR 16.401. Company rep to complete the required training and submit documentation reflecting completion of the required training to attending MI.

Observed several expired items in the first aid kit. Renew all expired items per 46 CFR 184.710

Observed lifesaving donning placards for the SAFEGAURD Type I life Jackets were not posted in the main cabin and stbd outside life jacket locker. Obtain and post a SAFEGUARD donning placards in the main cabin & in way of the stbd life jacket locker per 46 CFR 185.520

Observed a 15lb CO2 Fire Extinguisher in the engine room, port side was not mounted in an approved holder. Install and mount the extinguisher in approved bracket per 46CFR181.520

Observed return fuel oil hose on the generator was rubbing against fixed piping. To protect from wear, install chaffing gear on the generators fuel oil per 46CFR182.720

Observed the main electrical cable was loose and detached from main electrical switchboard junction box. Properly terminate electrical cable, per 46CFR183.340

Observe excessive oil residue in the ship service generator. Clean the generator per 46CFR183.220

Observed protection cover missing from generator flywheel. Install the protective guard per 46CFR183.220

Observed several openings (small holes) in the fwd engine room and fwd pump room bulkheads. Properly seal all openings per 46CFR171.114

During the man over board drill, observed the ladder slip. Modify the man over board ladder to prevent slippage & to the satisfaction of attending CG MI per 46CFR185.520
