During test of wet pipe water mist nozzle, near fire screen door 2.3.13, crew ruptured ampule and nozzle failed to deploy water. Additional test of nearest nozzle resulted in approximately 30s delay between ampule rupturing and water deploying.

Such doors and door frames shall be constructed of steel of other equivalent material. PSCO observed multiple A-60 fire screen doors with locking mechanisms removed, leaving an opening in the door. Additionally, self-closing A-60 fire screen door from incinerator space to main corridor is fitted with a ramp that prevents the door from closing, unless removed by crew.
