For determining the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk as shown in categories (1) to (14) below. PSCO observed several cardboard boxes in multiple category 10 A/C spaces on deck 1. 74 SOLAS (01) II-2/26.2.2

Fire protection systems and fire-fighting systems and appliances shall be properly tested and inspected. PSCO observed inconsisten monthly portable fire extinguisher servicing records. 74 SOLAS (20) II-2/14.1.2

PSCO observed missing insulation in way of pipie penetration 74 SOLAS 01 II-2/18.1.1

PSCO observed a cooling pipe leaking and causing secondary damage to the equipment below in the main electrical panel room. 74 SOLAS 04 II-1/26.1

PSCO Observed multiple locking devices found unsecured, electrical relay box found unsecured, not using lockout, tagout on port side evaporator distribution panel.

All waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed a large plastic receptacle in AC room 801 and two large plastic receptacles in Kings Court Galley. 74 SOLAS 01 II-2/18.5.

The arrangements for the storage, distribution and utilization of oil used shall be such as to ensure the safety of the ship and persons onboard. Oil accumulation of ~1 inch was observed under auxiliary engines numbers 1,2, and 4. A catch basin to collect leaking oil was also observed under aux engine no.2. 74 SOLAS 01 II-2/15.2 See narrative for details.

Risk assessment was made after locations of leaks found by crew on #2 Generator. #2 Generator will be taken out of service until repairs are complete. Any required automatic sprinkler, fire detection and fire alarm system shall be capable of immediate operation at all times and no action by the crew shall be necessary to set it in operation. It shall be kept charged at the necessary to set it in operation. It shall be kept charged at the necessary pressure to set it in operation. It shall be kept charged at the necessary pressure and shall have provision for a continuous supply of water as required in this regulation. During test of wet pipe water mist nozzle, near fire screen door 2.3.13, crew ruptured ampule and nozzle failed to deploy water. Additional test of nearest nozzle resulted in approximately 30s delay between ampule rupturing and water deploying. 74 SOLAS 01 II-2/12.1.1

The construction of all doors and door frames in “A” class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel of other equivalent material. PSCO observed multiple A-60 fire screen doors with locking mechanisms removed, leaving an opening in the door. Additionally, self-closing A-60 fire screen door from incinerator space to main corridor is fitted with a ramp that prevents the door from closing, unless removed by crew. 74 SOLAS 01 II-2/30.2

All “A” class doors located in stairways, public spaces and main vertical zone bulkheads in escape routes shall be equipped with a self-closing hose port of material , construction and fire resistance which is equivalent to the door into which it is fitted, and shall be a 150 mm square opening with the door closed and shall be inset into the lower edge of the door, opposite the door hinges or, in the case of sliding doors, nearest the opening. PSCO observed fire screen doors 1.4.1 and 1.8.1, in main vertical zone, which are indicated as escape routes, and are not fitted with hose ports. 74 SOLAS 01 II-2/30.7

Electrical installations shall be such that electrical services for safety will be ensured under various emergency conditions. Multiple main ship service and emergency lighting fixtures found inoperable on the vessels outer decks. 74 SOLAS 04 II-1/40.1.2

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces at least one of which shall give access to a stairway forming a vertical escape. PSCO observed 02 emergency escape doors seized to the point of being inoperable. 74 SOLAS 01 II-2/28.1.2

The construction of all doors in "A" class divisions, with means of securing them when closed, shall provide resistance to fire as well as the passage of smoke and flame. The following fire doors did not close properly due to warping/seizing or were missing latching mechanisms compromising their ability to contain smoke/flam: 7.9.03, 7.9.06, 1.6.03, 9.5.9, and 12.7.6. 74SOLAS(01) II-2/30.1

Doors and door frames in "B" class division and means of securing them shall provide method of closure which shall have resistance to fire equivalent to that o the divisions. The following fire doors did not close properly or were missing latching mechanism compromising their ability to contain smoke/fire: 4.6.3, door near ventilation shutdown 12.7.6, B-15 door port side next to pool entrance, and B-15 door near golf simulator room.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship is in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. the glass doors in escape routes leading to embarkation areas on both port and starboard sides Deck 7 had securing mechanisms seized, inoperable, and/or wasted which prevented the doors from opening. 74SOLAS(01)II-2/28.10

The means of escape... shall be marked by lighting or photoluminescent strip indicators... at all points of the escape route. Low location lighting in area near FD 11.8.6 was extinguished. 74SOLAS(01) II-2/28.10

The lines and letters shall be painted in white or yellow on a dark background. The deck line was painted blue, the same color as the hull. ILLC ANNEX I/8

Means of escape shall be provided with emergency lighting. Low location lighting did not illuminate on deck 11 in corridor near fire door 11.8.2 to exterior deck. 74SOLAS(01) II-2/28.1.5

The minimum for integirty of al lbulkheads and decks shall be as prescribed in tables 26.1 and 26.2. The following fire doors did not close all the way, did not operate as manufactures, or were missing locking mechanism, reducing smoke/fire spreading capability. Door 7.6.03, 7.6.01, 6.3.8, 1.7.33, 1.7.25, 1.4.27, 3.3.6, and fire screen door in garbage handing room (dbl leaf door). 74SOLAS(01) II-2/26

Openings in "A" class divisions shall be self closing. Rolling fire shudder blocking cooking appliances in boardwalk café does not close when activation knob. Knob fell off when used. 74SOLAS II-2/30.4.1

The minimum for integrity of all bulkheads and decks shall be as such prescribed in tables 26.1 and 26.2. Structural fire protection missing in overhead above elevator in lift room (4ft x 4ft section). 74SOLAS II-2/26

All life-saving appliances and arrangements shall comply with the applicable requirements of teh Code. Port side #2 lifeboat rudder did not respond to port/stbd movement. Stbd rudder was fixed while onboard but the engine will not shut off by normal means. 74SOLAS(01) II-2/28

Means of escape shall be free of obstruction. Hotel store room on deck 6 fwd space has multiple walkways and doors blocked with stowage. Medical corridor escape route in medical bay had multiple carts being stowed. 74SOLAS(01) II-2/28

Means of escape shall not be obstructed. means of escape in kennerl on deck 12 was blocked. 74SOLAS(01) II-2/28

Openings in "A" class divisions shall be self-closing. Rolling fire shudder door for kings court (7.4.3b) does not close with emergency closure switch. 74SOLAS(01) II-2/30

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Portable fire extinguishers that have been discharged shall be immediately recharged or replaced with an equivalent unit. 06 sprinkler heads in pool machinery space on the #6 deck had excessive corrosion build up. 01 sprinkler head in smoking lounge on deck1 had tar build up. Sprinkler head in incinerator room on deck 1 had paint covering it. 74SOLAS(01) II-2/14.2.1.2

All lifesaving appliances and arrangements shall comply with the applicable requirements of the Code. STBD side #11 lifeboat/tender port engine would not start. 74SOLAS(01) III/34

Cargo ports and other similar openings in the sides of ships below the freeboard deck shall be fitted with doors so designed as to ensure the same watertightness and structural integrity as the surrounding shell plating. Sideshell openings on luggage loading deck were allowing water to leak through seal. ILLC Annex I / Reg. 21, 16a

Firefighting systems and appliances shall be kept in good working order and readily available for immediate use. Access panel to activate hi fog system in Boardwalk Café galley was missing a handle and could not be opened. SOLAS 20 II-2/14.2.1, 16a

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: fire detection and fire alarm systems. A flame detector located near the anchor locker was cracked and duct taped together. SOLAS 20 II-2/14.2.1, 16a

Within the perimeter of stairway enclosures, only public toilets, lockers of non-combustible material providing storage for non-hazardous safety equipment, and open information counters are permitted. On Deck 7 stairwell D STBD side, there are dining tables set up within the stairway perimeter. SOLAS 20 II-2/13.2.3, 10c

Electrical installations shall be such that all electrical auxiliary services necessary for maintaining the ship in normal operational and habitable conditions will be ensured without recourse to the emergency source of electrical power. Exposed electrical wiring was present in the service space to the left of Cabin 8124. SOLAS 04 II-1/40.1.3 10c

The company and the ship shall comply with the requirements of the International Safety Management Code. The company should ensure that: any nonconformity is reported, appropriate corrective action is taken,and records of these activities are maintained. Records of portable fire extinguisher and fire hose servicing at fire stations are not being recorded per company policy. SOLAS 20 IX/3.1 16 b

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "8" class divisions together with particulars of the fire detection and fire alarm systems the sprinkler installation, the fire-extinguishing appliances, means of access to different compartments, decks, etc., and the ventilating system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. Fire control plan was not updated to reflect that the store/electrician's workshop on Deck B has been converted from two separate compartments into one and that one doorway has been rendered inoperable due to welded shelving. SOLAS 20 II-2/15.2.4.1 50a

All life saving appliances and arrangements shall comply with the applicable requirements of the code. Hydrostatic release units, if disposable, shall be marked with a means of determining its date of expiry. Two hydrostatic releases on life rafts (one on port, one on stbd) were not marked with date of expiration. SOLAS 20 III/34, 10c

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection. Fire doors 9.6.6, 7.8.1, 4.1.7, 4.8.3, B.6.10, would not close. SOLAS 20 II-2/14.2.1, 10c

Escape routes shall be maintained in a safe condition, clear of obstacles. Means of escape from the galley of the Birdwalk café located amidships on deck 13 was blocked. SOLAS 20 II-2/13.1.2 10c

Fire fighting systems and appliances shall be kept in good working order and readily available for immediate use. Section Valve 72V had excessive air in the pipe when exercised. SOLAS 20 II-2/14.2.1.2, 10c

Code: 2025 Every crew membr with assigned emergency duties shall be familiar with these duties before the voyage begins. Assigned teams failed to adequately launch the davit-launch liferaft during the abandon ship drill. Cite: 74SOLASIII/19.2.1

Code: 2099 Above the bulkhead deck there shall be at least two means of escape. The second means of escape was locked between the photo lab and golf simulator room. Cite: 74SOLASII-2/13.3.2.2

Code: 0915 Means of escape must enable passengers to identify the routes of escape. Sign was missing from secondary means of escape inside the broadcast room for Theatre Illuminations on deck three. Cite: 74SOLASII-2/13.3.2.5.2

Code: 0999 Minimum fire integrity of all bulkheads and decks shall be as prescribed in tables 9.1 and 9.2. The cover plate for void between category 10 spaces was missing by fire door 3.8-20 in Queen's Room deck 3. Cite: 74SOLASII-2/9.2.2.3.1
