A ready means of escape shall be provided to the open deck and thence to the lifeboats and liferafts, above the lowest open deck the means of escape shall be stairs or a door to an open deck. The access doors to the Bosun stowage locker were found secured from the outside and prevented escape from the area in the event of a fire or other incidents. Deficiency rectified on the spot.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Marine Inspectors observed two ventilation fans whose electrical supply conductors were spliced using electrical tape into a single conductor and then plugged into a single electrical outlet causing an explosion hazard. These fans were ventilating fuel tanks that had not been gas freed.
