To have a Certificate of Inspection endorsed to carry NLS cargo, a ship must have a Procedures and Arrangements Manual. Barge is missing a Procedures and Arrangements Manual at time of inspection. Barge will need Manual prior to loading Subchapter "O" cargoes listed in 46 CFR.

The master, owner, operator, agent or PIC of a vessel equipped with ballast tanks that operates in waters of the United States, must employ 1 of the following methods: Alternative Management System (AMS), that meets the requirements of 33CFR151.2026 of this subpart, may also be used, so long as it was installed prior to the date that the vessel is required to comply with the Ballast Water Discharge Standard (BWDS) in accordance with 33CFR151.2035(b). Vessel conducted untreated empty refill exchange after compliance date listed in 33CFR151.2035(b). Vessel must conduct a treated ballast exchange outside of 12nm utilizing their AMS prior to discharging ballast in US waters. 29JAN25: Vessel unable to fix BWTS in Boston. Main and Auxiliary machinery shall conform to subchapter F of 46 CFR. #2 Generator did not shut down as required when low lube oil or overspeed was tripped. Vessel may not operate #2 generator until repair or replacement has been made as appropriate.

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a .5 inch diameter and is at least 1 inch long. Starboard manifold aft vapor connection, and starboard hose rack manifold connection had no permanent stud. Repair as appropriate.

Each watertight door must be designed to withstand a head of water not less than 10 feet. Door to the forward void space did not seal appropriately.

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel missed annual inspection from anniversary date. Vessel must complete inspection prior to cargo operations.

The installation of all electrical systems must meet the requirements of subchapter J (Electrical Engineering Regulations) of this chapter. Electrical equipment installed in hazardous locations should ensure that the electrical equipment is not a source of ignition. Three intrinsically safe cargo level indicators manufactured by Deck Master had faulty electrical cables that need to be replaced/repaired. 46 CFR 32.45-1 46 CFR 111.105-3(a)

Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open. MI's unable to determine relief test completed for any of the relief valves. Valves must be tested prior to cargo. 46 CFR 35.35-70(b)

30 × 30 corrosion-resistant wire mesh shall be fitted on all openings to atmosphere. Flame screens shall be made of corrosion-resistant wire. MI found the flame screens on 6 port and 2 starboard of the cargo tank P/V vales to be damaged. Must be replaced prior to cargo. 46 CFR 39.2011(b)(2)

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. Multiple vapor lines missing stud and must have repaired prior to cargo. 46 CFR 39.2001(j)

All pressure vessels other than unfired steam boilers shall be protected by pressure-relieving devices that will prevent the pressure from rising more than 10 percent above the maximum allowable working pressure, except when the excess pressure is caused by exposure to fire or other unexpected source of heat. Air reciever is missing tag on relief valve, MI's unable to determine device setting. 46 CFR 54.15-5(c)

A fixed pressure-sensing device must be installed as close as practicable to the vessel vapor connection on a tank barge with a vapor collection system. The pressure-sensing device must measure the pressure vacuum in the main vapor collection line and have a pressure indicator located where the cargo transfer is controlled. 46 CFR 29.2015

Certificates of Inspection will be issued for a period of 5 years; COI deactivated on 21JAN21 due to missed periodic inspection. Complete COI Inspection prior to conducting cargo operations.

Vessel has leaking closed gauging windows on tanks 1-7P and 2,3,4,5,7 stbd. Closed gauging was rtv'd shut. 46 CFR 151.15-10(c)

Vapor trunks from the cargo pump to the pump room had holes and were missing the band clamps making them not vapor tight. 46 CFR 111.105-31(k)(1)

Generator room has the hydraulic 440V pnl with a door that does not close. 46 CFR 111.01(a)(2)

Generator room has 110v f/o alarm panel that has the visual indicator lighting that was punched out and could allow a person access to the wiring within. 46 CFR 111.01(a)(2)

There must be ground detection for each: (b) Ship's power service system. Faulty ground detection noted on the 120V system. O/O to mitigate deficiency accordingly.

Fwd rake entrance is within haz zone. Entrance has open wire runs that impede vapor tightness.

#3S cargo tank P/V valve had plug in the place of drain.

Abandon ship drills and fire fighting drills were not properly logged. Barge Captain was using company drill schedule and record sheets, however these drills were being documented and incorporated into other drill-types, and not as separate drills and individual items on the log.

Spare hoses at manifold need blank flanges and to be tested prior to use.

120V and 240V panels in generator room are missing breaker covers.

Ground detection for 120V system in not functioning.

The gaskets on port and starboard aft rake hatches are not continuous.

Life rings not properly marked with name of vessel and hailing port.

C/R sideshell plating IWO canted frame C to anchor slide PS forepeak; approx. 8-10 ft. by 12-15 ft. section to the satisfaction of the attending CG marine inspector. Vessel is allowed to discharge all cargo, then proceed to do repair to the satisfaction of the attending CG marine inspector. Prior to each movement contact the CG Sector NY Command Center at [phone withheld].

Repair fuel leak on #2 cargo pump prime mover IWO fuel filter.

Repair oil leak on #2 cargo pump prime mover oil cooler IWO after cooler.

Remove extension cord providing power to chest freezers in the stbd pump room.

Properly stow all flam liquids, paint, and used batteries from the machinery spaces and decks.

Vessel is not to carry more than 2 persons as permitted on the COI. Contact CG Sector St Pete to de-conflict the COI manning with actual operations.

All four Vapor Control connection piping was not properly painted/color coated. 46 CFR 39.20-1

House genereator was inoperable and not ready for inspection. 46. CFR 111.12
