A vessel with more than 49 pax and not more than 65 ft must have a fixed power pump and portable hand pump. Vessel did not have required pumps IAW table 182.520(a) and 182.520(j). Vessel is restricted from the carriage of more than 49 pax.

A self priming, power driven fire pump must be installed on each vessel of not more than 65 ft in length that carries more than 49 pax. Vessel did not have power driven fire pump and is restricted from the carriage of more than 49 pax.

All cable and wire must be installed with supports spaced nor more than 24 inches apart and in such a manner as to avoid chafing and other damage. Install supports for wires.

Electrical equipment on a vessel must be installed and maintained to provide services necessary and protect personnel from hazards. Dead end wires in engine room must be addressed.

Standard rigging on a sailing vessel must be suitable for the intended service. Pins must have suitable rings to prevent them from falling out.

Penetrations and openings in watertight bulkheads must have means to make them watertight. P/S side fwd bulkhead must be made watertight.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. P/S side engine room wire penetrations must be sealed.

Gauges to indicate jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Vessel was not equipped with gauges at operating station.

The owner or managing operator or a qualified servicing facility shall conduct inspections and tests as applicable for fixed gas fire extinguishing systems and must provide evidence of the required servicing to the marine inspector. Vessel's fixed gas fire extinguishing system has not been inspected or tested since MAY 2023. Provide satisfactory evidence of required inspections, maintenance procedures, and tests by a qualified servicing facility. Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. 46CFR176.810(b) 46CFR176.810(b)(2)

The OCMI may require the vessel to get underway. Get underway in presence of marine inspector to conduct drills for COI completion. 46CFR176.404(a)

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. Vessel's hand orange smoke distress signals are expired. Vessel has seven red hand flare distress signals and is required to have 5 hand orange smoke signals in order to meet the regulation. Vessel restricted to a Lakes/Bays/Sounds route or 30 min runs. 46CFR180.68

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI. Vessel is missing a fire bucket and none are labeled. 46CFR181.610

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Labeling of life jacket stowage is missing letters and numbers. Provide proper labeling on hatches for stowage of life jackets. 46CFR18078(a)(5)

On a nonmetallic vessel, the enclosures and frames of electrical equipment must be bonded together to a common ground by a normally non-current carrying conductor. Bonding conductor for stbd engine room and stbd steering gear room is disconnected. 46 CFR 183.372(a)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Stbd engine room seawater hose is collapsed and the port engine room sea water hose is chaffed. Replace the stbd engine room hose and/either replace or install chaffing gears on port hose. 46CFR183.100

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessel's chain plates have been removed for maintenance; Repair of mast and hull required prior to re-installation. 46CFR176.802(3)

Each officer employed on any vessel subject to this subchapter must have his or her license or merchant mariner credential onboard and available for examination at all times when the vessel is operating. Master did not have MMC; therefore could not get vessel underway to conduct sea trials. Master to schedule sea trials with OCMI with valid MMC onboard.

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and general description of the drill scenario and training topics. Vessel unable to provide logs for fire fighting drills and training. Vessel rep to submit proof of drill in log

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and scenario and training topics. Vessel unable to provide drill/training logs. Vessel rep to submit proof of drills in log.

An adult life jacket must be provided for each person carried on board a vessel. 33 life jackets were cracked/ripped and in unserviceable condition. Replace 33 adult life jackets to serviceable condition. Reduced passenger count to 17 plus 3 crew, 20 total adults.

Child size life jackets equal to 10% of the number of persons permitted on board must be provided. 01 Child size life jacket unserviceable. Replace the life jacket to serviceable condition. Reduced child passenger by 01, 05 total child size permitted.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life ring buoy is waterlogged and missing the plug. Replaced on scene, cleared deficiency.

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. Vessel is missing 02 red flares. Replaced after inspection, cleared deficiency.

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 and Flexible nonmetallic hose must be complete with factory-assembled end fittings requiring no further adjustment of the fittings on the hose, or field attachable type fittings may be used. Hose end fittings must comply with SAE J-1475. Flexible nonmetallic hoses are not SAE J-1942 from Racor fuel filter and fuel return line back to tank for both engine rooms.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First aid kit is missing aspirin and iodine swabs, eye wash expired.

The space containing propulsion machinery must be equipped with a fixed gas fire extinguishing system in compliance with 181.410 of this subpart. The vessel's starboard engineering fixed gas fire extinguishing system in inoperable.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first-aid kid was missing ammonia inhalants. Replace ammonia inhalants in first-aid kit.

Nothing in this subpart limits the marine inspector from conducting such tests or inspections he or she deems necessary to be assured of the vessel's seaworthiness. Identified areas of corrosion around head stay stem fitting. Provide third party rigging survey for this identified area prior to operating under sail. 46 CFR 176.500 (b) (1) (iv)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Vessel missing light fixture cover on overhead light in port midships compartment. 46 CFR 183.200(b)

The owner shall be prepared to conduct tests and have the vessel ready for inspection onf the following: On a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessel does not have current inventory list in accordance with USCG NVIC 02-16. 46 CFRE 176.802(a)(3)

An adult life jacket must be provided for each person on board a vessel. Identified 07 PFDs with insufficient snap hook spring clip tension allowing the snap hook to disengage from the D-ring of the PFD's chest strap. Renew snap hooks or replace identified PFDs. Vessel is restricted to carrying not more 47 total persons on board.

On a vessel of at least 26 feet in length, a visual and audible indicator must be provided at the operating station indicate high water level in each normally unmanned space with a through-hull fitting, machinery space, shaft alley, space with non-watertight closure and other spaces subject to flooding. Each hull has 3 spaces that are required to have alarms. There is only one visual indicator for each hull. Install and demonstrate operation of audible and visual indicators for each space required.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Stbd engine room indicator is inop. Repair visual indicator and prove proper operation.

A vessel must comply with applicable requirements for any radio installations as set forth in 47CFR80. VHF-DSC shall be programmed with MMSI and connected to or integrated with GPS.

When a flexible fill pipe on a fuel tank is a non conductor of electricity, the metallic sections of the fill pipe separated thereby must be joined with a conductor for protection against generation of a static charge when filling when fuel. Properly bond fuel fill.

After the rig is re-installed and statically tuned, a Coast Guard Marine Inspector shall attend for operational tests and sea trials prior to the vessel carrying passengers.

Provide original COD bearing valid endorsement.

Add new retro reflective material to 02 adult life jackets and 02 child life jackets

Repair/Replace visual indicator at the operating station for the port side high water alarms.

Repair/Replace high water alarm float switch for the starboard engine compartment.

Metal dead light cover was not watertight. Vessel o/o requested a 30 day extension to fabricate the dead light prototype he created.

The furler jib sail or foresail was not installed at the time of the inspection. The vessel is prohibited from sailing with a furler jib sail or foresail until sea trails have been completed to the satisfaction of the attending inspector.

Install metal deadlight covers IAW 46CFR 175.400 and 46CFR 179.350(b), or request and receive OCMI special consideration to have no port light covers IAW 46CFR 175.550.

Replace "overflow hose" on steering hydraulic rams with hose that meets 46CFR 182.720(e) or request and receive OCMI special consideration to be allowed to use the non SAE J1942 rated hose which is currently installed.

Hose test all watertight hatches.

Provide means to inspect voids forward of the collision bulkhead in both hulls.
