74SOLAS(97) II-2/18.5 / 10c / 07199 All waste-receptacles shall be constructed of non-combustible materials with no opening in the sides or bottom. PSCO observed combustibles being disposed of in a combustible waste-receptacle in the galley.

74SOLAS(04) II-1/15.7.1.4 / 10c / 03107 Each power-operated sliding watertight door, shall be provided with an individual hand operated mechanism. It shall be possible to open and close the door by hand at the door itself from either side. PSCO observed manual mode not operational.

07105 - Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures shall close no more than 40 seconds and no less than 10 seconds from beginning of their movement. The approximate uniform rate of closure of sliding doors shall be no more than 0.2 m/s and no less than 0.1 m/s with the ship in upright position. PSCO observed the following hinge doors did not meet the time requirement: 4-2-026, 4-2-024, 4-2-017, 4-2-002, 4-3-034, and 4-3-029. The following sliding doors did not meet the time requirement: 12-3-004, 12-6-051, 12-7-055, 12-7-060, 11-7-057, 11-5-048, and 11-3-038. 74 SOLAS (04) II-2 / 9.4.1.1.4.2 40AC

04109 - Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fighting systems and appliances that may be called upon to use. PSCO observed 03 galley cooks were unfamiliar with the operation of the water mist system for the deep fat fryers in main galley, crew galley and specialty restaurant. 74 SOLAS (20) II-2 / 15.2.2.1 40 AC

07199 - All waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed numerous combustible type waste receptacles in the crew lounge on deck 5 port side. 74 SOLAS (04) II-2 / 18.5 10c

07103 - Storage of combustibles is prohibited in category 10 and 13 spaces. Flammable liquids were found in the following category 13 spaces: 01-2-012. Flammable were found in the following category 10 spaces: 06-01-003, 05-05 (AC room), and 04-04 (AC room). 74 SOLAS (04) II-2 / 9.2.2.3.2 10c

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO noticed a wasted salt water cooling pipe leading to the main engines. SOLAS 20 1/11

Survial craft muster and embarcation arrangements, muster and embarkation stations shall ve illuminated. PSCO observed 03 embarkation lights on STBD side to be inoperative. SOLAS 97 III/11.4

Openings in "A" class divisions. Fire doors shall close no faster than 10 seconds upon activation. PSCO observed 02 fire screen doors closing in less than 5 seconds. SOLAS 01 II/30.4.1

Openings in "A" class divisions, doors shall be self closing. PSCO observed fire screen door (A60) 12-5-036 did not close completely. SOLAS 01 II-2/30.4.1

The number of openings in watertight bulkheads shall be reduced to the minimum compatible with the design and proper working of the ship; satisfactory means shall be provided for closing these openings. PSCO observed watertight door 01-04-01 did not close utilizing the local hand pump.

The company and the ship shall comply with the requirements of the International Safety Management Code. PSCO noticed that crew was using a "dirty water" bottle to test OCM. SMS states that a bottle brush will be used to test system. SOLAS 20 IX/3.1

74 SOLAS 2014(CONS) II-2/10.6.1.1. Passenger ships carrying more than 36 passengers shall be equippedwith an automatic sprinkler system complying with the requirements of FSS code in all accomodation and service spaces. 02 sprinkler heads on deck 05 crew smoking area were painted over, 01 sprinkler on deck 06 main galley dislodged, and 01 sprinkler on deck 07 "New Bar" port side dislodged.

74 SOLAS 14(Cons) II-2/13.3.2.5.1. Means of escape, including stairways and exits, shall be marked by lightin or photoluminescent strip indicators at all points of the excape route. Low level lighting on deck 09 port aft passenger accomodation corridor and deck 11-13, port aft stair inoperable.

46 USC 3507(a)(1)(b). Each passenger stateroom shall be equipped with entry doors that include peepholes or other means of visual identification. Lenses were missing for 03 passenger rooms on deck 10.

The company should establish procedures, plans, instructions, including checklists for key shipboard operations concerning the safety of the personnel and ship. Identified non-conformity in vessels SMS in that numerous PFD lights were found inoperable despite the reported completion of numerous monthly checks. Recommend audit.

A life jacket complying with the requirements of 2.2.1 or 2.2.2 of the code shall be provided for every person on the ship. Numerous PFD's were found with inoperable and recalled lights.

"A" class divisions shall be so constructed as to be capable of preventing the passage of smoke and flame. Nine doors in the seven primary escape trunks from each machinery space main vertical zone would not prevent the passage of smoke and flame due to penetrations, misadjusted hose ports, and misaligned door hinges. 74SOLAS(97) II-2/3.3.3 17C 10C

Markings for means of escape shall identify all the routes and readily identify escape exits. It was noted that the door next to FST 5-6-44 in the staff Messroom had been permanently shut thus preventing passage through it. In addition, there was an emergency exit sign installed above it. 74SOLAS(97) II-2/28.1.10 16ac

0750- Fire Prevention- 10c Where the contents and use of a space are such that there is a doubt as to its classification, it shall be treated as a space having the most stringent boundary requirements. A flammable liquid locker was found in a Category 3 corridor outside the engine workwhop on Deck 3. 74 SOLAS (97) II-2/26.2.2

2035- Fire Control Plan- 50ac Fire control plans shall be kept up-to-date; any alterations shall be recorded as soon as practicable. Storage rooms (Cat 13) ST 10-4-030 and ST 09-7-063 have been converted to accommodation spaces, but the fire control plan has not been updated. 74 SOLAS (14) II-2/15.2.4.1

0920- Safety Plan- 10c Stairways and ladders shall be arranged to provide ready means of escape from spaces in which the crew is normally employed. Escape from the Technical Office, door B-04-5-057 can only open partially due to a large printer installed. 74SOLAS(97) II-2/28.1

0750- Fire Prevention- 10c Construction of all doors in A class divisions shall have a means of securing them when closed. FSD 04-7-144, 03-5-044, 4-5-042, 5-7-077, and 5-4-048 did not latch when closed. 74SOLAS(97) II-2/30.2

Before the ship leaves port, and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Diesel fuel supply hose on lifeboats No.26 and No.28 had the outer rubber protective layer missing in places with severe corrosion on layer beneath. 74 SOLAS (2014) III/20.2

1420: Means shall be provided to control leaks of flammable liquids. Aft outboard of port engine in fwd E/R compartment #14 has accummulation of oil in way of outboard side of engine. Oilt is all over cables, pumps, pipes, and insulation. 74SOLAS14 II-2/4.1.1

0715-Detection-Fire detection system and equipment shall be designed to withstand ambient temp changes, vibration,humidity,shock,impact and corrosion. The PSCO observed 5 faults on the fire alarm panel. Ships crew stated cause of faults were due to humid conditions. 74SOLASII-2/13.3.1 50a

0676 - The public address system shall be clearly audible above the ambient noise in all spaces and shall be provided with an override function from one location on the navigation bridge. The public address system is inaudible in the arcade due to background noise. 40c SOLAS II-2/5.2

0750 - Fire doors in main vertical zone bulkheads shall be self-closing. FSD 04-6-079, FSD 12-05-036, FSD 0-13-050 and FSD 13-05-014 failed to fully close and latch when tested. 40c SOLAS II-2/30.4.1

0960 - Escape routes shall be maintained in a safe condition, clear of any obstacles. A piano is being stored and used for practice in a corridor (cat. 3 space) backstage of the theatre on deck 5. 40c SOLAS II-2/13.1.2

0715 - Any required fixed fire detection and alarm system with manually operated call points shall be capable of immediate operation at all times. A manual call point was inoperable. 10c SOLAS II-2/13.1.1

0950 - Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards shall be ensured. Galley deck 6 circuit breaker panel had approximately 6 slots with no breakers and no banks. 40c SOLAS II-2/40.1.3

0750 - Means shall be provided to control leaks of flammable liquids. Fuel was observed leaking from DG3. 10c SOLAS II-2/4.1.1

0610 - Lifeboats - Passenger ships engaged on internation voyages shall carry partially or totally enclosed lifeboats. The engine shall be provided with two independent rechargeable energy sources. PSCO observed 07 batteries on 06 different lifeboats were not capable of starting the lifeboat engines. SOLAS III/21.1.1.1; LSA 4.4.6.2

0910 - Each power-operated sliding door watertight door: shall be provided with an individual hand operated mechanism. It shall be possible to open and close the door by hand at the door itself from either side. Watertight doors # 01-05-01, 01-05-01, and 01-05-03 were not able to close using the hand operated mechanism at the door. SOLAS 74 (97 amend.) II-1/15.7.1.4

0745 - Exhaust ducts from galley ranges where grease or fat is likely to accumuluate shall be fitted with a damper located in the lower end of the duct. The fire shutter failed to close when activated remotely. Until repaired the bimini grill may not be used. SOLAS 74 (97 amend.) II-2/41.2.4.3.2

0945-SAFETY IN GENERAL: MUSTER AND EMBARKATION STATION SHALL BE ADEQUATELY ILLUMINATED BY LIGHTING SUPPLIED FROM THE EMERGENCY SOURCE OF POWER. NUMREOUS EMBARKATION LIGHTING BOTH INBOARD (P) AND OUTBOARD (S) locations ARE INOPERABLE, POSING A SAFETY HAZARD TO CREW AND PASSENGERS. SOLAS 1997 III/11.4

0610-LIFE SAVING APPLIANCES: BEFORE THE SHIP LEAVES PORT AND AT ALL TIMES DURING THE VOYAGE , ALL THE LIFE SAVING APPLIANCES SHALL BE IN WORKING ORDER AND READY FOR IMMEDIATE USE. #10 LIFEBOAT FWD HOOK RELEASE REQUIRES "READJUSTING" IN ORDER TO RESET THE PINS TO RETRIEVE THE LIFEBOAT. SOLAS 1997 III/19.2

Class A boundry has unapproved penetration of a bolt holding a clothing line in the female dressing room at FSD 06-1-122.

Install steel blank IWO A-60 bulkhead adjacent FSD 02-5-054. Vessels crew removed piping for sink drain, which penetrated the A60 bulkhead...they never intallad steel plate to cover hole which penetrated the bulkhead.

Prior to departure Class is to examine the condition of the Marine Evacuation System (MES) tension lines for the starboard side. Tension lines appear to have been hit near their lowest end. Ship to provide a Class report from DNV prior to departure.

Remove items from Telephone Electrical room (Deck 6, fwd, stbd side)

Remove/reconfigure stowage of carpets, etc., in carpet store room (04/D03) deck 04, as per height limit markings in the space.

Demonstrate proficiency of certified life boat commanders in use and knowledge of emergency equipment and supplies.
