Tankers shall be equipped with at least one portable instrument for measuring oxygen and one for measuring flammable vapor concentrations, together with a sufficient set of spares. Both portable instruments for measuring oxygen and flammable vapor concentrations are malfunctioning and a fixed system to measure these concentrations is not installed. A U.S. Coast Guard PSCO must clear this deficiency prior to conducting cargo operations.

Resolution: Two new portable meters were brought on board. PSCO verified calibration and proper functioning of meters. Cargo tanks were randomly sampled with average oxygen of 2.6%.

Bridge engine order telegraph failed to keep engine in all stop after all stop command and positioning of the engine order telegraph to all stop. Class provided a written report to USCG stating that until repairs are completed, Maine Engine is to be operated in local engine room control only. The Pilot is comfortable transiting the vessel with this issue. Tugs will assist the vsl away from pier.

Resolution: The minor crack between Cargo Oil Tank 6 starboard and Water Ballast Tank 4 starboard has now been re-welded. DNV has been on board the vessel and approved the repair.
