At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. PSS shaft seal was excessively leaking water when shaft was rotating while underway and at clutch ahead. Stop leakage of shaft seal to the satisfaction of attending marine inspector. 46 CFR 176.830(a)

A hatch exposed to the weather must be watertight. Hatch covering lazarette has section of gasket missing and drainage holes drilled into the hatch flange allowing water to drain down into the lazarette. Make hatch watertight to the satisfaction of the attending marine inspector before September 23, 2025. 46 CFR 179.360(a)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel EPIRB registration has the incorrect beacon identification number. Provide a valid EPIRB registration with the correct beacon identification number. If resolution is after the COI is issued, the vessel route is reduced to 3 miles until resolved to the satisfaction of the attending Marine Inspector. 46 CFR 184.502

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit missing: 1) the required 48 doses of analgesic medication (aspirin); 2)Iodine swabs (10). Provide to the satisfaction of the attending Marine Inspector 14 days after the COI is issued. 46 CFR 184.710

Equipment specifications for life floats and buoyant apparatus. The equipment required for life floats and buoyant apparatus must meet the following specifications: The light must be a floating water light approved under approval series 161.010 or other standard specified by the Commandant. The floating water light must be attached around the body of the life float or buoyant apparatus by a 3/8-inch lanyard, resistant to deterioration from ultraviolet light, and at least 18 feet in length. Floating water light lanyard not 18 feet in length. Provide correct length lanyard for floating water light. If not resolved after the COI is issued, this item must be resolved to the satisfaction of the attending Marine Inspector 14 days after COI issuance. 46 CFR 180.175(f)(4)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel's last valid drydock examination is expired. Complete a credited drydock examination to the satisfaction of the attending marine inspector prior to carriage of passengers or cargo. 46 CFR 176.600(c)(1)

Each life jacket must be marked with Type I retroreflective material. Numerous life jackets had an unapproved type of retroreflective material. Replace retroreflective material with an approved type to the satisfaction of the attending marine inspector. 46 CFR 185.604(h)

Each life float must have Type II retroreflective material. The retroreflective material on both life floats is deteriorated and needs to be replaced. Replace retroreflective material with an approved type to the satisfaction of the attending marine inspector. 46 CFR 160.027-2

Each distribution panel or switchboard must be totally enclosed and of the dead front type. The switchboard has an opening and is not totally enclosed. Enclose panel to the satisfaction of the attending marine inspector. 46 CFR 183.330(b)

A vessel not required to have a power driven fire pump must have at least three 2 1/2 gallon buckets placed so as to be easily available during an emergency. Mark the third fire bucket with the correct vessel name to the satisfaction of the attending marine inspector. 46 CFR 181.610

The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float and buoyant apparatus. This number must: Be the number of persons the device is equipped for; and not be greater than the number of persons the device is approved for as shown on its nameplate. Life floats missing proper labeling of number of person. Label life floats properly. 46 CFR 185.604(e)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release not marked with expiration date. Vessel route restricted to no more than three miles from shore until: 1) purchase receipt is provided for in service HRU is provided and expiration date marked with date of receipt; 2) new HRU is purchased and installed with expiration date marked to the satisfaction of the attending MI. 46 CFR 185.740(b)

A hatch exposed to the weather must be watertight. The mid deck FWD transverse hatch frame for the gasket fractured from vessel deck structure. Provide repair proposal for re-attaching the FWD hatch frame to vessel deck structure. Additionally, the gasket material for all of the vessel's deck hatches is wasted, missing, over compressed or sealing at joints/corners is not tight. Properly renew hatch gaskets to restore water-tightness. 46 CFR 179.360

A conductor of a control, interlock, or indicator circuit, such as a conductor for an instrument, pilot light, ground detector light, or potential transformer, must be protected by an overcurrent device. Vessel's high water alarms do not have overcurrent protection. Install fuse. 46 CFR 183.380(c)

The design of all exhaust systems must ensure minimum risk of injury to personnel. Protection must be provided in compliance with § 177.970 of this chapter at such locations where persons or equipment might come in contact with an exhaust pipe. Engine turbo was not jacketed with protective insulation. Install protective insulation on engine turbo. 46 CFR 182.430(a)

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. The aft compartment has a penetration to the engine room that is not vapor tight. The forward space containing toilet has spaces which allow vapor passage. Provide a proper seal to prevent the passage of vapors. 46 CFR 177.405(c)

Installation of nozzles, flanges or other fittings for pipe connections to the tanks must be acceptable to the cognizant OCMI. The fuel tank selector valve is wasted. Remove and replace valve. 46 CFR 182.44 (2)(iv)

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. The vessels name faded and unreadable on one paddle of each life float. Renew vessel name. 46 CFR 185.604(e)

Any time passengers are embarked on a passenger vessel, the vessel must have the crew complement required by the COI, whether the vessel is underway, at anchor, made fast to shore, or aground. However, the master may allow reduced crew for limited or special operating conditions subject to the approval of the OCMI. The vessel does not have written procedures, AED pads, and Medical grade oxygen as required by Work Instruction 31.Provide written procedures, AED pads, and medical grade oxygen. Vessel cannot reduce the COI manning under Work Instruction 31 until the requirements above are to the satisfaction of the attending MI. 46 CFR 15.515(b)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. EPIRB registration sticker as per 47 CFR 1061(f) is missing. Vessel route restricted to no more than three miles from shore until: 1)proof of EPIRBS' current registration is provided to the satisfaction of the attending MI. 46 CFR 184.502

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Aft space high water level indicating light would not illuminate. 46 CFR 182.530(a)

Each life float and buoyant apparatus must be stowed as required under this section, the float-free link required by § 180.130(a)(1) must be secured to the painter at one end and to the vessel on the other end. The life float float-free link was not attached to vessel. 46 CFR 182.530(a)

Lifeline and pendants. The lifeline and pendants must be as furnished by the manufacturer with the approved life float or buoyant apparatus. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter. Pendants on life floats were not properly secured in hanks with required light twine. 46 CFR 180.175(f)(1)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life ring buoy grab line is worn. Replace grab line. 46 CFR 176.808(b)

Vessel must be provided with satisfactory arrangement for draining any watertight compartment. Vessel was unable to demonstrate operation of the aft bilge pump. Demonstrate proper operation of the aft bilge pump in the presence of Marine Inspector. 46 CFR 182.500

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found in engine compartment. 46 CFR 183.200 (c)

Electrical equipment must be installed to protect from electrical hazards. Wire exposed at battery box terminal in engine compartment. Clean off and insulate wire. 46 CFR 183.200

Firefighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and general description of the drill scenario and training topics. 46 CFR 185.524(d)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and scenario and training topics. Vessel unable to provide drill/training logs. 46 CFR 185.520(f)

The EPIRB test required by paragraph (a) shall be logged or otherwise documented. Vessel unable to provide log for EPIRB tests. 46CFR185.728(c)

The inspection and maintenance of survival craft shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel unable to provide maintenance logs. 46 CFR 185.702(d)

Endorsements on Certificates of Documentation are valid for 1 year. Vessel had sent off COD for renewal did not have current COD onboard. 46 CFR 67.163

Except as provided in 80.13(c), stations must have an authorization granted by the Federal Communications Commission. Vessel did not have valid FCC station license. 47 CFR 80.13

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position or perform the duties and functions of a position, required by the vessel's COI. Marine employers may form or otherwise use sponsoring organizations, or may use contractors to conduct the random chemical testing programs. Vessel's O/O enrollment in chemical testing program is expired. Provide satisfactory evidence of a chemical testing program. 46 CFR 16.230(a)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG for DSC operation. 47 CFR 80.905

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following: Test of circuit breakers by manual operation. Circuit breaker for VHF radio inoperable. 47 CFR 176.806(b)

Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the "Emergency Instructions" listed in §185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Update emergency instructions to list manual activation of the Fire-boy as an response to fire in the engine compartment. 46 CFR 185.510(b)

All vessels must have operable navigation lights in accordance with the Navigation Rules. Vessel is currently unable to display navigation lights for a vessel engaged in diving operations. Navigation light attachment is broken. Provide navigation lights that are in compliance with the Navigation Rules. Vessel is restricted to daytime operations when conducting diving activities.

Vessel must show proof of enrollment in a chemical testing program for all crew members operating in safety-sensitive positions. Vessel did not have current documentation showing enrollment. Vessel was previously enrolled with the Maritime Consortium, but expired 30JUN2019.

Each lifejacket must be marked with Type 1 retro-reflective material. 7 adult size and 1 child size lifejacket were not properly marked. Provide new retro-reflective material on lifejackets. Vessel restricted to 12 passengers until verified by attending marine inspector.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Labeling of life jacket stowage is not separate or marked directly on the location of the life jackets. Provide proper labeling and stowage of life jackets.

Vessel required to have one 24" ring lifebuoy fitted with 60' lifeline and floating water light attached with a corrosion resistant clip to allow quick disconnect. Ring lifebouy found cracked, faded, and worn. Provide serviceable ring lifebuoy onboard vessel and properly arranged.

Each paddle required for life floats must be at least four feet in length, lashed to the life float to which it belongs and buoyant. Life float paddles are less than 4 feet and are not lashed to the life floats. Provide the appropriate number of paddles properly arranged to life floats onboard vessel.

Hatch covers must have securing devices and be attached to the hatch frame or coaming by hinges or other devices of substantial strength to prevent its loss. Hatch cover on forward starboard cabin overhead did not have proper device to secure hatch. Provide proper means to secure hatch.

Fixed gas fire extinguishing systems are required to be serviced and inspected. Vessel required to provide satisfactory evidence of the required servicing. Vessel was unable to show servicing history of the fixed fire extinguishing system installed onboard the vessel. Provide satisfactory evidence of required servicing to attending marine inspector.

A pre-engineered fixed gas fire extinguishing system installed on a vessel must automatically shut down all engines that draw intake from within the protected space. Prove proper installation and operation of pre-engineered fixed gas fire extinguishing system. Demonstrate proper operation prior to the carriage of passengers.

All engines must have at least two means for stopping the engines under any operating conditions. Provide a second mean of securing engine aside from the fuel oil shutoff. Demonstrate proper operation prior to the carriage of passengers.

Portable extinguishers must be inspected and maintained in accordance with NFPA 10. Provide two serviceable 2-A portable extinguishers prior to the carriage of passengers.

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Provide means of securing shunt/terminal block behind operating station console.

On a vessel of at least 26 feet in length, a visual and audible indicator must be provided at the operating station to indicate high water level in each normally unmanned space with a through-hull fitting, machinery space, shaft alley, space with non-watertight closure and other spaces subject to flooding. Aft visual alarm found inoperable. Install and demonstrate operation of visual indicators for each space required.

To include position information in a distress alert, the VHF-DSC radio should be properly connected to a GPS Receiver or have a VHF-DSC marine radio with integral GPS. Vessel's VHF-DSC RADIO is not properly connected to a GPS received.

A hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Provide a means of securing all four hatch covers on vessel.

A vessel on a coastwise route must carry six hand red flare distress signals and six hand orange smoke distress signals. Provide three additional hand orange smoke distress signals. Vessels route is limited to Lakes, Bays, and Sounds until corrected.

At each inspection the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Conduct satisfactory underway drills with the presence of a marine inspector.

Each paddle required for life floats must be at least four feet in length, lashed to the life float to which it belongs and be buoyant. Life float paddles are less than four feet in length and are not lashed to life floats. Provide paddles that meet the required specifications to attending Marine Inspector.

Painter must be at least 100 feet in length, have a breaking strength of at least 1500 lbs., be of a dark color if synthetic or certified to be resistant to deterioration from ultraviolet light and be stowed in a way that runs out freely when life float floats away from sinking vessel. Painter installed on vessel is less than 100 feet. Provide painter that meet the required specifications to attending Marine Inspector.

The light must be a floating water light approved under approval series 161.010 or other standard approved by Commandant. The floating water light must be attached around the body of the life float by a 3/8 inch lanyard resistant to deterioration from ultraviolet light of at least 18 feet in length. Both life floats are missing floating water lights. Provided approved floating water lights and properly install to satisfaction of attending Marine Inspector.

Each life float must be secured to the vessel by a painter with a float free link permanently attached to the vessel. The float free link required must be certified to meet 46 CFR 160.073 or other standard specified by the Commandant, of proper strength for the size of the life float as indicated on its identification tag, and secured to the painter at one end and to the vessel on the other. Weak link is missing tag. Provide weak link that meets the required specification to attending Marine Inspector.

Vessel has not completed credit drydock. 835 issued until credit drydock is given

Seal all machinery space penetrations in forward bulkhead IAW 46CFR177.405 (c)

Provide ventilation closures for machinery space IAW 46CFR 182.465 (h)

Provide written policy and emergency response procedures IAW work instruction 31 to maintain reduced manning capabilities.
