Tankers shall be equipped with at least one portable instrument for measuring oxygen and one for measuring flammable vapor concentrations, together with a sufficient set of spares. Both portable instruments for measuring oxygen and flammable vapor concentrations are malfunctioning and a fixed system to measure these concentrations is not installed. A U.S. Coast Guard PSCO must clear this deficiency prior to conducting cargo operations.

Resolution: Two new portable meters were brought on board. PSCO verified calibration and proper functioning of meters. Cargo tanks were randomly sampled with average oxygen of 2.6%.
