During the Expanded MARPOL exam, master stated the vessel experienced flooding in the engine room on or about the 25 August 2019, while at sea. It was reported C/E utilized the vessels emergency bilge suction system to discharge oily mixtures from bilges to the sea. This incident was not recorded in the ORB or any of the vessels other official logs.

A ship when in a port or an offshore terminal of another party is subject to inspection by officers duly authorized by such party concerning operational requirements under this Annex, where there are clear grounds for believing that the master or crew are not familiar with essential shipboard procedures relating to the prevention of pollution by oil. During an expanded MARPOL Exam, PSCO discovered debris in the starboard emergency bilge suction strainer, indicating discharge of oily water from the bilges. Upon further findings, Captain stated (August 25, 2019)he was told flooding from the ships water cooling pump filled the bilges up to the deck plates. The ships installed emergency bilge pump was used to discharge oily water to the sea. During review of the ships logs, there was no indication of oily water discharge officially recorded.

PSCO believes the vessels treatment system and associated piping is contaminated as a result of unapproved oil discharges into the system. Ships records did not align with PSCO's findings.

PSCO noted the lower engine room void trunks port and starboard sides were not made tight. Loose missing hardware and gaskets were noted, which left the vessel at risk of progressive flooding, fire, in the event of damage or mishap. Further, PSCO noted several hand rails in the engine room that were missing hardware causing hazardous sway and or creating danger to crew especially at upper levels, platforms and near ladders.
