Starboard light was not operational. Make repairs to the satisfaction of the attending Coast Guard marine inspector.

Winches for vehicle ramps were not operating. Machinery and electrical systems must be designed and maintained to provide safe operations of the towing vessel and safety of persons onboard under normal and emergency conditions. Make repairs to the satisfaction of the Coast Guard marine inspector.

Navigation light on vessel was not operational. Make repairs to the satisfaction of the attending Coast Guard inspector.

Two Nav Lights extinguished.

Drills unsat, poor communication.

Life buoy water light will not light as required. A vessel must carry one floating watertight unless it is limited to daytime operations. Repair/replace watertight to the satisfaction of the attending Coast Guard Marine inspector. Vessel limited to daytime operations.

Each child size life jacket must be stowed in a location that is marked and is separated from adult life jackets. Work vest must be stowed in a separate location from life vest. Each stowage container for life jackets must not be capable of being locked. Remove work vest from life jacket locker, labeled locker appropriately and insure locker is able to open freely with out pin or lock during underway operations to the satisfaction of the attending Coast Guard Marine Inspector.

Anchor line is not stored with anchor and stored separately under equipment in a deck locker that would prevent anchoring in a timely fashion . A vessel must be fitted with ground tackle and mooring lines for the vessel to safely anchor. Stow anchor line with anchor or provide means to have anchor line readily available so the vessel can safely anchor to the satisfaction of the attending Coast Guard Marin Inspector.

Life buoy light battery was not operational.

Replace retro-reflective tape on child PFD identified

Replace line on ring life buoy identified.

Floating light attached to ring life buoy must have a lanyard of at least 3 feet no more than 6 ft.

Prove proper operation of bilge high water alarm. On a vessel of at least 26 ft in length, a visual and audible must be provided.

Replace fire extinguisher brackets with correct bracket as required by each extinguisher.

On a vessel authorized to carry one or more vehicles, suitable chains, cables, or other barriers must be installed at the end of each vehicle runway. In addition, temporary rails or equivalent protection must be installed in way of each vehicle ramp, in compliance with the following: Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers and crew and must be be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Please consult with this office before installation to ensure that it meets the intent of this regulation.

Properly install chains and gaskets for deck hatch covers as appropiate IAW 46 CFR 176.25-1 Old T.

Properly service and repair/replace damaged nozzles on portable fire ext. Found 01 fire ext over charged and nozzles on all ext need cleaned out IAW 46 CFR 176.25-25 "Old T"

Replace worn cable indicated, used for access ramp IAW 46 CFR 176.25-1 "Old T"

Child life jackets shall be seperated from adults and their location shall be clearly marked IAW 46 CFR 180.25-10 "Old T"

Replace retro-reflective tape on PFDs identified IAW 46 CFR 180.30-1 "Old T".

Remove standing water from bilges for inspection.

All cable and wiring must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as means of support.

All vessels 26' in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment. Current system in place is not functioning as intended.

Lifeline was worn on the ring bouy attached to the float light.

Fire hose on port side, next to crew space worn. Was not tested but agreed that the material condition was in need of replacement

Navigation lighting is exposed in way of the electrical connection box. Repair the connections and sheath the wiring.

Stowage of life jackets are in a space that contains a gasoline fire/bilge pump. If a fire occurs in this space there would ne no access to these jackets. During vessels dry dock vessel is undergoing a rebuild of the crew/life jacket stowage area.

Properly route the dead wire rope used to raise and lower the vehicle ramp through the pulley. In the current arrangement the wire rope is run under the pulley and on top of the beam that supports the vehicle ramp system. This arrangement is creating signs of chaffing and could result in the failure of the vehicle ramp system.

Drills were not being logged correctly in the log book. They were being conducted but not listed properly.

Properly paint lines on the deck to indicate vehicle runways and aisle spaces in accordance with 46 CFR 185.340(a).

Properly clean all soiled/dirty gaskets and knife edges in way of all watertight hatches for the 10 void spaces on the deck of the vessel. Any worn/wasted gaskets must be replaced to the satisfaction of an attending Coast Guard Marine Inspector. Each void was found to have at least one inch or greater standing water.

Properly stow all adult and children's life jackets so that they are readily available in the event of an emergency to the satisfaction of an attending Coast Guard Marine Inspector. While making a spot check of all the life jackets, it was found that some of the jacket's straps were tangled together or caught in between the shelves. In addition there were other materials and equipment stowed

Evaluate the cause of the loss of propulsion and subsequent damage to the main diesel engine and make necessary repairs or replacement of the engine on the UTV JENNY ANN. Prior to carrying passengers, conduct an underway evaluation of the vessel's propulsion system in the presence of and to the satisfaction of an attending Coast Guard Marine Inspector.

The vessel being towed alongside by the JENNY ANN suffered a loss of steering while carrying passengers. Evaluate the cause for the steering loss, make necessary repairs to the steering system and conduct an underway evaluation of the steering/propulsion system to the satisfaction of an attending Coast Guard Marine Inspector prior to carrying passengers.

Provide documentation that Augusta Ferry, as a marine employer, is enrolled in a chemical testing program and all employees in a safety sensitive position are subject to random drug tests. All employees must be tested under pre-employment testing since the Marine Employer could not prove enrollment in a program since 2010.

Provide and post a company drug and alcohol policy statement in a common space visible for all crew members.

Demonstrate proper operation of the bilge high level alarm located in starboard void #2.

Remove the all non approved wiring in way of the bilge high level alarm and replace with approved marine grade wiring in accordance with 46 CFR 183.340(d)(1). In addition, remove all plastic wire ties that are used to secure the wiring and replace with metal supports in accordance with 46 CFR 183.340(a)(4).

Remove all unapproved wiring servicing the navigational lights and replace with with UL approved marine grade wiring in accordance with 46 CFR 183.340(d)(1). In addition, ensure the connection at the base of the navigation light and the wires leading into it are made watertight in accordance with 46 CFR 183.340(b)(3).

Provide and post "NO SMOKING" signs in various locations on the vehicle deck and ensure there is no smoking permitted while the vessel is underway transporting vehicles.

Provide and post emergency instructions in the event of fire and man overboard using 46 CFR 185.512 as a recommended format.

Provide a valve handle at the bilge manifold for service to the #1 port void discharge line and demonstrate proper operation of the same.

Provide one (01) portable bilge hand pump in accordance with table in 46 CFR 182.520(a).

Provide a battery charger that has an ammeter connected in the charging circuit in accordance with 46 CFR 183.350(f). In addition, remove the all non approved wiring in way of the battery chargerand replace with approved marine grade wiring in accordance with 46 CFR 183.340(d)(1). Properly secure the battery to prevent shifting while the vessel is underway in accordance with 46 CFR 183.350(b).

Remove all wire nuts from navigation light's electrical system to meet regs.

Small hole located in both ends of ferry under apron for loading ramp. Holes wee welded.

Three hatches/scuttles were found wasted beyond repair and need to be replaced.

Install life jack donning instructions.

Properly install cable hangers in void spaces.

Crop and renew approximately one foot by two foot by twenty foot section of starboard bilge knuckle in way of worn out bottom plating.

PLACE 6 ADDITIONAL ADULT SIZE LIFE PRESERVERS ON BOARD. UNTIL COMPLETED, THE MAXIMUM NUMBER OF ADULT PASSENGERS ALLOWED ON BOARD IS REDUCED FROM 47 TO 41.

PROVIDE DRAWING OF 110 V WIRING FROM THE TOW VESSEL TO THE BARGE DECKHOUSE THAT INDICATES THE WIRING MEETS THE REQUIREMENTS OF 46 CFR 183.340(C) AND 46 CFR 183.340(D).

INSTALL BILGE HIGH LEVEL ALARM IN VOID CONTAINING THE FIRE PUMP SEA CHEST, IN ACCORDANCE WITH 46 CFR 182.530.

Remove or properly blank off unused electrical wires in overhead of crew house.

Provide globes over exposed light bulbs in crew house.

Submit proposal and timeline for vessel to comply with 46CFR181.400 regarding fixed fire extinguishing and or detection systems. This proposalmust be submitted to Supervisor Marine Safety Detachment Cincinnati no later than April 8, 2000.

Submit electrical plans addressing new 110V wiring installed from towboat to crew house on ferry for review by Supervisor Marine Safety Detachment Cincinnati no later than April 8, 2000.

PLACE ON BOARD A COAST GUARD APPROVAED FIRST AID KIT, APPROVAL SERIES 160.41, OR A KIT WITH SIMILAR CONTENTS.

PROVIDE 3-6 FT OF FLOATABLE LINE AS A LANYARD FOR THE RING BUOY WATERLIGHT,HAVING A SNAP ATTACHED TO ONE END IN ORDER THAT THE WATERLIGHT CAN BE ATTACHED AROUND THE BODY OF THE RING BUOY ONLY WHEN NEEDED.

PLACE 1 ADDITIONAL APPROVED ADULT LIFE PRESERVER ONBOARD THE VESSEL. UNTILTHE ADDITIONAL ADULT LIFE PRESERVER IS PLACED ONBOARD, THE MAXIMUM NUMBER OF ADULT PASSENGERS ALLOWED ONBOARD IS LIMITED TO 46.

Obtain and keep onboard the current certificates of documentation of the barge OLE AUGUSTA, O.N. 253301 and tug OLE AUGUSTA, O.N. 286568.

DEMONSTRATE SATISFACTORY OPERATION OF FIXED BILGE AND FIRE FIGHTING SYSTEM.

PROVIDE AN OPERABLE COAST GUARD APPROVED WATER LIGHT FOR THE LIFE RING. THIS VESSEL IS RESTRICTED TO DAYLIGHT OPERATION UNTIL SUCH TIME THAT THE LIFE RING LIGHT IS PLACED ON BOARD.

REMOVE STANDING WATER FROM ALL VOIDS AND MAKE ALL HATCH COVERS WATERTIGHT.

REMOVE NON-MARINE APPROVED (KEROSUN) HEATER FROM THE VESSEL.

REMOVE DRUMS OF DIESEL FUEL FROM THE DECK OF THE VESSEL.

PROVIDE A WEIGHT AND STRAINER FOR THE SUCTION HOSE ON THE GASOLINE POWERED FIRE PUMP.

PROVIDE GLOBES FOR ALL 12-VOLT DECK LIGHTS.

MAKE REPAIRS AS NECESSARY TO THE CONDUIT AND WIRING IN WAY OF THE RAIL OPENING AT THE PIVOT ARM.

SERVICE ALL ONBOARD FIRE EXTINGUISHERS.

SECURELY BRACE AND SUPPORT THE VESSEL'S LANDING LIGHTS.

WIRE THE NAVIGATION LIGHTS SEPARATE FROM THE LANDING LIGHTS.

STABILIZE AND PERMANENTLY MOUNT THE NAVIGATION LIGHTS.
